Become the Case Manager you always wanted to be
Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans
Participate in planning for and the execution of patient discharge experience.
Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
Facilitate team conferences weekly and coordinate all treatment plan modifications.
Complete case management addendums and all required documentation.
Maintain knowledge of regulations/standards, company policies/procedures, and department operations.
Review/analyze case management reports, including Key Care Indicators, and plan appropriate actions.
Understand commercial contract levels, exclusions, payor requirements, and recertification needs.
Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs.
Meet with patient/family per Patient Arrival and Initial Visit Standard within 24 hrs. of admission.
Perform assessment of goals and complete case management addendum within 48 hours of admission.
Educate patient/family on rehabilitation and Case Manager role; establish communication plan.
Schedule and facilitate family conferences as needed.
Assist patient with timely procuring/planning of resources to avoid discharge delays or issues.
Monitor compliance with regulations for orthotics and prosthetics ordering and payment.
Make appropriate/timely referrals, including documentation to post discharge providers/physicians.
Ensure accuracy of discharge and payor-related information in the patient record.
Participate in utilization review process: data collection, trend review, and resolution actions.
Participate in case management on-call schedule as needed.
Qualifications
- License or Certification:
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACM™ certification upon entry into this position OR within two years of entry into the position.
CCM or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position.
- Minimum Qualifications:
For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred.
2 years of rehabilitation experience preferred.

POSITION SUMMARY
The Mental Health Coordinator is responsible for the development, implementation, clinical and/or administrative supervision, and monitoring of a Behavioral Health services delivery system in MA DOC Secure Treatment Units meeting NCCHC/ACA standards. The Mental Health Coordinator works with the Regional Manager, Site Administrator, Medical Director, and Psychiatrist to coordinate total contract services in MA DOC Secure Treatment Units

REQUIRED CERTIFICATIONS, EDUCATION, AND EXPERIENCES
Master's degree in Psychology, Social Work, or related field from an accredited college or university.
Licensed to practice behavioral health services in the state where services are being delivered (LICSW or LMHC required).
ESSENTIAL FUNCTION
Acts as point of contact for administrative staff for discussion of treatment unit programs and problem resolution as needed.
Coordinates development and revision of policies and procedures for the identification, assessment, and provision of required clinical services for inmates housed in Secure Treatment Units.
Supervises the planning, development and implementation of treatment programs contracted to VitalCore.
Provides clinical and/or administrative supervision to staff of Behavioral Health Services within Secure Treatment Units.
Monitors efforts of Behavioral Health Services in Secure Treatment Units to ensure clinical services are being provided in a timely, professional manner.
Provides and assigns clinical on-call duties.
Provides direct clinical services including individual and group treatment, crisis intervention, as well as providing shadowing and mentor opportunities to front-line clinical staff.
Ensures treatment programs contracted are integrated and appropriately staffed.
Provides administrative support to behavioral health staff.
Coordinates with the facility's Behavioral Health Quality Improvement Program.
Attends training and meetings as required.
Maintain proficiency with all software required for the position, and more specifically, clinical documentation in the designated electronic health record system.
Other duties as assigned.

The Behavioral Health Case Manager I - Youth & Adolescent - Northern Virginia is responsible for performing case management telephonically and/or by home visits within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs.
Primary duties may include but are not limited to:
* Uses appropriate screening criteria knowledge and clinical judgment to assess member needs.
* Conducts assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment.

* Current active unrestricted license such as RN, LCSW (as applicable by state law and scope of practice), LMHC, LICSW, LPC (as allowed by applicable state laws), LMFT, LMSW (as allowed by applicable state laws) or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required.
Preferred qualifications, skills, and experiences:
* Experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders preferred.
* Experience working with specialty populations preferred.
* Knowledge of the Virginia Foster Care system is a plus.
* Prior experience working with the Community Services Board (CSB) and/or Department of Social Services (DSS) is a plus.

