Job Summary
The Oasis Case Manager is responsible for providing intensive in-home case management services to parents and their youth who are at-risk of becoming further involved with the juvenile justice system due to substance use, truancy or other behavioral issues at school, mental health concerns, and parent/child conflict. The Oasis Case Manager completes assessment information, an individualized family service plan and makes referrals to community resources that will accentuate strengths as well as address needs. After hours rotation for on-call crisis and emergency situation for clients. After hours is defined from 5pm-8am Monday-Friday and all-day Saturday and Sunday.
Essential Job Functions
Assessment Activities
Assess child and family utilizing interview and standardized assessments and complete an individualized family service plan with family
Meets with family and child and gathers information from family members and collaterals for the development of the service plan
Case Management Activities
Provides case management services to the family to maintain the children in their homes
Continuously assessing progress and identifying areas of need compromising the child's ability to remain in the home
Assists family in managing crisis situations due to financial, psychological, legal, medical or any other circumstances that present a danger to youth's well-being
Community Liaison

Maintains relationships with standard referral sources and payer case managers. Contacts identified referral sources and seeks referrals as appropriate. Records outcome of calls and keeps the SAR informed.
Monitors/tracks referral sources' satisfaction levels, tracks/reports on conversion ratios and provides summary reports to management at requested intervals.
Implements/maintains, with the up line management, operational processes to ensure compliance with Company policies, requirements and regulatory mandates.
Adheres to and participates in Company's mandatory HIPAA privacy program/practices and Business Ethics and Compliance programs.
Participates in quarterly growth planning meetings/activities including discussions around staffing and recruitment needs.

Bachelor's degree from an accredited college or university with a major in gerontology, nursing, health, social work, counseling, human development, or family studies, or related area as approved by KDADS SCA/OAA Program Manager, and that individual has at least one (1) year experience in the aging and /or disability field; or

About This Role
The Case Manager supports on-campus students by offering solution-focused, non-clinical assistance for a wide range of needs. They meet with students regularly, communicate through email, and document cases. Case Managers help students identify challenges, develop effective solutions, and build skills for success. They collaborate closely with Housing and Dining Services and Student Support and Accountability staff to identify students needing support, and frequently connect students to additional campus resources for further assistance. This is a full-time, 12 month term position that reports to the Director of Housing and Residence Life.
Duties include:
Active Case Management: Provide direct support to walk-in students facing a range of challenges, including mental health or medical concerns, academic difficulties, relational conflicts, trauma, social adjustment issues, and transitions to college. Make timely referrals to appropriate campus and community resources and help students develop problem-solving and self-advocacy skills. Communicate concerning student situations to support early intervention.
Outreach: Plan, create, and deliver presentations on student life services. Participate and provide an active presence at various outreach opportunities and events. Manage, schedule, and respond to all requests for presentations.
On-Campus Housing Community Building & Relationships: Develop, coordinate, present and actively participate in educational initiatives and training opportunities. Serve as a liaison to residence hall Community Coordinators and other departmental staff within assigned residential communities.
Student Follow-Up & Assessment: Coordinate satisfaction surveys and complete final case close-outs to evaluate resolution of student concerns. Conduct semi-annual audits of student cases and contribute to regular sharing of student and program success stories.
Campus Community Office Liaison: Collaborate with students and campus partners to address concerns, provide support, and advocate for student needs across the university.

What You'll Need to Succeed
Minimum Qualifications:

We are seeking a Case Manager, Family Preservation Services to join our team. Salary Range: $44,000 - $46,000 Your role will be working with families who are in need of support and coaching to ensure that their children are able to remain safely at home. You will work in the home of the family to teach them the specific skills they need to safely remain together and connect families to community resources to help stabilize them and increase their support system.
WHAT YOU WILL DO:
* With assistance from a Family Support Worker, carry a caseload of 12-15 families.

WORK ENVIRONMENT

Crisis Clinicians provide crisis intervention, risk assessments, and supportive counseling with the goal of connecting individuals with the necessary resources and to help support the individual in their treatment and recovery.
Essential Job Responsibilities:
Conduct crisis assessments which include assessing for risk and safety, interventions, and appropriate referrals.
Work in collaboration with the individual served, family members or others they want involved in their treatment and other treatment team members (internal and external) to help support the individual in their treatment and recovery.
Provide person-centered services with a rehabilitation and recovery focus designed to promote skills for coping with and managing behavioral health symptoms.
Provide education, supportive counseling, problem solving, role modeling, referral, advocacy and other information as needed.
Coordinate and facilitate referrals to appropriate services in the community, including but not limited to behavioral health care and substance use services.
Remain prepared to quickly respond to calls at all times during scheduled shift, when not providing a televideo service.

Complete follow-up calls to provide postvention for persons served following initial service.

Attend Care and Treatment hearings for individuals referred for involuntary hospitalization.
Complete all documentation from screen accurately and before conclusion of shift. All other documentation should be completed within 24-hours including any occurrence/incident reports as necessary.
Complete and remain current in all agency and program trainings.
Credentialing requests should be completed within the first 24-hours of shift schedule.
Establish appropriate and professional boundaries with persons served and treaters. Treat all individuals with dignity and respect at all times.
Consistently demonstrate the ability to work effectively within a team as well as individually.
Attend regularly scheduled staff meetings.

Job Description
ESSENTIAL FUNCTIONS
Completes admission assessments per agency procedure for all new or returning clients.
Facilitates individual, family and group psychotherapy to children, adolescents and families. Psychotherapy processes may be provided in the office, the client home, school or community.
Provides crisis intervention with families as deemed appropriate.
Works closely with the client, family, teachers, school professionals and the treatment team to identify treatment goals, objectives and discharge criteria.
Ability to work effectively with individuals with complex needs and multi-system involvement.
Maintains regular contact with school personnel and is available for consultation regarding individual and school-based mental health issues/concerns.
Participates in the collection of mental health and student outcomes data to be used for the monitoring of the overall performance and effectiveness of services provided.
Ability to maintain a positive, solution-focused demeanor when responding to conflicts or
problems.
Meets productivity expectation of Family Service & Guidance Center.
Be familiar in the philosophy of WrapAround Services both for the purpose of assessing needs and delivering services.
Participates in weekly team meetings for the purpose of case assignments, case review, individual case planning and collaboration with team members.
Completes clinical documentation (admission assessments, progress notes, 90 day progress reviews, referrals for other services, etc.) in compliance with time lines established by Family Service & Guidance Center's Quality Assurance standards as well as those set by funding sources.
Provides clinical oversight of clinical cases and direct treatment among multiple Family Service & Guidance Center providers when assigned the role of primary provider.
Master's or Doctoral Level Psychologists may be assigned the responsibility of completing psychological testing under the supervision of the licensed psychologist coordinating assessment services.
Respect and demonstrate ability to maintain client confidentiality.
Expected to stay informed of agency developments by reading memorandums and other agency related written communication.
Act as an ambassador of Family Service & Guidance Center in the community setting. This may include doing public educational and informational presentations.
Supports and actively promotes the vision and mission of Family Service & Guidance Center and the philosophy of the strengths perspective.

POSITION QUALIFICATIONS
Education/Licensure:
Master's degree in a recognized mental health discipline.
Valid license to practice in the state of Kansas required; advanced licensure preferred; LAC also preferred.
Active membership in professional organizations strongly encouraged.
Must successfully pass the KBI, DCF Central Registry and all background investigations.
Must possess a valid driver's license, a good driving record, current auto insurance and reliable transportation.
Experience:
2+ years' experience with children and families in school based services required.
