Duties Include:
Will act as a liaison between our Residents and the Director of Case Management
Address Resident concerns, which may include; roommate issues, room changes, behavioral problems, etc.
Help all new Residents in orienting to the Assisted Living Program
Assist Residents in obtaining SSI/SSD benefits and Access-A-Ride applications & services
Coordinate medical appointments; including scheduling transportation, insurance eligibility, etc.
Assist with the documentation necessary for re-admissions from hospitalization or rehabilitation
Maintain organized incident reports and proper documentation as necessary
Order durable medical equipment based on physician orders
Collaborate with the finance office to assist with Medicaid recertification or SSA concerns
Ensure completion of Mental Health evaluations and coordination of services as needed
Act as a resident spokesperson when necessary
Requirements:
Work experience in human services, social services or other related fields strongly preferred
Proficiency in computer programs such as MS Office & MS Excel
Strong work ethic, intense drive, and initiative for quality customer service
Excellent written & oral communication skills and great problem-solving skills

Adult Protective Case Manager

The primary role of the Adult Protective Case Manager is to manage the services of individuals per the Oneida County's Department of Social Services Adult Protective qualifier listed below and client needs within the Rep Payee Program itself.
Individuals unable to meet their essential needs for food, shelter, clothing or medical care, secure entitlements due them or protect themselves from physical or mental injury, neglect, maltreatment or financial exploitation; are in need of protection from actual or threatened harm, neglect or inaction of themselves or other individuals; and have no one readily available who is willing and able to assist them responsibly. This will also consist of referrals to the Rescue Mission of Utica's Representative Payee services when deemed appropriate.
PRIMARY DUTIES:
Conduct face-to-face and home visits to all clients to assess client safety, risk, and to determine client needs at least once per month.
Screen program referrals from OCDFCS on the date of referral.
Meet with OCDFCS staff member and client within three (3) days of referral.
Determine client current needs for day-to-day living (i.e. food, clothing, housing, medical care, and social supports) and assist clients in meeting those needs.
Meet with OCDFCS to discuss client status and progress on a monthly basis, or more as requested.
Provide monthly status reports of all clients to include financial and personal situation details.
Maintain accurate client files and provide progress notes to OCDFCS for their permanent case record. The notes shall be updated and provided no later than ten (10) days from an added event.
Contact OCDFCS of any changes in client's personal situation, which may require additional intervention on their part.
Maintain accurate mileage log related to home visits.
Provide to the client networking resources for additional needed services.
Report critical client concerns to Program Manager and/or administration staff as directed in a timely manner.
Prepare Social Security or other reports as assigned specific to client caseload.
Prepare additional paperwork for other networked agencies as necessary.
Meet with Rep-Payee Program Manager weekly to discuss client issues, provide status updates and other concerns and upcoming client developments
Attend and participate in regular departmental meetings and staff meetings as requested by supervisor.

MINIMUM REQUIREMENTS:
Bachelor's degree in Human Services, Social Work, Sociology, Psychology, Child Development or equivalent required
Minimum (3) years of experience in Human Services with a public or private agency.
Experience with one-to-one interaction with client with identification of client's needs and goals for the development of a service plan.
Intermediate skills with Microsoft Word, Excel and Outlook.
Possession of a valid NYS driver's license and own transportation required

As part of the Patient Service Team the Program Case Manager will work closely with the Medical Team to provide excellent patient-centered care to address social determinants of health which affect health, functioning and quality of life outcomes, working with patients enrolled in the program reduce health inequities by increasing access and acceptability of HIV primary care, sexual health, behavioral health and supportive services.
The primary responsibility of this position is to provide supportive services to patients enrolled in the Center for Young Adults NYS Department of Health AIDS Institute Grant-Based Program. The Program Case Manager will work with patients enrolled in the program to provide resources and referrals to address health disparities and inequities, as well as assists with case finding, and supports outreach and program promotion.
Key Responsibilities:
* Complete social determinants of health assessments, and risk assessments with all enrolled patients and document an individualized plan with patients to address identified issues and provide follow-up assistance in reaching goals.
* Work as part an interdisciplinary team to provide LGBTQ affirming, culturally appropriate case management services to adolescents and adults.
* Participate in all required and approved trainings.

The Case Managers will assist individuals with identifying barriers to housing stability and engage in goal planning that aims to link unsheltered homeless and vulnerable clients to the resources and skills that they need to achieve temporary housing and long-term housing stability. The Case Manager will provide crisis resolution to include triage based on urgency, personal safety and de-escalation of conflicts.
Essential Job Functions:
Provide case management services to assist and engage clients to obtain housing stability, obtain health insurance, mainstream benefits, addiction services, mental health services, obtain earned or other income, and so that clients can sustain housing.
Ability to triage clients based on urgency and personal safety of clients
Ability to assess and target resources and supports for clients
Assess client level of need to provide the appropriate level and length of assistance.
Refer clients to services based on assessment.
Take the lead in the de-escalation of conflicts
Participate in case conferencing with project partners as needed
Function as a contributing member of a multidisciplinary team with the organization's employees, staff, volunteers, and community agencies
Assure all relevant information pertaining to clients is disseminated to all appropriate staff members in a timely fashion
Maintain accurate client files and notes including entering data into HMIS and other electronic records
Represent the Mission in a professional manner at all times
Practice effective time management and communication skills
Demonstrate positive role modeling for clients and contribute to the health, well-being and comfort of clients
Transport or arrange for transportation as needed
Participate in staff meetings and trainings as directed by supervisor
Other related duties as assigned
MINIMUM JOB QUALIFICATIONS
High School Diploma or GED
AAS Degree in Human Services or related field preferred
Experience working with special populations in the area of recovery, homelessness and mental illness preferred
Working knowledge of Microsoft Word, Excel and Outlook
Clean/valid NYS Driver's License Required

The Supportive Housing Case Manager assists program clients in securing and maintaining stable housing which meets required habitability standards. This is done through assessing client needs, referring to additional services, and through the direct service provision of Tenancy Support Services.
The Supportive Housing Case Manager position is also responsible for scheduling and conducting assessments, providing referrals, advocacy and direction, coordination of care with all agencies involved in care including the treatment team, ensuring compliance with state, federal and Agency regulations and operating standards, completing all records and reports, participating in Agency activities and providing transportation as required.
CORE RESPONSIBILITIES
Assist individuals in all activities related to locating and securing housing.
Case Manager will meet with clients face to face at least monthly and will participate in home visits.
Develops an individualized plan with the client to ensure the individual remains housed.
Schedule and conduct assessments, evaluate needs, and develop treatment planning processes for adults.
Develop and maintain an effective communication network with the client, treatment team staff, families, community agencies and other agency departments.
Maintain regular contact with client's landlord regarding needs of client or landlord and ensuring lease stipulations are met by both parties.
Provide referrals for clients and/or their families to appropriate services.
Provide advocacy, direction and support to clients and/or their families as needed.
Complete all required documentation according to program regulations/guidelines.
QUALIFICATIONS
High school diploma required.
5 years of related experience working with persons with Mental Health and Substance Use Diagnosis.
Must have a valid NYS Driver's License.
