Serve as a liaison with the VA to coordinate services for veterans enrolled in the GPD CaseManagement program.
Collaborate with other MACV staff and partner agencies, participate in veteran case conferences, and ensure the necessary documentation and HMIS data collection.
Partner with MACV staff to submit monthly, quarterly, and annual grant reports.
Conduct outreach services, maintain veteran caseloads, and verify veteran eligibility.
Address all barriers to obtaining and retaining housing, such as chronic homelessness, mental health concerns, substance use disorders, under-employment, lack of income, interpersonal skills, meals, tenant responsibilities, and other obstacles.
Problem-solve urgent situations, including interactions with EMT and Fire, police, psychiatric services, and other community agencies and staff.
Prevent and/or resolve conflicts using a strengths-based approach.
Offer supportive counseling, guidance, and referrals to community resources.
Engage regularly with individuals to ensure program participation and progress.
Assist program participants in housing placement, education on living skills, navigating resources and referrals, eviction diversion, and addressing physical and mental health issues.
Communicate effectively, both in writing and verbally, with team members, supervisors, volunteers, and program participants.
Maintain timely and comprehensive case notes, documenting relevant client interactions.
Enter, update, and confirm information in the Homeless Management Information System (HMIS) and other databases as required.
Attend weekly program staff meetings, monthly all-staff meetings, weekly supervision with the Program Manager, and training sessions.
Report program, client, and facility needs to the appropriate individuals.

Requirements:

JOB SUMMARY
The Mental Health CaseManager will be embedded in one of Catholic Charities' permanent supportive housing program and will be assigned to manage a case load of clients with mental health needs. ESSENTIAL FUNCTIONS
Conduct individual assessments, assessing functional strengths, resources and needs.
Develop individualized, person-centered, trauma informed community support plans.
Implement interventions identified in support plan, support client with referrals to external providers, maintain connection with external providers and provide case coordination support.
Develop therapeutic relationship with team and client. Supports and advocates on behalf of client in the procurement of services and benefits entitlement, assisting with required paperwork as necessary.
Maintains complete case documentation and records, as required by Catholic Charities, the funders, or licensure/regulatory bodies. Ensure confidentiality of all records and interactions.
Coordinate services both internally and externally including county casemanager, family/guardian of client, work personnel, medical personnel, etc.
Attend supervision, case consultation meetings and participate in team-based decisions regarding all clients and services.
Maintain client files, update client files, update treatment plans a minimum of once every six months and do all required documentation as specified in DHS guidelines, utilizing professional writing skills.
Responsible for reviewing and ensuring the relevance, accuracy, and timeliness of data and reporting for purposes of accountability and client outcomes.
Provide casemanagement to support housing retention, including supporting timely rent payment, renewal of benefits, maintaining a habitable housing unit, etc.

Summary:
The Children's Mental Health Targeted CaseManager provides strengths-based, culturally responsive case-management services to children and youth diagnosed with a Serious Emotional Disturbance (SED). The role supports families in accessing mental health, educational, medical, social, and community resources. This position focuses on building stability at home, school, and in the community through assessment, coordination, advocacy, and ongoing monitoring.
Essential Duties and Responsibilities:
Carry a caseload of 7-8 children/families
Conduct initial and ongoing functional assessments for children with SED.
Review diagnostic assessments and gather relevant background information.
Determine program eligibility and complete all intake paperwork and releases.
Develop an Individual Family Community Support Plan (IFCSP) within regulatory timelines.
Identify mental health, developmental, educational, social, and environmental needs.
Collaborate with the child, family, and treatment providers to establish measurable goals.
Connect families to appropriate services, including therapy, psychiatry, skills work, educational supports (IEP/504), social services, housing supports, and community programs.
Facilitate communication between the family, school, county, and other providers.
Support families navigating system complexities (insurance, benefits, school meetings, etc.).
Maintain regular contact with clients (often monthly minimum) through home, school, office, or community visits.
Track progress toward goals and adjust service plans as needed.
Ensure services are delivered in accordance with the treatment plan and state guidelines.
Advocate for the child's needs within schools, healthcare systems, and community settings.
Support families in understanding their rights and accessing culturally responsive services.
Assist with crisis planning and connect families to crisis resources when needed.
Support transitions such as hospital discharge, residential treatment return, or placement changes.
Maintain accurate and timely case notes, assessments, IFCSPs, and billing documentation.
Meet all state, county, and agency requirements for CMH-TCM services.
Participate in team meetings, case consultations, and required training.
Other related duties may be assigned.
Qualifications/Experience:
Spanish speaking preferred.
Bachelor's degree in Behavioral Science, Social Work, Psychology, Sociology, or a related field.
Experience working with children or youth with mental health or behavioral needs.
Strong communication skills and ability to collaborate across systems.
Valid driver's license and reliable transportation for community-based work.

