About the Role
BNI Treatment Centers is seeking a dedicated Clinical Counselor to join our team. In this role, you will collaborate with our clinical staff to support the treatment and growth of adolescents in our care. You will play a key role in supervising clients, facilitating clinical activities, and helping shape a constructive therapeutic community.
Responsibilities
Provide direct care and supervision to clients while maintaining the structure of the facility.
Conduct one-to-one counseling sessions and support clients with individual challenges.
Monitor, record, and communicate client progress; identify when additional services may be needed.
Facilitate therapeutic groups, workshops, social/recreational activities, and outings.
Utilize appropriate interventions, including de-escalation and crisis response strategies.
Establish positive and professional relationships with adolescents while maintaining healthy boundaries.
Prepare clinical documentation and communicate findings with the clinical team in a timely manner.
Support clients in self-administering medication in accordance with policies.
Participate in weekly team meetings, monthly drills, and ongoing staff development.
Ensure safety, cleanliness, and adherence to facility policies, including house runs and overnight checks when scheduled.
Provide transportation for approved clients in company vehicles.
Qualifications
High School Diploma or higher education required.
Counselor Certification preferred but not required.
Experience working with high-risk adolescents and families strongly preferred.
Strong communication, documentation, and decision-making skills.
Ability to work effectively as part of a multi-disciplinary team.
Valid California driver's license required.
Must meet federal, state, and local background clearance requirements.
If in recovery, at least one year of sobriety/clean time is required.
Knowledge & Abilities
Understanding of clinical documentation standards and treatment planning.
Familiarity with adolescent development, therapeutic communities, relapse prevention, and recovery models.
Ability to coordinate activities, model positive behavior, and maintain professionalism at all times.

The People Concern's Santa Monica C3 Team is seeking a case manager to join our multidisciplinary street outreach team. This team is part of a City of Santa Monica strategy to combat homelessness. The goal of Santa Monica C3 is to identify, engage, and connect or re-connect homeless individuals to interim and/or permanent housing and supportive services. The team works closely with each client to develop individualized service plans. The team helps enroll clients in public benefits (i.e. General Relief, SSI/SSDI, Medi-Cal), submit housing applications, and access appropriate medical and behavioral health treatment. Essential Duties and Responsibilities:
Work as part of an interdisciplinary team to engage and provide field-based services, meeting currently homeless clients where they are in order to eliminate barriers to services.
Interface and represent the agency with partners, including community health providers, the business community, and social service/housing providers to reduce the number of homeless people living on the streets.
Engage clients in developing mutually agreed-upon goals documented in each client's confidential service plan.
Provide initial contact, needs assessment, information, education and referrals to the homeless.
Refer target clientele to appropriate services (e.g., health, mental health, interim housing, etc.).
Work in collaboration with other team members to remove barriers to permanent housing; e.g., acquisition of identification, legal aid, benefits assistance, life skills acquisition.

* Assists students with navigating the health care system for mental and physical health resources.
* Tracks and follows up with students for a 2-4 week (typically) period of time after providing them with appropriate resources in order to ensure continuity of care, and to ensure that people get setup with resources and do not end up "falling through the cracks"
* Work with the Moorpark College SHC medical/mental health teams to obtain release of information, and all necessary records, documentation and treatment plans in the aftermath of a student psychiatric hospitalization.
* Works with the Moorpark College SHC medical/mental health teams to help students with complex mental health needs connect with off campus providers of service.
* Provides limited direct crisis management services to students in distress.
* Researches national trends, implements safety, and support programs for students.
* Coordinates with student service programs and any other relevant collaterals on campus to ensure effective programs are in place that support student success.
* Collaborates with the Basic Needs Center providing assessment of need and assistance for foster youth and homeless students for on and off campus resources as needed e.g. mental health care, food pantries, housing, showers, and financial assistance. This will involve creating a screening tool to determine student need.
* Collaborates with mental health providers to organize trainings and informational sessions for students/employees with community mental health resources (i.e. Coalition for Family Harmony, RISE program, Transitional Aged Youth (TAY)).
* Maintains weekly scheduled appointments and documentation in electronic records systems. Ensures student contact documentation is ethical, accurate, and timely.
* Communicates subject matter clearly, correctly, effectively and maintains required office hours.
* Adapts methodologies for students with special needs and different learning styles.