Position Overview:
The Special Education Case Manager position is for 11 months. Typical working hours are Monday from 8:00 AM to 4:30 PM and Tuesday through Friday from 8:00 AM to 4:00 PM.
Reporting to the Special Education Coordinator, the Case Manager is a credentialed special education professional responsible for key functions in the following areas: administrative support and program management, record keeping and compliance, training and professional development, and delivery of specialized instruction.
Administrative Support and Management:
Manage all Special Education (SpEd) calendars in collaboration with the SpEd Coordinator, schedule meetings, and respond to inquiries related to special education services as needed or when requested by families.
Facilitate effective communication between special education staff, parents, and administrators.
Assist with the implementation of special education programs and services, including support for annual IEP meetings and related activities.
Collect and analyze data related to the effectiveness of special education programs and services.
Supervise the team of dedicated aides, including managing daily coverage schedules.
Under the guidance of the Special Education Coordinator, provide support to Resource Room staff in selecting and implementing appropriate curriculum materials.
Record Keeping and Compliance:
Maintain accurate student records, including IEPs and all related documentation.
Collaborate regularly with general education teachers and service providers to address student needs and monitor progress.
Prepare and mail dated IEP Progress Reports in alignment with the schools Progress Report and Report Card schedule.
Maintain copies of all IEP Progress Reports in individual student files.
Disseminate IEP information to general education teachers and relevant service providers (e.g., therapists, transportation staff).
Distribute IEPs to general education teachers and collect required signatures.
Send Notices of IEP Team Meetings to all invited participants at least 10 calendar days prior to the meeting date; document all responses (e.g., administrator, parent, related service providers, general education teacher, student age 16+).
Facilitate IEP team meetings to review and discuss draft IEPs.
Finalize IEP documents following team input.
Provide finalized IEP copies to parents/guardians
Ensure that record keeping and administration complies with all local and federal laws regarding students with IEPs and those referred to special education.
Organize and maintain student records (both paper and electronic) in accordance with school and OSSE standards.
Participate in student enrollment and attendance-related meetings as needed, supporting appropriate placement decisions and promoting consistent school participation for students with IEPs.
Track and manage inventory of special education materials.
Training and Professional Development:
Assist in the development and delivery of staff training on special education laws, procedures, and best practices.
Plan and facilitate Monday Early Release professional development sessions, with guidance from the Special Education Coordinator.
Specialized Instruction and Case Management
Manage a small caseload of special education students and the implementation of their IEP plans in an inclusive setting.
Provide direct and indirect services to students on assigned caseload.

The Case Manager is responsible for contributing to the development of a goal-directed, age-appropriate plan of care through an interdisciplinary team process that is prioritized and based on determined medical diagnosis, patient needs, and expected patient outcomes. The Case Manager collaborates with the interdisciplinary team to develop and document a discharge plan in the most appropriate setting to meet the patient needs while serving as an advocate to patients and families. The Case Manager ensures that activities to facilitate, broker, and coordinate the plan are being implemented, and that the plan is continuously modified based on the patient's changing needs.
Duties Include:
Negotiating services and resources when deemed needed while using creative problem solving for complex discharge planning.

Maintaining knowledge of reimbursement modalities, community resources, and clinical and legal issues that affect patients and providers of care.
Assists with the arrangement and follow-up with community resources. Case Manager will assist in providing a seamless transition utilizing the entire continuum of care and helping to facilitate patients' movement to the next level of care.
Education
Bachelor's degree in Nursing (BSN) is required.
Master's degree in Nursing (MSN) is preferred.
Graduate of an accredited school of nursing is required.
Experience
Minimum five years of professional practice experience in an acute care setting preferred.
Prior Case Management or Utilization Management preferred.
Certification & Licensure
Licensed as a Registered Nurse in the State of Virginia is required.

What You'll Do:
Staff Development and Leadership

Conduct weekly supervisions with staff that are data-informed and foster accountability with measurable goals to continuously improve performance where goals are being achieved at 90% or greater, and correct deficiencies when goals are below 89%.
Review and evaluate staff performance by completing monthly and annual case and performance reviews on each team member, setting improvement goals to ensure each individual team member meets monthly/quarterly/yearly contractual goals; develop a continuous process improvement plan and/or corrective action plans to reflect necessary contractual achievement progression
Represent Fedcap at monthly and quarterly meetings with the ability to translate information into actionable items as well as look for innovative approaches to keep Fedcap at the forefront of excellence
Collaborate with local leaders and other program staff to develop solutions and partner with other program leaders to execute process and projects for the growth and continuous improvement of the program
Design and execute a minimum of 1 solution or specific training based on funder requests, feedback from internal reviews, etc monthly to drive towards continuous process improvement
Participant and Partner Relationship-Building
Build rapport with existing participants, collect satisfaction surveys, interact with high-priority program participants, advise Case Managers on proper case handling, etc.
Build relationships and coordinate a robust schedule of rotating community presenters (schools, employers, resources), peer groups, and other two-generational workshops that occur weekly to ensure activities for program participants and are exciting, fresh, and relevant.
Responsible for execution of case coaching (model expands beyond traditional case management); while staff members are primary case load holders, there may be select individual cases assigned to the supervisor, or times where assigned caseloads need to be covered by a supervisor during times of vacancies and/or vacations.
Observe, and assist, when necessary, a minimum of 10% of participant interactions a week, including bi-weekly coaching sessions and/or weekly non-compliance/re-engagement attempt meetings, including in-office and home visits to ensure services are provided with a high level of accuracy, efficiency, and customer service

