This role will have a focus on supporting their adult population with coordination and delivery of ongoing services.
Core Functions:
Demonstrates strong customer service within and outside the organization through consistent and thoughtful communication.
Gains and maintains a deep understanding of regulations and procedures governing extended care support to stay compliant and provide accurate guidance.

SUMMARY:
Law Enforcement Assisted Diversion (LEAD) is a harm reduction program that works to connect low-level substance users with social services to reduce future involvement with the criminal justice system and improve quality of life.
The LEAD Complex CaseManager (CCM) works directly with individuals who are struggling with substance use disorder (SUD), mental health issues, homelessness, and other challenges and provides stabilizing supports and services. The CCM meets with LEAD participants and community referrals, conducts intakes and assessments, and connects participants with available resources. The CCM develops and maintains collaborative relationships with housing providers, service providers, referral sources, including law enforcement officers, and other community organizations.
ESSENTIAL DUTIES AND RESPONSIBILITIES: Include the following. Other duties may be assigned.
Adhere to LEAD principles and policies.
Serve as a point of contact for LEAD referrals.
Respond to law enforcement for warm handoff according to LEAD policy.
Complete LEAD candidate intake and psycho-social assessment, as needed.
Provide complex casemanagement services to LEAD participants.
Develop individually tailored casemanagement plans designed to meet each participant's identified goals.
Develop and maintain client files according to agency and contract requirements.
Document all activities according to agency and contract requirements.
Document all program referrals in the casemanagement database.
Update client data in the casemanagement database.
Follow up with referral sources and document LEAD participant outcomes.
Provide street outreach to engage individuals who have been referred to LEAD.
Work with community-based referral sources to engage participants when needed.

Description of Job:
The Division of Adult Parole exists to manage and supervise inmates/parolees on a 24 hour per day, seven day per week, 365 day per year basis to provide public safety and opportunities for successful re-entry of inmates/parolees returning to the community. Re-entry support services collaborate with this setting.
This position provides comprehensive pre-release re-entry services to offenders currently incarcerated in the Colorado Department of Corrections who will be transitioning from incarceration to society.
Job duties and responsibilities include, but are not limited to:
* Conduct meetings of inmates within six months of release to identify and collect meaningful information regarding criminogenic needs through a review of formal assessments, in addition to the information contained within the offender's electronic file and any prior casemanagement notes, case plans, or progress summaries as identified per policy;
* Determine appropriate resource needs and collaboratively develop transitional release plans with the inmate;
* Monitor, evaluate and record inmate progress with respect to transitional release plans and make modifications as status changes;

Position Summary:
The BSW or MSW Intern will support adults involved in the legal system by providing short-term casemanagement, resource navigation, and emotional support. Working under a credentialed supervisor, the intern will help assess client needs, connect them with community services, and collaborate with legal teams to ensure comprehensive support. This role involves managing a small caseload, engaging clients in various settings, and building relationships with community partners. Other activities may include building community connections, assisting in developing and/or presenting trainings, program development, and evaluation.
Essential Duties:
Conduct interviews and relationship building with adult clients to assess legal, social, medical, psychological, and other needs to support their navigation of the legal system
Provide short-term advocacy and support, including referrals to community resources such as therapy, medical care, housing, public benefits, and social services
Collaborate with legal teams to identify client needs and coordinate tasks to support case resolution and resource connection

Required Qualifications:

We're hiring an Open Access Clinician to join us at our Crestone Recovery Services facility at 41 Montebello Road in Pueblo.
What You'd Be Doing
As an Open Access Clinician you would be working closely with our clients seeking services at any one of our outpatient programs to conduct assessments, and develop initial treatment plans, assist with evaluation of clients who are reporting risk for suicidal thoughts or homicidal thoughts and assist them with resources to reduce risk, and assist with casemanagement and initial enrollment paperwork when needed. You would be a valuable member of team that is one of the first individuals who are seeking services will interact with and it is crucial you provide high-quality care and compassionate responsiveness to client needs. You would represent Health Solutions amongst our community agency partners like probation, parole, and the department of social services as the first point of contact for their clients when engaging in services at Health Solutions.
Specifically, the Open Access Clinician
conducts substance abuse/mental health/co-occurring assessments for adults, adolescents and children;
provides casemanagement services;
maintains accurate, complete, and up-to-date written and electronic records;
participates in collaborative treatment planning with clients
provides exceptional clinical care to all clients
responsible for timely documentation of clinical service
maintains timely caseloadmanagement by progressing clients appropriately through treatment and discharging those who have completed treatment or are no longer engaged in a suitable manner
participates in team meetings and in case staff meetings, and consults with other team members as needed;
works collaboratively with community agencies and partners; and
performs other job-related responsibilities as assigned.

The Clinician I is responsible for performing a wide variety of patient care activities as directed by the Clinical Director. Provides excellent individual, family, and group therapy to assigned patients following program curriculum provided by Clinical Director.
Completes and ensures appropriate sign-off of all required documentation ensuring compliance with all company policies and procedures, state regulations, Joint Commission standards, and Medical Needs Criteria. This includes the following documents:
Biopsychosocial Assessment and Integrated Summary (72 hours)
Comprehensive Problem List
Integrated Master Treatment Plan and Updates (24-72 hours based on payer)
ASAMs (24 hours, based on state standards)
Individual, family, and group therapy session notes (24 hours)
Discharge Summary (72 hours)
Uses substance abuse and co-occurring therapeutic interventions and de-escalation techniques to assist patients with accomplishing all treatment plan goals and objectives and successfully complete treatment.
Assesses patient for substance use disorders, risk to self or others, and co-occurring mental health disorders.
Uses therapeutic interventions to assist patients with accomplishing all treatment plan goals and objectives and successfully complete treatment.
Manages aftercare recommendations and support that will prepare the patient for maintaining successful recovery.
Provides group therapy.
Provides caseloadmanagement as per state requirements.
Participates in crisis intervention with patients as needed.
Participates with multidisciplinary teams such as Financial CaseManagers to provide appropriate aftercare planning and securing resources.
Completes documentation daily to meet metrics of 92% completion within compliance.
