Position Highlights:

Counsel and educate patient and families regarding health plan benefits, health plan network and other programs and tools available to help improve quality of lifestyle

Required Qualifications:
Master's degree from four-year college and/or a professional certification beyond a four-year college.

The salary range for this role is $110,000 to $166,500 annually based on full-time employment. ABOUT THE YALE NAVIGATION CENTER
The Yale Navigation Center is a 425-bed interim housing site that provides shelter and supportive services for individuals experiencing homelessness in Orange County. PATH's Interim Housing programs provide low-barrier, high-tolerance settings for program participants, referred to as guests, who are experiencing homelessness and have a variety of complex health, functional and social needs. These home-like environments are coupled with support and casemanagement services to assist guests with developing a plan to break the cycle of homelessness, stabilize in housing and reintegrate into the community.
WHAT IS A CASEMANAGER?
CaseManagers help Interim Housing guests break the cycle of homelessness by assisting them with the establishment and pursuit of a permanent housing plan through accessing and maintaining necessary support services in the community. Responsibilities of the CaseManager include:
Facilitate psychosocial rehabilitation groups for guests as required
Accepting and processing referrals from funders
Conducting intakes and needs assessments and collaborating with guests to develop Individualized Service Plans (ISP) that address barriers to obtaining services, housing, and appropriate health care and their short- and long-term goals
Meeting with established guests to support their progression towards ISP goals
Providing on-going casemanagement support for guests' ISP goals including assistance with benefits establishment, obtaining identification documents, employment linkage, housing referrals and accessing health care
Utilizing and developing community resources to broker and link guests to services
Maintaining documentation as set forth by the program contract and PATH policies
WHAT YOU BRING

Job Summary: The casemanager works to coordinate community-based services customized to individual needs. POSITION SUMMARY:
Under the supervision of the Program Director or Program Manager (or designated supervisor), the Community Supports (CS) CaseManager I, II, III is responsible for managing and overseeing all casemanagement activities for the assigned programs, including the Victim Intervention program and housing resources. o Review and ensure adherence to the scope of work requirements for Cal AIM Housing Navigation/Sustainability program, as outlined by the Program Director/Manager.
o Fulfill program or contract requirements as directed by the Program Director/Manager.
· Client Support & CaseManagement
o Provide culturally responsive, empathetic care, maintaining appropriate boundaries while serving clients in the housing navigation services, and other related social services.
o Ensure client eligibility for services and assess client needs, including those specific housing support, and broader social services.
o Deliver casemanagement services, including but not limited to intake, assessment, care planning, linkage, and coordination with community resources, with a special focus housing navigation and sustainability, and related social services.
o Assist clients in navigating housing resources, such as emergency shelters, permanent supportive housing, housing retention, and other community-based housing services.
