ESSENTIAL FUNCTIONS:

•Provide oversight for Registered Behavior Technician on a 1:1 basis working directly with clients - in accordance with the BACB, funding, and Center guidelines, to include documentation requirements.
•Maintain documentation of services in accordance with the funder's requirements utilizing the appropriate reporting formats and submitting within the timelines required.
•Understands and adheres to all external accreditation review standards, applicable state, local and Federal laws and/or regulations including maintaining patient confidentiality through abiding by HIPAA laws/regulations.
•Performs additional duties as assigned.

Provide information, health education, support, resources and referrals to participants to address family needs; parenting and self-advocacy skills; optimize perinatal and child health; promote child growth and development; and help families to progress toward identified goals.
Responsibilities:
* Work with a caseload of pregnant and interconceptional women, and infants/toddlers/children through five years of age and their families as part of a project effort to reduce the infant mortality and low birth weight rates as well as ethnic/racial disparities in perinatal outcomes in the Healthy Start/Empowerment Project Areas
* Plan, prepare and conduct regular home visits with participants/families engaged in the Healthy Start/Empowerment Projects. Maintain an average of 3 completed visits or attempted visits/completed visits per day
* Facilitate the identification of family goals and strategies to achieve identified goals as desired by families with appropriate referrals and connections to community resources
* Provide appropriate referrals with each family, assisting with access to community resources and collaborating with community agencies as appropriate

Heartland Bridges houses two programs, Bridges Transitional Living and Bridges Crisis Care. Transitional Living is a short-term transitional housing program enriched with supportive services designed to assist people with a diagnosed mental health or co-occurring condition while they transition to stability in the community. Crisis Care is a residential stabilization service designed for individuals who need a safe, secure environment for three to five days while stabilizing from a crisis before returning to the community where they live.
SUMMARY DESCRIPTION OF WORK
The role of the Case Manager III (Overnight) is to provide facility oversight, assist in duties to ensure the day-to-day operations of the program, and respond to client needs. Decision making, problem solving, and highly developed interpersonal skills are critical.
Compensation: between $19.74 and $23.54 per hour (wage is determined by total years of relevant experience)
Work Hours: two overnights per week (10:00 p.m. to 8:00 a.m.)
MINIMUM QUALIFICATIONS

The Refugee Wellness Program of USCRI Des Moines provides case management to refugees residing in the U.S. for less than five years who have chronic or critical physical and mental health conditions. The Case Manager will support clients with gaining access to needed medical, social, and other services, while also serving to enhance the capacity of local medical providers to serve refugee communities. POSITION REQUIREMENTS

OVERVIEW
The Refugee Wellness Program of USCRI Des Moines provides case management to refugees residing in the U.S. for less than five years who have chronic or critical physical and mental health conditions. The Case Manager will support clients with gaining access to needed medical, social, and other services, while also serving to enhance the capacity of local medical providers to serve refugee communities.
