Become the CaseManager you always wanted to be
* Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
* Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans
* Participate in planning for and the execution of patient discharge experience.

* Meet with patient/family per Patient Arrival and Initial Visit Standard within 24 hrs. of admission.
* Perform assessment of goals and complete casemanagement addendum within 48 hours of admission.
* Educate patient/family on rehabilitation and CaseManager role; establish communication plan.
* Schedule and facilitate family conferences as needed.
* Assist patient with timely procuring/planning of resources to avoid discharge delays or issues.
* Monitor compliance with regulations for orthotics and prosthetics ordering and payment.
* Make appropriate/timely referrals, including documentation to post discharge providers/physicians.
* Ensure accuracy of discharge and payor-related information in the patient record.
* Participate in utilization review process: data collection, trend review, and resolution actions.
* Participate in casemanagement on-call schedule as needed.
Qualifications
* License or Certification:
* Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
* If licensure is required for one's discipline within the state, individual must hold an active license.
* Must meet eligibility requirements for CCM or ACM certification upon entry into this position OR within two years of entry into the position.
* CCM or ACM certification required OR must be obtained within two years of being placed in the CaseManager II position.
* Minimum Qualifications:
* For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
* For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred.

Education:
Tennessee Requirements: Bachelor's degree required; degree in behavioral health related area preferred. Virginia Requirements: Bachelor's degree in behavioral health related area preferred.

MAJOR DUTIES AND RESPONSIBILITIES:

2. Provide assessment, service planning, crisis assistance, and linkage, referral and advocacy to/for referred individuals.

5. Remain current in knowledge of community resources and how to access those resources.
6. Keep supervisors apprised of all critical or emergency situations.
7. Responds to emergency/crisis or walk-in situations calmly, rationally and in accordance with center policy and procedures.
8. Participate in inservice training and orientation and provide consultation and education services to the community as requested by supervisor/administration.
9. Be involved in the admission, hospital stay and discharge of individuals on caseload who are admitted to an impatient psychiatric facility.
10. Be available to consumers 24 hours a day for crisis assistance. After hours, the agency's after hours system may be utilized; however, the on-call staff as appropriate may contact the CaseManager.
11. Must be able to safely operate a motor vehicle to ensure the delivery of community based services
12. Attend and participate in regularly scheduled staff meetings and inservices and individual program planning staffings as needed.
13. All other duties as assigned.

* Current active unrestricted license such as RN, LCSW (as applicable by state law and scope of practice), LMHC, LICSW, LPC (as allowed by applicable state laws), LMFT, LMSW (as allowed by applicable state laws) or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required.
Preferred qualifications, skills, and experiences:
* Experience in casemanagement and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders preferred.
* Experience working with specialty populations preferred.
* Prior experience working with the Community Services Board (CSB) and/or Department of Social Services (DSS) is a plus.

JOB SUMMARY AND QUALIFICATIONS:
The SSVF program assists Veterans who are homeless or at-risk of homelessness end their housing crisis. The goal of the SSVF CaseManager is to assist veterans in ending their housing crisis, enhance their independent living skills by providing supportive services and education, connect them with community resources, and empower them to maintain long-term housing stability and self-sufficiency.
* This position requires a Bachelor's degree in social work or related field; at least five (5) years of experience in the field and no degree; or a Veteran with three (3) years of related work experience.
* Requires a demonstrated ability to provide a reasonable combination of the following knowledge, abilities and skills: family needs assessment, knowledge and ability to access local resources, advocacy, counseling, and working cooperatively with local service providers. Must have a commitment to the principles of the project.
* Excellent verbal and written communication skills, high degree of organization, and creative problem-solving skills.
* Must have personal automobile, valid driver's license, liability insurance, and be willing and able to travel between the counties we serve up to 70% of the time.

6. Understand the barriers that clients in both urban and rural areas face and how to best serve clients in both of these areas.
7. Comply with all policies and procedures of the program and the Council on Accreditation.

While Cedar Recovery provides care for all substance use disorders, it has a primary focus on treating opioid use disorder with medications such as Buprenorphine and/or Naltrexone, as well as Methadone (MAT).
This role will oversee the therapy staff and programming at Cedar Recovery's buprenorphine treatment Office Based Addiction Treatment (OBAT) and methadone treatment Opioid Treatment Program (OTP) facilities. Both facilities are based in Bristol, VA.

The Lead Counselor is expected to demonstrate expertise in: individual and group counseling, crisis intervention, consultation, advocacy, casemanagement, substance abuse assessments, intakes, discharge/continuing care planning, and appropriate referrals. The counselor may also perform other related tasks that are a part of Cedar Recovery's operation. The focus in this position requires knowledge of opioid addiction and direct practice in facilitating evidenced based treatment. Counselors are entrusted to maintain their expertise and continually gain knowledge via continuing education and training within the scope of opioid or substance abuse treatment.

Orient patients to the treatment program's policies and procedures.
Establish and build rapport with clients.
Conduct initial assessments and formulate individual plans of care. Includes activities related to managing and improving the care of the patient population across health, substance abuse and mental health treatment, and social service providers.
Establish SMART goals with each patient and monitor progress.
Maintain licensure/certification and complete required CEU's.
Provide referrals outside of Cedar Recovery as clinically indicated.
In collaboration with the OTP Program Director, ensure compliance with 42 cfr, accreditation guidelines, and other state and federal regulations governing OTPs.

The minimum requirements for this position are a Master's level degree individual and is licensed.

Program Overview:
In partnership with Tennessee's Department of Children's Services (DCS), Youth Villages provides family-centered, strength-based services to youth from birth to age 18 through the Family Service Worker (FSW) program.
