What you will do

Supportive Services for Veteran Families provides Rapid Rehousing services for Veterans experiencing homelessness. The focus of the program is to assist Veterans and their families in finding a permanent place to call home. The program is funded through a grant with the Veterans Administration.

Responsibilities
What You'll Do:
The CaseManager III facilitates admission intakes, treatment planning, discharge planning, obtaining collateral, clinical interventions, crisis management, assists in the legal process and initiates personalized care for individuals who are in crisis and stabilizing from crisis. Works collaboratively with the interdisciplinary treatment team to coordinate care and discharge planning for Individuals, their family members, clinics, local providers, and/or other referral sources. This position will carry a caseload.
* Conducts a thorough, accurate, clinically sound Psychosocial Assessment with the individual in a timely manner.
* Ability to review and sign off on Psychosocial Assessments completed by Certified AAC or associate level staff members, if fully licensed.
* Educates and informs individuals of consents for treatment and authorizations for use and disclosures.
* Participates regularly in Interdisciplinary Treatment Team meetings.
* Collaborates with the Treatment Team to develop and implement an individualized person-centered Treatment Plan that is appropriate for brief intervention. Complete the signed Treatment Plan and review with each individual and any other involved parties.
* Gather collateral from the individual and involved parties to provide a thorough assessment of an individuals' needs for treatment and discharge.
* Maintain a working knowledge of the Involuntary Treatment Act (ITA) and Evaluation and Treatment (E&T) Involuntary Commitment process. Monitor the petition process to ensure timeliness are adhered to, and forms are complete and thorough. When subpoenaed, attend E&T hearings.
* Actively coordinate care and provide referrals for individuals to ensure services are coordinated with both facility staff and external providers.
* Identify and address gaps in service needs for participating parties in service delivery to the individual and make appropriate recommendations to meet the individuals' needs upon discharge.

* Provides casemanagement services to promote recovery and community reintegration.
* Maintain the clinic record, including documentation of activities performed as part of the service delivery process. Obtain signatures from individuals regarding treatment, as necessary. Document all services and individuals' activities in the medical record regarding brief interventions, coordination of care, discharge planning, treatment planning, and any other pertinent information.
* Prepares clinical reports and individualized service plans utilizing agency electronic clinical documentation software. Ensures documentation complies with best clinical practices, contracts, regulatory, and other legal requirements.
* Maintain a working knowledge of the Washington State behavioral health system and potential funding sources.

What You'll Do
The SSVF (Supportive Services for Veteran Families) CaseManager will work closely with Homeless and Formerly Homeless Veterans and their Families and is part of the YWCA's Specialized and Integrated Services Division. Casemanagers will identify and engage with current and previously homeless families to assist them in obtaining and retaining permanent affordable housing. This will require establishing effective casemanagement relationships with formerly homeless veterans and homeless veteran families, building relationships with property managers and other social service providers in coordination with the SSVF Program Manager I.
This position assists in connecting families to supportive services to help ensure success in permanent housing. This position will be expected to respond to crisis situations which could result in loss of permanent housing if not resolved. This position has a caseload of families seeking housing as well as families experiencing difficulty in maintaining housing. SSVF casemanagers will provide engagement and outreach in conjunction with the SSVF Outreach Intake Specialist.

· Provide information to DFPS staff to facilitate development of the DFPS case plan.
· Update database regularly with most recent Service Management Plan of Care.
· Review information submitted by providers into the database to ensure material is appropriate and consistent with guidelines for data inclusion.
· Encourage behavioral health providers to use evidence-based practices and confirm that behavioral health providers and PCP's are sharing information as required.
· Comply with established referral, pre-certification and authorization policies, procedures and processes by related medical management staff.
· Maintain compliance with federal and state regulations and contractual agreements.
· Coordinate service management functions with other departmental functions.
· Monitor data to address trends or potential quality improvement opportunities.

Responsible for the operational delivery of the plan's casemanagement and coordination programs and processes.
