The on-boarding case manager provides support to the On-Boarding Department to obtain initial approval for client's home health benefits while ensuring quality patient care. The On-Boarding Case Manager establishes and ensures continuity of client care. They are responsible for establishing and safeguarding client benefits through advocation, management, coordination, and supervision of the related clinical records and activities utilizing professional standards of care and Agency Policies & Procedures. The On-Boarding Case Manager reports directly to the Lead On-Boarding Authorization Specialist. The On-Boarding Case Manager is a vital component of the care team. They must always support and maintain a positive work environment through a personal commitment to the Company's vision and values while demonstrating and maintaining strong leadership skills. Essential Functions: Performs initial assessment of client and their home, building rapport with the client and his/her family. Provides education to client regarding benefits, utilizing appropriate team members within the company structure and/or outside agencies to accomplish desired objective. Maintains communication with client until on-boarding approved via weekly visits or phone calls, answer client questions while maintaining a friendly, positive attitude.