Project 180 is a leader in the forensic treatment space, offering high-quality, specialized treatment programs that serve thousands of justice-involved individuals. Our programs span criminal justice intercepts and include pre-arrest interventions, court-based diversions, jail in-reach, and intensive post-release services. Project 180 operates in partnership with County agencies including the Department of Mental Health (DMH), the Department of Health Services (DHS), and the Justice, Care and Opportunities Department (JCOD).
The Program
The Rapid Diversion Program (RDP) is a County-operated program that diverts criminal defendants out of the criminal legal system at the early stages of the court process. The program provides in-court mental health evaluation, linkage to treatment services, and case management support to participants, whose criminal case is dismissed upon successful program completion.
The Position
Under the supervision of the Program Supervisor, the Rapid Diversion Program (RDP) Case Manager is responsible for providing wraparound, field based case management and rehabilitative services to a caseload of approximately 20 diversion clients. A successful RDP case manager will foster strong collaborative relationships with external treatment providers and RDP court staff. Success in this role hinges on actively engaging clients, supporting them in achieving meaningful recovery goals, and enhancing their utilization of community resources. Job title (I-III) to be established based on years of relevant experience.
Essential Functions
Provide wraparound, field-based services to support recovery goals
Actively engage clients in services through the development of strong rapport, consistent contact and use of motivational interviewing. Provide outreach services as needed, meeting clients “where they are at” (service location, client's point of view).
Services to target cornerstones of recovery oriented care (instilling hope, empowerment, self-responsibility and engaging in meaningful roles). Ensure service delivery is consistent with ethical standards and a harm reduction, strength based, trauma informed framework.
Develop and maintain knowledge of available community resources. Complete needs assessment and link clients to community resources. Monitor ongoing participation in services/use of resources and facilitate problem solving to target barriers. Accompany clients to linkage appointments when appropriate.
Teach prosocial and recovery skills (i.e. communication, problem solving, anger management skills) for long term stability in the community.
Engage and join with collaterals (family members, court) as appropriate. Provide education related to mental health recovery to better support clients. Engage in client advocacy.
Conduct custody pick ups from LA county jails.
Support clients in successfully adhering to court requirements
Support new clients in understanding the court process, appropriate court conduct, and their specific responsibilities for diversion.
Assist clients in attending scheduled court appearances, providing transportation and accompaniment as necessary.
Procure written progress reports from community providers and submit them to court within established deadlines.
Cultivate and maintain strong relationships with RDP court staff, engaging in ongoing collaboration to ensure clients receive high-quality services consistent with clients responsibilities for diversion.

Under the supervision of the Program Manager, the Interim Housing Case Manager will provide on-site services to homeless individuals who are enrolled in our Encampment Resolution Interim Housing Program. This program offers short-term interim housing combined with linkages to community resources and housing to ensure a stable housing outcome.
ESSENTIAL DUTIES & RESPONSIBILITIES
The Case Manager's key areas of responsibility include:

Ensure the safety of all residents by implementing, monitoring and enforcing the rules and regulations of communal living.
Ensure that participants day-to day living needs are being provided.
Develop and implement an individualized service plan with clients that supports housing goals, increases income and mainstream benefits, supports employment opportunities, and addresses mental health, physical health and substance use needs, relapse prevention, and any other risk factors that could impede accessing and/or maintaining permanent housing.
Support clients as they navigate the housing process, including, but not limited to, housing location, landlord advocacy, rental application process, credit repair support, move-in support, and budgeting.
Provide ongoing case management services that will ensure successful permanent housing, decrease social isolation and reduce relapse risks, advocacy to deter evictions, and linkage to any needed services such as medical, dental, mental health, substance use, life skills training, money management, meaningful community activities, volunteer services, job placement, etc. in tandem with on-site team.
Utilize a trauma informed, harm reduction, and housing first model when working with clients.
Maintain high-quality case notes and enter HMIS within 24 hours.
Complete all forms required by Interim Housing and referring agencies.
Obtain and retain all required documentation (e.g., ID, income verification) as required by the program.
Participate in all meetings with collaborative agencies and other community meetings as directed.
Complete all requests for reporting in a timely manner.

Knowledge, Skills & Abilities:

Demonstrate experience in interviewing clients, making appropriate referrals and performing crisis intervention. Previous experience with Crisis Housing and/or rapid rehousing programs or other similar programs preferred.
Demonstrate knowledge of substance abuse programs, homeless programs, and mental health services within the Los Angeles Continuum of Care. Candidates must have demonstrated knowledge of case management principles and intervention techniques specific to hard-to-reach, difficult to serve populations.
Requires an ability to work flexible hours including some early mornings, evenings. A bilingual/bicultural background is a plus.

Education:
