Program Summary
In the Adult Partial Hospitalization Treatment and Intensive Outpatient Treatment programs, we focus on creating a therapeutic environment where people can learn strategies for coping and improve their functioning. Each client's treatment plan is centered on group therapy which allows for sharing of experiences while gaining support and validation. When people need intensive, comprehensive treatment for psychiatric and chemical dependency issues but do not need inpatient or residential hospital care, Natchaug offers intensive, structured clinical treatment in programs that may include up to six hours of treatment a day, five days a week. These intensive treatment programs can help prevent the need for hospitalization. They can also help people make the transition to home after inpatient or residential treatment. Natchaug brings this clinical care closer to people's homes by offering programs throughout Eastern Connecticut.
Job Summary
Lead Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in the Outpatient department. Clinical services include individual, group and family therapy. Maintains ethical principles and professional standards of practice in adherence to the discipline's professional code of ethics. Assumes leadership responsibilities as assigned by clinical leadership. May assume clinical oversight of daily operations in the absence of Program Manager.
Key responsibilities of Lead Behavioral Health Clinician:
Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate.
Provides direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end -of-life care, referral and discharge planning.
Refers patient to appropriate levels of care and available resources that promote the individualized care plan.
Assists in the coordination of programming which may include staff scheduling, scheduling clinical activities, program development, utilization review, case load assignment, and attending meetings as directed by manager.

Qualifications

Qualifications
Master's degree in social work, or counseling, or marriage and family therapy or related Human Service field required.
Minimum of one year of experience as a licensed Clinician working with behavioral health and/or chemical dependency population.
Experience in leading clinical care teams preferred.
Current Connecticut license; LCSW, LPC, LMFT, LADC or PsyD.
Excellent communication skills, with the ability to establish relationships within and outside their entity.
Valid Driver's License.

Clinical Case Manager Job 41 miles from Holden
Responsibilities -
Schedule is 8-4:30 pm with every 4th weekend rotation.
The VNA RN Case Manager is responsible for providing skilled care and case management to patients/families under the State Nurse Practice Act and accepted professional standards of practice.
The RN Case Manager provides services utilizing nursing theory and processes, communicates/documents observations and assessments, and promotes and maintains the health of individuals through direct care, teaching, counseling, and appropriate rehabilitative measures.
This position reports to the VNA Clinical Team Manager and operates within established organizational and departmental policies and procedures.
Required Skills -
Graduate of an approved, accredited school of professional nursing required;
BSN preferred
Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required
Current American Heart Association BLS certification is required
Minimum of one (1) year of Medical/Surgical nursing experience required; home care/hospice experience preferred
Demonstrated excellent verbal and written communication skills and strong interpersonal skills required
Demonstrated organizational and time management skills required
A current driver's license, a good driving record, and reliable transportation required
