    On-Boarding Case Manager JOB DESCRIPTION The On-Boarding Case Manager is the initial liaison between the Agency and new clients. The on-boarding case manager provides support to the On-Boarding Department to obtain initial approval for client's home health benefits while ensuring quality patient care. The On-Boarding Case Manager establishes and ensures continuity of client care. They are responsible for establishing and safeguarding client benefits through advocation, management, coordination, and supervision of the related clinical records and activities utilizing professional standards of care and Agency Policies & Procedures. The On-Boarding Case Manager reports directly to the Lead On-Boarding Authorization Specialist. The On-Boarding Case Manager is a vital component of the care team. They must always support and maintain a positive work environment through a personal commitment to the Company's vision and values while demonstrating and maintaining strong leadership skills. Essential Functions: Performs initial assessment of client and their home, building rapport with the client and his/her family. Provides education to client regarding benefits, utilizing appropriate team members within the company structure and/or outside agencies to accomplish desired objective. Maintains communication with client until on-boarding approved via weekly visits or phone calls, answer client questions while maintaining a friendly, positive attitude. Attends all pertinent MD appointments with the client as needed, or coordinates coverage within the team if unable to fulfill obligation. Manages care implementation with attention to quality patient care and patient family satisfaction. Coordinates care certification requirements with the On-Boarding Case Manager and other field team members and assists in ensuring ease of care transitions. Ensures proper implementation, evaluation, and oversight of all assigned clients and their Plan of Care with field staff. Works with management team to ensure that program regulatory compliance and care standards are maintained. Maintains knowledge of all relevant chapters in the EEOICP Manual and program updates and provides education to field staff when needed to establish, safeguard, and advocate for client benefits. Maintains accurate information in QuickBase for all assigned clients. Inputs updated information on all assigned clients within a timely manner. Works alongside Lead On-Boarding Specialist and Authorizations Manager to manage time effectively and schedule work load for maximum efficiency as needed and remains detail oriented to deadlines and cost efficiency. Maintains communication within the Authorization team, including a weekly update with the Lead On-Boarding Authorization Specialists, Authorizations Manger, DON/Clinical Manager, and Operations Manager. Coordinate with Quality Assurance staff in documentation development as needed to maintain documentation compliance. Demonstrates positive attitude and exhibits appropriate behaviors expected to maintain and promote a cohesive work environment. Maintains professionalism and exhibits behavior appropriate for the position. Participates in required Company in-service/education programs as scheduled. Participates in designated company committees per direction/instruction. Abides by and promotes all Agency Policies & Procedures. Demonstrates effective delegation of tasks, working with and through others as needed as directed by Lead On-Boarding Authorization Specialist. Maintains a continuing knowledge of competencies related to the Nursing profession by participating in formal education programs, conference, workshops, and professional organizations. Participates in management and leadership team discussions when directed to identify and attend to the need for reassignment of responsibilities per company development and growth. Treats company clients, physicians, and DOL MBE's with absolute customer service and professionalism. Treats current and prospective clients and families with absolute customer service and compassion with close communication, friendly service, and professionalism. Provides clinical guidance and coaching as needed for staff and client families. Available and present during assigned company office hours (unless pre-arranged with Lead On-Boarding Authorization Specialist) to enable accessibility for staff, colleagues, and clients. Notifies direct supervisor of any and all unavailability. Maintains up-to-date notes regarding client's certification process within the Agency's database.
