New Season
For over 30 years, New Season Treatment Centers have been a leading national health care service provider of outpatient treatment centers that specialize in providing safe, quality and best in class care for individuals living with Opioid Use Disorder ("OUD").
Operating in over 70 treatment centers in multiple states, our team members are engaged in medication-assisted treatment, counseling, support, and care management of individuals living with OUD. We treat the whole person and, in so doing, address the underlying causes of OUD in an effort to provide a continuum of care that not only addresses treatment needs but supports the patient on their journey to recovery.
Job Summary:
New Season operates a number of nationally recognized and accredited methadone treatment centers dedicated to helping individuals with prescription drug abuse and/or opiate addiction regain control of their lives.

Essential Functions:
Works with patients to complete all intakes, admissions, discharges, and transfer paperwork.
Documents patient progress through counseling and interaction through groups.
Completes patient psychosocial and an individualized treatment plan within the required time frame.
Identifies any clinical/casemanagement needs and works to address those needs.
Performs individual, group, and family counseling as required.
Performs at least 20 hours of direct one-on-one patient contact per week through individual or group counseling.
Reports patient abuse, neglect and exploitation as required.
Reports patient grievances as required.
Educates patient in all aspects of treatment, corresponding health issues and steps to recovery.
Obtains Urine Drug Screens and initial patient photo identification.
Assists in monitoring all patient activities on center premises.
Actively participates in community relations activities as directed and authorized.
Ensures the reading and understanding of the Policy and Procedures Manual.
Ensures compliance with 42 CFR Part 2 (Federal Confidentiality Regulations) and 45 CFR, Parts 160 & 164 (HIPAA)
Is responsible for obtaining or maintaining proper licensure and/or certification according to specific state requirements.
Actively participates in preparation for surveys and inspections conducted by CARF, the State, DEA, Board of Pharmacy and any other agency site visits as dictated by the state laws or regulations.
Participates in all staff meetings
Ensures compliance with local, State, Federal and Colonial Management Group, LP rules, regulations and policies.
Always acts in the best interests of the program and company; honors, supports, and protects the proprietary data and rights of the company.
Performs other tasks as assigned by clinic, region or corporate leadership.
Minimum Qualifications:
Education/Licensure/Certification:
Experience in substance abuse field is not required, but preferred
Qualified candidates will have a counseling Certification in a related field (State Specific)
Bachelor's degree is preferred but not required
Experience Required:
Minimum of 500 hours of experience in substance abuse is preferred

The CaseManager plays a vital role in supporting clients by assessing needs, developing individualized service plans, and coordinating care across multiple systems. This position involves advocacy, assisting with aftercare services post inpatient hospitalization, and connecting clients to community-based resources to promote stability and well-being. The CaseManager provides comprehensive services for individuals navigating mental health challenges, substance use disorders, intellectual disabilities, and complex social service systems such as: SSDI, Housing, Medicaid, and Medicare.
Key Responsibilities
Client Support & Advocacy
Conduct IM CANS assessments every 6 months.
Develop individualized treatment plans addressing social determinants of health, financial, mental/medical health, and legal needs.
Review and update treatment plans every 90 days to reflect client progress or changes.
Advocate for clients to access healthcare, financial, housing, and social services.
Care Coordination
Assist clients with applications for IM-CANS, SSDI, LIHEAP, Childcare programs, and other assistance programs.
Serve as a liaison with community agencies, healthcare providers both internal and external.
Arrange transportation for clients to essential services and appointments as needed.
Documentation & Compliance
Maintain accurate, confidential case files, documenting all client interactions, assessments, and follow-ups.
Ensure compliance with organizational policies, state regulations, and legal requirements.
Update IM CANS assessments every 6 months to ensure awareness of current client needs.
Update treatment plans every 90 days to reflect evolving client needs.
Qualifications
Education
Minimum of High school diploma required.
Experience
2 or more years of experience in casemanagement, crisis intervention, healthcare, and/or social services.
Experience with IM-CANS, SSDI applications, and acute mental illness preferred.
Skills & Abilities
Strong organizational skills and attention to detail.
Excellent communication and interpersonal skills, with the ability to build rapport and advocate for clients effectively.
Ability to handle sensitive and confidential information with professionalism and discretion.
Strong electronic documentation skills required (EHRs, Microsoft Teams, Excel, and SharePoint).