• Maintain service and support records electronically or by other means as required by Innovative Day policy;
• Retrain and counsel staff who do not complete documentation on time and in required form, and complete disciplinary action when appropriate;
• Write Court Reports that meet Innovative Day's standards, distribute them per policy, and present in Family Court as required;
• Develop SMARTER goals for inclusion in the Individual Support Plans (ISP's) for people in the assigned case load. When the ISP is being developed, if team members do not understand how SMARTER goals are developed, the Case Manager will provide technical assistance to ensure this goal standard is met;
• Review data collection on a schedule that allows for corrective action to be taken for staff if the data is not collected as planned. Reviews must take place at least weekly to ensure that there are no gaps in collection;
• Chair regularly scheduled meetings with staff to ensure training is up to date, and that any issues that have arisen in care are identified and addressed with immediacy;
• Review the MCIS (see MCIS Policy) at the start of business each day in order to timely address any issues or alerts that have been identified by external monitors;
• Daily review the electronic calendar to ensure and emails to ensure critical report dates and/or meetings are not missed, and that information is distributed and shared appropriately;
• Timely complete audits and review and distribute these audit reports per Innovative Day's Quality Assurance Plan. If deficiencies are discovered during audit, take corrective action immediately, and follow through until correction is complete;
• Produce written plans for assigned staff to cover during both planned and unplanned absences;
• Coordinate with the Quality Assurance Team generally to ensure Innovative Day meets full compliance with the Provider Certification and Department of Health Annual Licensure Survey requirements and address the areas of improvement identified during the Provider Performance Review;
• Interact professionally as an advocate, mentor, and information resource to families, external advocates, attorneys, and DDS Service Coordinators and QRS personnel; and
• Attend ISP and other meetings in support of people who receive residential services from ILS.
Requirements

