Position Overview
Are you passionate about creating safer communities and reducing violence? We are seeking a dedicated and compassionate Client CaseManager to join our team. In this vital role, you will focus on assessing client needs, developing customized case plans, and providing or connecting individuals to crucial resources and services,
including both perpetrators of gun violence and victim services. Our programming and services encompass a variety of support avenues, including effective emotional or trauma-related therapies, legal aid services, housing services, employment training, job placement services, family engagement, re-entry services, and much more.
As a Client CaseManager, you will work closely with individuals to understand their unique situations and develop strategies that empower them to overcome systemic challenges, such as those posed by gun violence. Responsibilities
Conduct comprehensive intake assessments to identify client needs.

Qualifications
Bachelor's degree in social work, psychology, counseling, or a related field (preferred).

Responsibilities
Complete all required documentation and data entry in accordance with agency and program standards, including but not limited to LOCUS, IM-CANS, HRA/PHA, and progress notes.
- Ensure 100% of IM-CANS are completed prior to due dates.
- Submit progress notes and other required paperwork within 48 hours of service delivery.
Provide services identified in the IM-CANS to meet consumer needs and program requirements, including:
- Maintaining required service levels and timely completion of authorizations.
- Delivering clinical services such as community support, casemanagement, therapy, and medication training.
- Supporting a clean, safe, and healthy living environment for residents.
- Providing services in the most effective setting, including consumers' apartments or community locations.
- Linking consumers to additional clinical and community resources as needed.
- Completing clinical screenings of new tenant housing referrals when assigned.
Promote the recovery model of treatment by assisting consumers in achieving self-directed recovery and incorporating trauma-informed care principles such as hope, empowerment, and respect.
Maintain positive relationships within Chestnut and with community partners including hospitals, courts, medical offices, and social service agencies to ensure coordinated, high-quality care.
Complete and maintain accurate documentation to meet housing funder requirements, including attendance and census tracking.
Provide referrals, advocacy, and linkage to essential community resources such as healthcare, food, financial aid, and housing supports.
Participate in meetings, case staffings, and program development activities. Provide backup and support to other team members as needed.
- Attend 100% of required supervision (Rule 132) if not an LPHA.
- Complete all mandatory Chestnut trainings and continuing education requirements through Relias.
Utilize evidence-based practices focused on skill-building, therapy, and recovery support for individuals, families, and groups.
Take a leadership role on assigned projects and initiatives.
Uphold Chestnut's culture of customer service excellence by adhering to organizational behavioral standards.

Qualifications
Education and Experience (one of the following):
- Bachelor's degree in a behavioral health field; or

About the position:
The Foster Care Child Welfare Specialist involves providing casework services to children and families involved with the foster care system and their foster families. They will provide reunification services and permanency planning to assigned clients

Collaborates with existing agencies and coordinates services for home providers.

Provides child care referral services to families seeking child care within Home Child Care Network.
Produces necessary reports and projections to administrative personnel within established timelines.

Qualifications

Education and Experience

The Family Planning Clinician is part of the staff team that provides reproductive health care services at the health center of assignment in accordance with Planned Parenthood of Illinois' policies, protocols, and procedures in the PPIL Manual of Standards and Guidelines.
Essential Functions:1. Understand, commit to, and practice a market and customer-oriented approach to health care delivery.2. Provide well-person care to patients who present themselves with contraceptive needs and assess physical and psychosocial status of the client.3. If privileged to do so under the direction of the Chief Medical Officer, provide colposcopy/LEEP/med AB/expanded gynecological services/in-clinic procedural abortion services; furthermore, take responsibility for the follow-up of any medical problems, abnormal lab tests, etc. consistent with PPIL protocol.4. Conduct contraceptive education and medical interviews, such as medical history, and respond to client's queries for information of a medical nature, either in person or by telephone.5. Following Medical Standards and Guidelines and protocol of Planned Parenthood of Illinois, the Clinician will be accountable for:• performing laboratory screening and microscopic examinations• conducting pelvic examinations and associated physical assessments• making appropriate referrals• providing contraceptive supplies and therapeutics and educating upon their use6.
