Position will require a weekend rotation, usually once every 5th to 6th weekend. Major Responsibilities: Evaluates the patient's physical, perceptual, cognitive, social, and vocational abilities and their impact on the patient's activities of daily living. Conducts an occupational therapy evaluation of each patient and establishes a plan of care and treatment. Maintains and updates accurate clinical and patient records according to state and federal guidelines. Plans and implements individual treatment to re-train and teach compensatory techniques for residual disability in activities of daily living, adapted homemaking techniques, bathroom transfers, functional application of mobility skills and, when appropriate, training in adapted work methods. Discusses caseload issues and changes in case census with management. Communicates problems to physicians and management and clearly states plans, actions and goals so changes are reflected as they occur. Communicates with physicians and other team members to confirm and update patient plan of treatment and provides physician with status report on a regular basis. Manages assigned caseload, including scheduling and informing site leadership of coverage needs and ability to provide coverage. Manages the patient's medical condition including medications. Teaches appropriate treatment procedures and supportive activities to patients and caregivers, and develops plan for obtaining adaptive devices as needed. Recommends adaptive equipment and provides usage training as needed. Acts as a patient advocate to insure optimal outcomes. Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Master's Degree or Doctorate in Counseling, Psychology or Social Work Independently Licensed/In-Training Licensed as a clinical provider in the State of Wisconsin 1 or more years industry specific experience in Outpatient (OP), Intensive Outpatient (IOP), Partial Treatment (PHP), or Residential (RTC) programs for mental health treatment. Knowledge of behavioral health treatment for mood, anxiety, and eating disorders Demonstrate ability to clinical conceptualize a client's needs as it pertains to behavioral care Experience in the use of electronic health records Highly skilled in communicating ideas and instructions, verbally and in writing, clearly and concisely Knowledge, Skills, and Abilities: Which may be representative, but not all inclusive, of those commonly associated with this position. This casual part-time position is responsible for providing direct services to sexual assault victims including crisis counseling, advocacy, and support by delivering service that is culturally competent, strength-based, and client directed. The On-Call Victim Advocate provides afterhours hotline and in-person crisis response on nights, weekends, and holidays. Their priority is to provide 24/7 crisis intervention, and the position does not maintain their own caseload. This position includes the following minimum time requirements: * Completion of Advocate Training-Part 1, mandatory trainings, and approximately four in-person training sessions (approximately 40 hours) * Minimum of 40 hours per month of on-call shifts * Attend at least one staff meeting per month * Meet at least once a month with Program Manager for supervision Key Responsibilities * Provide advocacy, support, information, and referrals to victims of sexual assault victims and their families, both over the phone and in-person. * Provide options for reporting and accompany victims during law enforcement interviews and forensic evidence collection at the hospital. * Document all client activity and relay activity to the Program Supervisor by the following business day. * Assist with the community outreach and prevention education, when needed. Qualifications Education: * Required: High School Diploma Experience: * Required: 0-1 years of experience working in a social services field. * Preferred: 1-3 years of experience working with sexual assault survivors or similar experience. Skills, Competencies, and Other Requirements: * Must be at least 18 years of age. * Valid Wisconsin driver's license, with reliable personal vehicle. Maintain required auto insurance and acceptable driving record requirements. * Maintain a 30-minute response time to downtown Green Bay while on-call. * Strong interest in working with sexual assault victims both adults and children. * Ability to work under stressful situations. * Outgoing with strong communication skills. * Ability to recognize and control one's own biases.