JOB SUMMARY
The Mental Health CaseManager will be embedded in one of Catholic Charities' permanent supportive housing program and will be assigned to manage a case load of clients with mental health needs. The Mental Health CaseManager will report to the Program Manager with oversight from the Director of Behavioral Health. The practitioner will provide services that are rehabilitative and enable the client to develop and enhance psychiatric stability, personal and emotional adjustment, social competencies, community skills and independent living skills. The practitioner will support the client in strengthening skill areas needed according to their treatment plan. Catholic Charities provides services to clients in Hennepin or Ramsey County.
ESSENTIAL FUNCTIONS
Conduct individual assessments, assessing functional strengths, resources and needs.
Develop individualized, person-centered, trauma informed community support plans.
Implement interventions identified in support plan, support client with referrals to external providers, maintain connection with external providers and provide case coordination support.
Develop therapeutic relationship with team and client. Supports and advocates on behalf of client in the procurement of services and benefits entitlement, assisting with required paperwork as necessary.
Maintains complete case documentation and records, as required by Catholic Charities, the funders, or licensure/regulatory bodies. Ensure confidentiality of all records and interactions.
Coordinate services both internally and externally including county casemanager, family/guardian of client, work personnel, medical personnel, etc.
Attend supervision, case consultation meetings and participate in team-based decisions regarding all clients and services.
Maintain client files, update client files, update treatment plans a minimum of once every six months and do all required documentation as specified in DHS guidelines, utilizing professional writing skills.
Responsible for reviewing and ensuring the relevance, accuracy, and timeliness of data and reporting for purposes of accountability and client outcomes.
Provide casemanagement to support housing retention, including supporting timely rent payment, renewal of benefits, maintaining a habitable housing unit, etc.
May transport clients

Summary:
The Children's Mental Health Targeted CaseManager provides strengths-based, culturally responsive case-management services to children and youth diagnosed with a Serious Emotional Disturbance (SED). The role supports families in accessing mental health, educational, medical, social, and community resources. This position focuses on building stability at home, school, and in the community through assessment, coordination, advocacy, and ongoing monitoring.
Essential Duties and Responsibilities:
Carry a caseload of 7-8 children/families
Conduct initial and ongoing functional assessments for children with SED.
Review diagnostic assessments and gather relevant background information.
Determine program eligibility and complete all intake paperwork and releases.
Develop an Individual Family Community Support Plan (IFCSP) within regulatory timelines.
Identify mental health, developmental, educational, social, and environmental needs.
Collaborate with the child, family, and treatment providers to establish measurable goals.
Connect families to appropriate services, including therapy, psychiatry, skills work, educational supports (IEP/504), social services, housing supports, and community programs.
Facilitate communication between the family, school, county, and other providers.
Support families navigating system complexities (insurance, benefits, school meetings, etc.).
Maintain regular contact with clients (often monthly minimum) through home, school, office, or community visits.
Track progress toward goals and adjust service plans as needed.
Ensure services are delivered in accordance with the treatment plan and state guidelines.
Advocate for the child's needs within schools, healthcare systems, and community settings.
Support families in understanding their rights and accessing culturally responsive services.
Assist with crisis planning and connect families to crisis resources when needed.
Support transitions such as hospital discharge, residential treatment return, or placement changes.
Maintain accurate and timely case notes, assessments, IFCSPs, and billing documentation.
Meet all state, county, and agency requirements for CMH-TCM services.
Participate in team meetings, case consultations, and required training.
Other related duties may be assigned.
Qualifications/Experience:
Spanish speaking preferred.
Bachelor's degree in Behavioral Science, Social Work, Psychology, Sociology, or a related field.
Experience working with children or youth with mental health or behavioral needs.
Strong communication skills and ability to collaborate across systems.
Valid driver's license and reliable transportation for community-based work.
Ability to work with diverse families and maintain professional boundaries.
Experience with electronic health records or case-management documentation.
Bilingual or lived experience supporting underrepresented communities.
Work Environment & Expectations
Combination of office, home, school, and community-based visits.
Flexible schedule to accommodate family availability (some evenings may be required).

What will you do?
The Mental Health casemanager will offer direct assistance to a diverse range of individuals, fostering trusting relationships while being attuned to their experiences and traumas. Regular interaction with clients is expected, occurring at least once a month or as required. Over time, this position will promote personal development and skill enhancement while aiding in the growth of our company's division.
To be considered for this opportunity you will bring your skills and talent, bachelor's or master's degree in human services or related areas or qualify as a Mental Health Practitioner. Individuals with a social work degree must have valid and current Social Work licensure. At least one year of professional or internship experience in the mental health field required. Must have and maintain a valid Minnesota Driver's License, maintain automobile coverage and have access to an automobile to attend meetings in the office and community (CaseManagers are reimbursed for mileage.)
