Department/Unit: Neurology General Work Shift: Day (United States of America) Salary Range: $65,102.17 - $100,908.37 We have an exciting opportunity for a Case Manager to join our multidisciplinary Team at the Albany Med Center of Excellence for Alzheimer's Disease to make a significant regional impact on delivering high quality specialized care for patients with Alzheimer's disease and other related dementias.
The Case Manager with oversight by the Program Manager of the Center of Excellence for Alzheimer's Disease (CEAD) will administer the grant deliverables and program evaluation components of the CEAD, as specified in the contract with the NYS Department of Health (DOH).
Essential Duties and Responsibilities include:
* Works with the Center's physicians and other key personnel to operationalize grant deliverables, orchestrate change as indicated and ensure continuous quality improvement.
* In conjunction with Center RN, participates in patient visits. Responsible for reporting the patient history and complaints to the physician.
* Maintains confidentiality and adhere to HIPAA regulations.
* Administers baseline and routine cognitive testing (MMSE, MoCA) as well as ADL/IADL, CDR neuropsychological inventory, AD8.
* Provides counseling and case management services to patients and/or care partners to help them understand and accept their diagnosis and cope with the emotional stress and manage the day-to-day services.
* Facilitates community service referrals if/when needed and complete appropriate paperwork that is requested for services.
* Attends community meetings to strengthen collaborative partnerships and identify groups for potential trainings.
* Development of a targeted outreach plan reaching primary care providers/medical specialty physicians to highlight the importance and benefits of early diagnosis and the role of the CEAD as a resource to the 10 counties of our grant catchment area.
* Development of a targeted resource guide for caregivers on community-based providers/services for patient and family referrals in each of the 10 counties of our grant catchment area.
* In conjunction with the Program Director, Program Manager and Center physicians, the Case Manager will participate in and conduct training programs for medical professionals that will include information about the diagnosis, treatment and care management of individuals with AD/D, the importance of early detection, management of behavioral issues, need to address palliative care and ethical issues facing someone with dementia and the importance of screening individuals over the age of 65 for cognitive impairment as a component of regular care using a validated cognitive screening tool.
* As assigned by Program Manager, represents the CEAD at meetings, conferences and other events as required. Interface with any/all community agencies, which may serve to benefit the Center/CEAD or its constituency.
* In conjunction with Program Manager, assists with maintaining consistent follow-up with primary care providers/medical specialty physicians, non-physician health care providers and college and universities (for student trainings) to strengthen relationships.
* In conjunction with Program Manager, develop and maintain relationships with key personnel from Albany Med to identify training opportunities for Albany Med physicians and non-physician health care providers

OVERVIEW
The Case Manager for the Integration Program will provide support to recently arrived refugees via long-term case management beyond the initial resettlement period to aid clients in achieving all aspects of self-sufficiency, including health and medical assistance, employment, and English language instruction. Position Summary
QCH is seeking a qualified Case Management Supervisor (CMS) to lead a team of five Case Managers and oversee the screening and intake process for potential clients in our Older Adult Services program. Case Management Services help older adults with functional impairments access the services, benefits, and supports they need to safely age at home and maintain their quality of life.
The CMS will supervise staff, develop training plans, ensure completion of all required Department for the Aging (DFTA) and agency-wide trainings, and advocate for clients while maintaining high-quality case management standards.
Key Responsibilities
Provide ongoing supervision, guidance, and support to Case Managers, including staff evaluations and professional development.
Screen inquiries to determine eligibility for case management; provide information and referrals when appropriate.
Complete intakes within 3 business days and submit for assignment.
Ensure all required forms and documentation are completed accurately.
Transfer referrals via the Stars Rating System (STARS) for meal authorization and verify receipt with Home Delivered Meals (HMDL).
Advocate and intervene on behalf of clients with other agencies.
Accompany Case Managers on home visits as needed.
Perform other duties as assigned.
Qualifications
MSW or related Master's degree (social work, public administration, nursing, or public health); LMSW a plus
3-5 years of experience in aging services and at least 3 years of supervisory experience
Cultural competency and sensitivity to the needs of older adults, including those with visual or hearing impairments
Ability to multi-task, handle sensitive information professionally, and maintain attention to detail
Proficiency with computer applications; knowledge of STARS database a plus
Strong interpersonal, communication, and customer service skills; ability to exercise tact, diplomacy, and poise
Mandarin and/or Cantonese language skills are a plus

The Case Manager with oversight by the Program Manager of the Center of Excellence for Alzheimer's Disease (CEAD) will administer the grant deliverables and program evaluation components of the CEAD, as specified in the contract with the NYS Department of Health (DOH).
Essential Duties and Responsibilities include:
Works with the Center's physicians and other key personnel to operationalize grant deliverables, orchestrate change as indicated and ensure continuous quality improvement.
In conjunction with Center RN, participates in patient visits. Responsible for reporting the patient history and complaints to the physician.
Maintains confidentiality and adhere to HIPAA regulations.
Administers baseline and routine cognitive testing (MMSE, MoCA) as well as ADL/IADL, CDR neuropsychological inventory, AD8.

Facilitates community service referrals if/when needed and complete appropriate paperwork that is requested for services.

In conjunction with the Program Director, Program Manager and Center physicians, the Case Manager will participate in and conduct training programs for medical professionals that will include information about the diagnosis, treatment and care management of individuals with AD/D, the importance of early detection, management of behavioral issues, need to address palliative care and ethical issues facing someone with dementia and the importance of screening individuals over the age of 65 for cognitive impairment as a component of regular care using a validated cognitive screening tool.
As assigned by Program Manager, represents the CEAD at meetings, conferences and other events as required. Interface with any/all community agencies, which may serve to benefit the Center/CEAD or its constituency.
In conjunction with Program Manager, assists with maintaining consistent follow-up with primary care providers/medical specialty physicians, non-physician health care providers and college and universities (for student trainings) to strengthen relationships.

What your day might look like:
Provides direct clinical treatment including individual, family, and group therapies, and crisis intervention toward maximizing the clinical response of each person seen;
Conducts initial evaluations and identifies critical impairments that become the focus of treatment form effective planning and implementation of treatment;
Provides unscheduled and or emergent services as needed for walk-in, open-access, and crisis clients at any time during the scheduled shift;
Provides services beyond scheduled shift as required to meet the needs of acute walk-in, open access, and/or crisis clients;
Additional duties as assigned.
Your skills and qualifications:
Master's degree in Mental Health Counseling or Social Work is required.
Experience providing direct clinical service to the population served by the assigned clinical program is strongly preferred.
Knowledge of substance abuse issues including features of alcohol and various drug abuse signs and symptoms, intervention strategies, awareness of associated medical concerns. • Individual, family, and group therapy skills.
Broad clinical knowledge of psychiatric disorders, and psychiatric diagnosis.
Valid NYS driver's license and reliable transportation required for local and regional travel
Ability to work onsite
