Job Summary
The Department of Mental Health, Substance Abuse and Developmental Services (MHSADS) is currently seeking highly qualified applicants for the position of CaseManager for the Recovery Court Program with Loudoun County.
This full-time position is a member of the CaseManagement Program and will provide targeted casemanagement services to adults participating in the Recovery Court program who are on supervised probation and meet classification for substance use dependence. Targeted CaseManagement is an intensive, community-based service occurring within the individual's natural settings including their home.
The ideal candidate must have knowledge, experience, and skills in:
Supporting individuals with substance use disorders and mental illness.
Provision of casemanagement services.
Assessing an individual's strengths and needs.
Conducting comprehensive psycho-social needs assessments.
Developing and implementing a recovery-oriented treatment plan.
Supporting individuals with substance use disorders and serious mental illness.
Facilitating intra-agency and interagency coordination.
Working as a member of a multi-disciplinary team to include law enforcement, court officials and staff, treatment providers, psychiatrists, family members and other community service providers.
Judicial process and procedures.
Linking individuals to requested services and supports while promoting individual choice.
Monitoring service delivery.
Effective oral and written communication.
Strong organizational abilities.
Proficiency in electronic health records and operating software.
Work schedule requires the flexibility to include evening hours. Office locations include the Leesburg and Eastern Loudoun Centers.
Hiring salary commensurate with experience.
Minimum Qualifications
Bachelor's degree in social work, psychology, or a related field; three (3) years related professional experience in the provision of mental health, substance use, and/or casemanagement services.
Preferred Qualifications:
Master's degree
Experience and/or background in American Society of Addiction Medicine (ASAM), Illness Management and Recovery (IMR), and Moral Reconation Therapy (MRT).
Bilingual in both Spanish and English highly desirable.
QMHP eligible.

Fundamental Requirements:
This Technical Expert Role is focused on promoting the knowledge, skills, abilities, and well-being of the child welfare workforce. The TE I must have professional experience in child welfare, including working in or with a public child welfare agency in a role associated with workforce development and support. Client Service Delivery:

Key Responsibilities:

This role involves facilitating admission intakes, treatment planning, discharge planning, obtaining collateral, assisting in the legal process, and initiating personalized care plans. The CaseManager works closely with an Interdisciplinary Treatment Team to coordinate care and discharge planning for individuals, their families, local providers, and other referral sources. This position also carries a caseload and serves as an ambassador to community partners.
* Educates and informs individuals of consents for treatment and authorizations for use and disclosures.
* Participates regularly in Interdisciplinary treatment team meetings.
* Collaborates with the Treatment Team to develop and implement an individualized person-centered Treatment Plan that is appropriate for brief intervention. Complete the signed Treatment Plan and review with each individual and any other involved parties.
* Gather collateral from the individual and involved parties to provide a thorough assessment of what an individuals' needs are for treatment and discharge.
* Actively coordinate care and provide referrals for individuals to ensure services are coordinated with both facility staff and external providers.
* Identify and address gaps in service needs for participating parties in service delivery to the individual; and make appropriate recommendations to meet individuals needs upon discharge.
* Collaborate with interdisciplinary treatment team to engage, monitor and communicate with the individual ensuring that safety is maintained while meeting the individuals' needs.
* Maintain the clinic record, including documentation of activities performed as part of the service delivery process. Obtain signatures from individuals regarding treatment, as necessary. Document all services and individuals' activities in the medical record regarding brief interventions, coordination of care, discharge planning, treatment planning, and any other pertinent information.
* Maintain a working knowledge of the Virginia Involuntary Commitment process.
* Maintain a working knowledge of the DBHDS and DMAS behavioral health system
* Participate in individual and group supervision as required. Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency. Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor.

Serving as a primary therapist/casemanager, provides a wide range of professional treatment services to diagnostically, culturally, and socio-economically diverse client populations with serious mental illness, substance use, emotional, and co-occurring disorders as well as developmental disabilities.
Some of the duties of the Behavioral Health Specialist II include behavioral health assessment, direct substance abuse and/or co-occurring mental health services, casemanagement, education, supportive counseling, groups, crisis intervention and stabilization for persons incarcerated within the Adult Detention Center.
Work hours include weekend and evening hours, as well as some holidays. Position is a 100% onsite position at the Fairfax Adult Detention Center.
This position is eligible for an additional annual environmental bonus of $5,408 for full-time positions.

Makes behavioral health diagnoses using Diagnostic and Statistical Manual of Mental Disorders, International Classification of Diseases or other diagnostic tools as required;

Provides casemanagement, outreach and engagement services as needed;
Conducts wellness and health promotion services;
Maintains all electronic health records according to federal, state and department regulations, ensuring that records of clients on their caseload are complete, accurate and comprehensive;
Advocates on behalf of clients regarding rights and needed services;
Develops and maintains cooperative relationships with representatives of community groups and public and private agencies;
Assesses and monitors the impact of medication on client's functioning and consults with medical professionals as needed;
Administers medication as ordered by a physician in accordance with department regulations;
Provides crisis stabilization and crisis management with support and supervision as needed;
Provides ongoing risk assessments for dangerousness to self and others with support and supervision as needed;
Provides entry, navigation and/or referral services for individuals, families and other concerned persons;
Prepares and administers behavioral health educational curricula;
Testifies in court to regarding client's services, progress and recommendations as required.

Employment Standards
MINIMUM QUALIFICATIONS:
Graduation from an accredited college or university with a bachelor's degree in psychology, social work, nursing, counseling, or a related field, plus 1,500 hours of clinical experience;
OR a bachelor's degree and a valid Qualified Mental Health Professional (QMHP) certification or Certified Substance Abuse Counselor (CSAC) certification plus 1,500 hours of clinical experience;
OR a master's degree in psychology, social work, nursing, counseling, or a related field plus 500 hours of clinical experience.
CERTIFICATES AND LICENSES REQUIRED:
Valid motor vehicle driver's license with fewer than six demerit points (or the equivalent in another State) maintained throughout employment with CSB
CPR certification, within 90 days of appointment and maintained throughout CSB employment
Qualified Mental Health Professional (QMHP) - Required within 1 month of hire
NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry check, driving record check, and sanction screening to the satisfaction of the employer. A TB screening upon hire and annually thereafter.
This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies).
PREFERRED QUALIFICATIONS:
At least one year of experience providing behavioral health services in community settings.
Experience with providing behavioral health services to individuals receiving treatment with Mental Health Disorders, Substance Use Disorders (SUD) and/or Co-occurring disorders (COD).
Experience with and ability to interpret and follow work processes and procedures; work well with other service agencies; and work well in a team environment.
Master's degree in psychology, social work, counseling, or related field.
Licensed or license-eligible to practice in the Commonwealth of Virginia in one of the following:
Licensed Clinical Social Worker,

Summary/Objective
Under the close supervision of a Clinical Director, the CaseManager provides direct professional social work services and casemanagement to homeless populations such as children, seniors, substance abusers, and physically or mentally ill individuals.
The Case Worker will have two areas of CaseManagement focus: Housing and Linkage with Benefits and Community Service Providers. This worker will assess client needs and strengths, create service plans and implement through referral and linkage to Agency and Community resources for entitlement programs, employment and vocational rehabilitation services, mental health services as well as to numerous other essential support services for homeless clients. Monitoring and ongoing assessment are essential components of this workers duties to assist clients to become physically and mentally healthier, housed and in safe stable environments and as independent as possible. At all times, the CaseManager will provide -customer service to both internal and external customers that meets or exceeds the standards set by the Agency.
Key Responsibilities:
Data Collection and Reporting - All pertinent data will be entered into the HMIS database and will be updated in a timely manner. Client contact sheets will be maintained listing clients served, services rendered and the nature of direct and collateral contacts. Monthly, Quarterly and Annual reports will be generated which will include but not be limited to the number of unduplicated clients, client encounters, collateral contacts, access to casemanagement, and a breakdown of the services rendered. Case load numbers, demographic information and other data is to be tracked, analyzed, and reported.
Attends staff conferences and meetings related to the progress of clients. Contributes to program evaluation and research. Prepares case histories and other reports, maintains records on social services rendered, and monitors activities of patients/clients.

Training and Experience
