Essential Duties and Responsibilities Conduct comprehensive intake and needs assessments to identify client strengths, barriers, and service priorities. Provide linkage, referral, and coordination to community resources, including housing, employment, medical care, transportation, and social services. Support clients in obtaining and maintaining public benefits (e.g., SNAP, Medicaid, SSI/SSDI). Facilitate and document care coordination among multidisciplinary teams (counselors, peer specialists, prescribers, and outside providers). Provide crisis intervention, advocacy, and supportive counseling within scope of practice. Maintain accurate, timely documentation in compliance with Medicaid and SUPR billing standards (e.g., T1016, T1017, H0006). Participate in case staffings, supervision, and quality assurance activities. Educate clients on self-advocacy, health literacy, and relapse prevention supports. Maintain strict confidentiality under HIPAA and 42 CFR Part 2. Promote recovery principles: hope, choice, empowerment, and connection. Minimum Qualifications Bachelors degree preferably in social work, psychology, human services, or related field preferred. Minimum of one (1) year of experience in behavioral health, recovery support, or social service coordination. Knowledge of SUPR and Medicaid case management standards. Must possess strong organizational and interpersonal skills. Valid Illinois drivers license and reliable transportation required for community visits. Must pass background check per Illinois DHS and agency policy. Knowledge, Skills, and Abilities Familiarity with trauma-informed, strengths-based, and recovery-oriented care. Working knowledge of Illinois community and state resources. Excellent verbal and written communication. Ability to manage multiple cases, prioritize needs, and meet documentation timelines. Proficient in basic computer and electronic health record (EHR) systems. Demonstrated empathy, professionalism, and cultural competence. Preferred Qualifications Qualified as a QMHP, MHP, or RS-CADC under Illinois Medicaid standards. Experience in SUPR-licensed programs, housing navigation, or reentry case management. Knowledge of ASAM criteria, harm-reduction, and motivational interviewing. Bilingual (English/Spanish or English/ASL) preferred. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Work with CP guests in developing and executing an action plan/Life Recovery Plan. Assist and identify any potential participants to that could benefit from CP internal services and transitional housing opportunities. Assist guests in transitioning from homelessness or transitional housing to their own housing Coordinate and maintain positive working relationships with landlords and potential landlords Perform Case Work with Guests in the following areas: Intakes - Perform a comprehensive psychosocial assessment that addresses the patient's physical and psychosocial health, environment, support systems, educational needs, and financial resources. Assessments - determine areas of highest need. Action Plans - determine goals with each resident that are tailored to his/her identified needs and help them to implement an Action Plan. Referrals & linkage - refer to outside agencies as appropriate and provide advocacy as needed to ensure interagency collaboration. Follow-up - confirm implementation of Action Plan steps by the resident and referral agencies. Review Life Recovery plan with Guests periodically to determine progress and/or need for modifications in the Plan. Resource investigation and crisis intervention as required. Maintain documentation of interactions, advocacy, referrals, goals and outcomes and maintain accurate statistical and client records. Act as liaison between court services and clients when appropriate. Maintain files and all required data as determined by HUD and Carpenter's Place. Provide community education where appropriate. Develop and maintain current information on availability of referral sources and linkages. Develop a supportive, professional relationship with guests. Provide transportation to guests to community services as needed. The main role of the Case Manager is to ensure that each child is safe and cared for, and is reunited with a suitable parent, guardian, or sponsor in the most expeditious manner possible. This position requires the consistent exercise of discretion and judgment. ESSENTIAL FUNCTIONS: Ensure safety for all UC within each home; Coordinate and handle an estimated case load of 8-10 at any given time; Coordinate initial intake for each UC; Schedule/confirm each UC has an initial medical evaluation within 48 hours of arrival; Coordinate and assist the Clinician to assure the admission assessment and family tree are completed within 3 days of arrival; Coordinate and assist Clinician to assure the psychosocial summary and individualized Service Plan (ISP) are completed within 7 days of arrival; Manage reunification process for each UC; Keep information updated in UC portal; Coordinate the discharge and release of child to their sponsor; Respond to client emergencies within or outside work hours; Attends a minimum of 40 hours advanced training/workshops on an annual basis; Other duties as assigned.
