DCI Donor Services New Mexico Donor Services (NMDS) is looking for a dynamic and enthusiastic team member to join us to save lives!! This position will be the onsite Hospital CaseManager to facilitate all aspects of making organ donation happen.
SUMMARY FUNCTION:
The Hospital CaseManager is responsible for providing support for organ donation activities within the assigned facility/facilities to maximize opportunities for organ donation. Provides consistency and promotes trust in the donation process by ensuring excellent donor evaluation, management, and organ yield.
Works with donor hospital personnel, physicians, and Organ Recovery Coordinators (ORC's) or Donation Coordinators (DCs) to obtain organ and tissue authorization. Must utilize consistent communication and empathy for both the donor family and potential transplant recipients. Extensive on-call services and call duties are required. May assist with the bereavement program and provide care to both donor and non-donor families as applicable.

Key responsibilities this position will perform include:
Provides extensive on call services to obtain authorization for organ and tissue donation. On-call responsibilities may be affected by increased donor activity, staffing shortages, etc. Facilitates the authorization process for potential organ and tissue donor families prior to, during and after death declaration.
Evaluates potential donors. Assesses potential donor families, obtains appropriate authorization for donation, conducts Medical/Social History interview, and assists donor families through identification of potential end of life decisions, attending family meetings and providing donation information as needed. Responsible for medical management of donors prior to recovery activities. Directs the placement of anatomical gifts as necessary. Provides transplant surgeons with information necessary to determine appropriate recipients. Coordinates and assists in the surgical recovery of organs and perioperative management of the donor when necessary.
Provides support to Hospital Development Coordinator, in the assigned facility, to identify formal and informal leaders, assesses their respective roles, degree of influence and needs. Works collaboratively with these leaders and utilizes their expertise to improve and promote donation. Assists in policy and procedure development. Functions as an expert clinical resource for the hospital regarding organ and tissue donation.
Will visually assess donors, interpret charts, document information and communicate findings. Collaborates with hospital and medical staff to provide potential donor families with accurate and timely information regarding the patient's current clinical course. Maintains communication with hospital staff and attending physician regarding the potential donor family's understanding of the prognosis and acts as a family advocate to the health care team as necessary. In the event of neurological deterioration and/or cardiac cessation, provides education to the potential donor family to include signs and symptoms of brain death, the process of diagnosing brain death, or cardiac cessation and withdrawal of support. As appropriate, discusses organ Family Care Coordinator and tissue donation opportunities with the potential donor family with the goal being to obtain authorization for donation.
In the event the potential donor stabilizes and is no longer considered a potential organ donor, establishes an appropriate support system in collaboration with the health care staff, brings closure to the relationship with the family and returns if needed or requested.
Provides appropriate information for the bereavement program to all potential organ and tissue donor families who wish to participate. As appropriate, provides a follow-up letter to donor families. Facilitates donor family and recipient communication in accordance with company policy and procedure.
Performs other duties as assigned.
PHYSICAL TRAITS: Walks, stands and sits. Must drive to on call assignments. Communicates verbally and in writing with donor families, hospital personnel and physicians.
QUALIFICATIONS:
Education Required: RN/PA/Paramedic or related health care degree or licensure or BA/BS preferred and equivalent work experience. OPO experience.
Two to four years' Healthcare experience with families, counseling, bereavement and/or crisis intervention preferred. Acute care social worker experience strongly desired.
Licenses/ Certifications: Valid driver license with ability to pass MVR underwriting requirements.
Computer Skills: Working knowledge of computers and basic data entry skills required.

Ensuring health insurance subrogation claims are opened timely, (or getting authority to NOT open them)
Ensuring Medicare subrogation claims are open timely on the portal
Review and update/edit evaluation forms and demand packages before attorney meets with client.
Draft correspondence for attorneys, save/name all documents in SmartAdvocate in accordance with Naming Protocol.
Calendar all case information pursuant to the calendaring protocol.
Answer incoming calls via the Final Call Group
Perform backup intake calls in accordance with intake protocol.
Meet with clients during intakes, transition to litigation meetings, and settlement disbursement meetings.
Work with all staff assigned to your cases to move the case to a desired conclusion.
Keep client updated with copies of file documents.
Post all costs including expected costs that we have not been invoiced for (investigators, consultants, etc.)
Monitor dashboard Metrics/KPIs and successfully work toward keeping those metrics in top shape.
Follow and maintain all firm policies and procedures.

Incumbent will be required to work with other program staff and organizational programs in capturing and recording data and information related to elder clients, their stated needs, and in the planning and scheduling of casemanagement activities. Incumbent will also be responsible for implementing Family Caregiver activities, events, and responsibilities to ensure that both the client and the caregiver are supported. The position requires a high level of visibility within the community which requires excellent communication and interpersonal skills which are vital in maintaining a rapport with community elders, the general public, and agencies providing services. Incumbents contacts will include but are not limited to Tribal Council, Tribal Administration, Tribal Programs staff, Federal, State, and County agency representatives, elders of the community, and the general public.
Duties:
Facilitates client access to community and agency resources including but not limited to activities and services to establish social support networks, meet individual client needs to address emotional/mental health wellbeing;
Assists clients and families in developing goals and care plans in areas of identified client needs. This includes providing interpretation, guidance and preparation for assistance/support applications. Conducts regular follow up to ensure goals are met and care plans are followed.
Organize, facilitate and evaluate service delivery/care plans while conveying information and options which best meet the clients mental, physical, and emotional well-being.
In collaboration with other elderly services program staff, participates in conducting initial assessments/intake for new clients, annual 12 month and 6 month re-assessments for enrolled clients to ensure regular updates on client information and care plans.
Prepare notes for client files to ensure that delivered services and supports are properly documented and recorded.
Provide direct supervision to the Data Entry Clerk/Outreach Worker position to ensure compliance with organizational policies and procedures and to ensure that staff member is sufficiently fulling the duties and responsibilities of their position.
Enter service data into the SAMS/Harmony data base to ensure all services data is entered properly. Review data of entered by the Outreach Worker/Data Entry Clerk to ensure the accuracy of the entered units of service.
Conduct regular in-person congregate setting and home visits with clients to ensure their needs are being met, to ensure proper communication, and to ensure that they feel safe and supported.
Advocate for elder clients to ensure they receive the appropriate services and support.
Educate elder clients and their families on available resources, services, and support options. Assist clients in making informed decisions through advocacy and information.
Documents all client encounters, and contacts made on behalf the client(s) and program to ensure proper documentation of case related information.
Maintain comprehensive client files which includes management of formal and confidential documents held for safe keeping on behalf of the client. Respond to information access and retrieval requests from authorized individuals following all stated confidentiality guidelines.
In collaboration with other program staff, incumbent will plan and organize promotional activities to include but not limited to public education, group presentations, health fair participation, and development and dissemination of flyers, brochures, and newsletters.
Determines client needs through target assessments and works with program staff and other resources to plan/coordinate care and activities in an effort to improve the quality of life for elder clients.
Work with resource agencies on coordinated service provisions to promote the expansion of existing programs for social support, and educational activities for all enrolled clients and their families.
Manage the public relations for the Elder Services Program through the development of fliers, social media posts, and program brochures/newsletters. Provide up to date program and service information to all Homebound and Congregate elder clients.
Develop and maintain automated data bases, summary reports, and graphics using various software packages in support of Program activities, operations and grant proposals.
Attend meetings, trainings, and sessions concerning client services and other federal, state, or county meetings relating to the Elderly Services Program as directed. May be required to attend evening and/or weekend meetings.
Assist in scheduling appointments for clients, and assist program staff with follow-up cases to ensure that care plans are carried out and regularly evaluated. Interpret, support, and advocate for elders.
Participate in all emergency drills and environmental safety activities.
Attend and participate in grantee-sponsored trainings, program pre-service and in-service trainings, and other continuing education, career and professional development opportunities.
Participate in the program's self-evaluation process and grantee monitoring visits and comply with any applicable Program Improvement Plans that are developed.
Participate in general staff meetings and other meetings, conferences, and events planned by the grantees, and the program director.
Assist the Activities Planner as directed by the Program Director, in providing transportation and physical assistance services to elders for special functions and activities held off site.

Under close supervision, this position provides support and structure to residents in developing program plans by managing a caseload of client cases and administering the policies and procedures of the program as directed by the contracting agency and the Facility Director. Primary Duties and Responsibilities:
May perform the duties of a Monitor including resident intakes, check-ins, breath analysis and drug screens, to provide backup when necessary.
Prepares written personalized programs for each resident to establish measurable criteria of expected behavior and accomplishments and a time frame for achieving specified goals. Reviews plan with residents on a weekly basis and document progress or revisions so that they are gaining the life skills necessary to make a successful re-entry into the community.
Assists residents to establish personal budget, locate suitable housing, enroll in educational and vocational programs and participate in family and community activities, as appropriate, to ensure that they are working towards financial independence.
Monitors facility sanitation and safety. Assigns daily housekeeping chores to residents to comply with facility standards.
May orient residents upon arrival at facility to provide smooth assimilation into program including having them complete paperwork, take facility tours, or introduce to other residents.
Make entries into confidential resident files to provide the structure for case analysis and program planning and to provide documentation of program participation. Ensure files are secure to protect resident confidentiality.

DUTIES/RESPONSIBILITIES
Be familiar with each case in your caseload.
Posting all case information and case dates in SmartAdvocate.
When necessary, help with property damage claims
Speak to adjusters
Ensuring health insurance subrogation claims are opened timely, (or getting authority to NOT open them)
Ensuring Medicare subrogation claims are open timely on the portal
Review and update/edit evaluation forms and demand packages before attorney meets with client.
Draft correspondence for attorneys, save/name all documents in SmartAdvocate in accordance with Naming Protocol.
Calendar all case information pursuant to the calendaring protocol.
Answer incoming calls via the Final Call Group
Perform backup intake calls in accordance with intake protocol.
Meet with clients during intakes, transition to litigation meetings, and settlement disbursement meetings.
Work with all staff assigned to your cases to move the case to a desired conclusion.
Keep client updated with copies of file documents.
Post all costs including expected costs that we have not been invoiced for (investigators, consultants, etc.)
Monitor dashboard Metrics/KPIs and successfully work toward keeping those metrics in top shape.
Follow and maintain all firm policies and procedures.
Requirements
Minimum 2 years personal Injury casemanagement experience (Legal Assistant and medical field/behavioral health experience will not be considered)
Strong understanding of state, local, and federal court rules.

Essential Duties and Responsibilities:
* Adherence to the attendance and punctuality policies of Goodwill.
* Exhibit excellent customer service skills as related to your position.
* Contacts participants and provides casemanagement services to assigned caseload as required by the program guidelines and scope of work.
* Travel as required based on caseload, scope of work.
* Orientates participant to GINM policies and procedures including rights and responsibilities of the person served, GINM's mission, programs and services, grievance procedures and confidentiality policies and procedures.
* Provides casemanagement for participants; refers to other support services as needed, such as medical evaluation and treatment, and social services.
* Builds and maintains collaborative relationships with community agencies.
* Maintains case records containing pertinent, accurate and current information; documents all contact made with each participant in a timely manner.
* Completes all required documentation within designated timeframes including but not limited to case notes, program documentation and required reporting.
* Review and submit program documents such as participant budgets, reimbursements and other required program documentation as outlined in the program contract.
* Completes, verifies and submits monthly billing according to established timeframes.
* Ensures accurate caseload by closing all inactive cases on a monthly basis.
* Attends staff meetings; provides updates on persons served with complex issues and receives new referrals.
* Maintains standards and practice in accordance with applicable laws, regulations and requirements, as well as professional standards; complies with contract guidelines and regulations.
* Enhances professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops.
* Ensures HIPAA compliance.
* Responsible for the safety of all persons served under his/her supervision.
* Implements CARF standards into all programs and follows professional ethics as set in the Code of Conduct/Business Ethics policy.
* Maintains confidentiality of all privileged information.
* Performs other incidental and related duties as required and assigned.

Minimum Qualifications: The requirements listed below are representative of the knowledge, skills, and or abilities required for the position. Individuals must be able to pass a criminal background check and drug test. Individuals must have a valid New Mexico driver's license, liability insurance, insurability under the agency insurance carrier, dependable vehicle and a clean driving record.
* Bachelor's Degree in social services or related human services field, preferred. (Other educational requirements may be required by funding source).
* One to three years' work experience in casemanagement or social work is preferred.
* CCM, CRC or MSW preferred.
* Ability to acquire CPR and First Aid certification.
* Bilingual preferred.