Responsibilities
What You'll Do:
The Case Manager I facilitates admission intakes, treatment planning, discharge planning, obtaining collateral and assisting in the legal process and initiating personalized care for individuals who are in crisis and stabilizing from crisis. Summary: The Case Manager for the Before Thirty (B430) Program provides trauma-informed, youth-centered case management to transition-aged youth and young adults (ages 18-29) experiencing housing instability and related barriers. This role supports participants in achieving stability through individualized goal planning, housing navigation, employment and education support, and connections to community resources. The Case Manager also plays a key role in the Welcome Center by working scheduled Drop-In Center shifts, providing low-barrier support, crisis intervention, and resource navigation for walk-in youth and young adults. The position requires strong engagement skills, an understanding of youth development, and the ability to build trusting, culturally responsive relationships with diverse populations including LGBTQ+ youth and survivors of trauma. The Case Manager collaborates with internal teams, external partners, and community providers; participates in division-wide initiatives such as the annual PIT and HYC counts; and maintains accurate, timely documentation and data tracking. This role is essential in supporting Friendship Place's mission to promote stability, empowerment, and long-term success among youth experiencing housing insecurity.
Essential Duties and Responsibilities:
Case Management & Participant Support (70%)
* Provide youth-focused, trauma-informed case management to transition-aged youth (18-29), including singles and families.
* Conduct intakes, goal setting, case plans, safety plans, and ongoing assessments.
* Support participants in addressing barriers related to housing instability, mental health, employment, education, and access to benefits.
* Maintain accurate, timely documentation-including HMIS entries, case notes (submitted within 48 hours), service plans, and all required forms.
* Complete monthly reporting, dashboards, and data tracking in accordance with program and funder requirements.
* Respond to participant crises, assess safety risks, and activate appropriate emergency or mandated reporting protocols.
* Work two scheduled Drop-In Center shifts per week, providing low-barrier support, crisis response, resource navigation, and participant engagement.
Housing Navigation (15%)
* Assist participants with housing searches, applications, budgeting, and readiness tasks.
* Coordinate with leasing offices, property managers, and landlords regarding applications, inspections, W9 forms, and move-in logistics.
* Support participants in maintaining housing stability through regular follow-ups and landlord engagement.
Employment, Education & Life Skills Support (10%)
* Provide support with resumes, job searches, applications, and interview preparation.
* Connect participants to workforce partners, community resources, and training opportunities.
* Facilitate and co-lead workshops and small-group sessions focused on employment, financial literacy, wellness, and independent living skills.
Program Collaboration & Community Engagement (5%)
* Participate in CAHP Youth Outreach meetings and represent the program in community partnerships.
* Collaborate across Welcome Center programs to ensure coordinated support for participants.
* Maintain professional relationships with external providers and stakeholders.
* Participate in the annual Point-in-Time (PIT) Count and Homeless Youth Census (HYC), including training, field work, and accurate data collection.
LGBTQ+ Support & Cultural Responsiveness
* Engage in LGBTQ+-affirming practices and participate in relevant trainings.
* Serve as a member of the LGBTQ+ Workgroup when applicable.
Essential Position Requirements and Competencies
* Maintain participant safety by assessing risk, following safety protocols, and activating emergency or mandated reporting procedures when required.
* Attend and actively participate in staff meetings, case conferences, supervisions, and all required trainings.
* Maintain calendar transparency for field-based work and adhere to all communication and documentation protocols.
* Collaborate with internal teams and external partners to support coordinated services and high-quality participant care.
* Represent Friendship Place in a professional manner during community events, meetings, outreach activities, PIT/HYC counts, and partner engagements.
* Perform other duties as assigned to support program and organizational needs.
Requirements
Education/Experience
* Bachelor's degree, BA/BS in human services or social work field (preferred)
* 2+ years experience working with individuals experiencing homelessness, mental health, substance abuse, and/or vulnerable populations
* 1+ year experience working with youth and young adult population
Skills and Competencies
* Excellent time management, detail-oriented, organized, capable of maintaining confidentiality, and able to work independently with minimal supervision
* Experience with adult rehabilitation, mental health, addiction, and co-occurring disorders
* Experience working with families, youth, and school systems in culturally diverse environments with empathy and integrity.
* Must possess and be able to demonstrate strong intra- and interpersonal communication skills, with the ability to effectively express ideas, needs, and observations both verbally and in writing. This includes the ability to communicate clearly and respectfully with clients, colleagues, and supervisors, as well as to document interactions and incidents accurately, professionally, and in a timely manner.

* Proficiency in Office 365 system, specifically Word and Excel
* Must have or be able to obtain a First Aid/CPR/AED certification that includes a hands-on skills testing module. Friendship Place will provide First Aid/CPR/AED training.
* Valid driver's license and current auto insurance.
* Knowledge of youth development, trauma-informed care, harm reduction, and crisis intervention.
* Experience supporting housing navigation, employment readiness, or resource coordination.
* Ability to work effectively with diverse populations, including LGBTQ+ youth and survivors of trauma, DV, or trafficking.
* Strong engagement skills and the ability to build trusting, professional relationships with youth.
* Ability to respond calmly and appropriately in crisis situations, including de-escalation and safety assessment.
* Strong written and verbal communication skills, including timely documentation and case note completion.
* Proficiency with Microsoft Office; experience with HMIS or similar databases preferred.
* Ability to work independently, manage a caseload, and adapt in a fast-paced, low-barrier environment.
Working conditions: The position includes a combination of office-based work, field-based visits with participants and providers, and scheduled shifts in the Drop-In Center. Staff will also attend meetings, workshops, and community engagements as required. Travel within the DC Metro Area may be required for up to 25% of work time.
Physical and Environmental Requirements: The position requires the ability to work in both office and community settings, including low-barrier environments such as the Drop-In Center. Staff must be able to sit or stand for extended periods, travel via car or public transportation, and occasionally lift up to 20-25 lbs. The role involves direct engagement with walk-in youth and young adults, crisis situations, and dynamic service environments.

Specific Requirements: Positions requiring travel must have reliable access to personal transportation to fulfill job duties. The vehicle must meet all legal requirements, including valid registration and current auto insurance. Compensation: The hiring pay range is between $53,794.00 and $58,835.00 per year. The starting annual salary is based on a step system. A candidate's starting pay determination is based on meeting the basic position requirements and current completed education, credentials, and work experience.