Plan the well intervention activities and review and validation when completed, and for providing solution for wells with integrity issues with support of well completion specialist.
Be responsible for routine well Intervention, well diagnostics and proficient in downhole remediation practices and capable for preparing field operational procedures.

Ensure departmental compliance with mandated regulations and standards through constant monitoring and support for ongoing and future operations and make sure well barriers requirements are maintained in all types of intervention.

You will have 10-15 years' experience in rigless well intervention operations that includes design, planning and execution of wireline, slickline and coiled tubing operation in critical sour gas and preferably unconventional operational environment.

Summary: The Workforce Group, a LEMOINE company, is seeking an experienced CaseManagers supporting the Administration for Children and Families (ACF), Office of Human Services Emergency Preparedness and Response (OHSEPR) under the U.S. Repatriation CaseManagement and Financial Support Services Program. CaseManagers will provide direct support ensuring safe reception, service coordination, and access to critical resources.
As part of the ACF/OHSEPR Repatriation CaseManagement and Financial Support Services Program, this position may require travel and is contingent on contract award, with salary subject to change.
Location: Remote with occasional travel, must be local to Chicago, IL
Duties and Responsibilities:
Provide end -to -end casemanagement support, including intake, needs assessment, service coordination, and case closure in accordance with OHSEPR policy.
Meet repatriates at ports of entry or final destinations (as directed), ensuring safe reception and transition to temporary housing, medical, or other support services.
Coordinate with federal, state, and local partners, as well as community service providers, to connect repatriates with appropriate resources (housing, food, medical care, transportation, etc.).
Support complex cases involving vulnerable populations such as unaccompanied minors, individuals with medical or mental health needs, or other at -risk groups.
Document casemanagement activities, assessments, and referrals in OHSEPR's casemanagement system within established timelines.
Monitor client progress during the 90 -day temporary assistance period, conducting regular outreach and providing updates to OHSEPR.
Prepare case notes, reports, and correspondence that are clear, concise, accurate, and compliant with program requirements.
Participate in ongoing training, program improvement initiatives, and quality assurance activities.
Adhere to confidentiality, HIPAA, and federal data security standards.
Perform other job -related duties as assigned.
Qualifications, Knowledge, Skills and Abilities: Bachelor's degree in Social Work, Psychology, Human Services or a related field. Minimum 3 years of casemanagement experience in human services or related fields. Previous experience with ACF and OHSEPR, emergency response, refugee resettlement, or repatriation programs, is highly preferred.
Experience providing direct client services and conducting needs assessments.
Knowledge of social service resources and referral processes.
Familiarity with federally funded programs such as TANF, SNAP, Medicaid, and Vocational Rehabilitation Services preferred.
Strong interviewing, documentation, and case reporting skills required.
Ability to work effectively with diverse and vulnerable populations is required.
Proficiency in casemanagement databases (e.g., Homeless Management Information System) or ability to learn within 30 days.
Knowledge of Section 508 compliance and experience preparing accessible documentation preferred.
Proficiency in MS Word, Excel, PowerPoint, and Teams.
Bilingual or multilingual skills (Spanish and other common languages) are highly preferred.

In addition, the Community Support Team CaseManager will:
Must work closely with State Operated Mental Health Facility to plan for services needed for high rate users referred because they are to be discharged from inpatient care
Must work closely with community support treatment team triage
Must develop plans to refer, link, and place individuals with community based treatment and other needed resources
Must refer, link, and place individuals with recreation, transportation, financial and medical entitlement, vocational rehabilitation services, as appropriate and as needed
Must coordinate inter- and intra-agency services
Must conduct outreach and case findings
Must determine eligibility and conduct comprehensive assessment of service needs
Must develop the comprehensive master Individual Treatment Plan, with the input of the client customer and/or family unit

Job Summary The Youth In College CaseManager at La Casa Norte is responsible for providing comprehensive academic and casemanagement support to youth who are experiencing homelessness or housing instability and are enrolled in college. The role involves creating individualized academic plans, connecting youth with necessary resources, and acting as a mentor and advocate to ensure their academic success and overall well-being. The ideal candidate will be enthusiastic to help students, knowledgeable of the college process, and serve as a positive role model for the students. Key Responsibilities: CaseManagement:
