Job Summary The Department of Mental Health, Substance Abuse and Developmental Services (MHSADS) is currently seeking highly qualified applicants for the position of Case Manager for the Recovery Court Program with Loudoun County. This full-time position is a member of the Case Management Program and will provide targeted case management services to adults participating in the Recovery Court program who are on supervised probation and meet classification for substance use dependence. Targeted Case Management is an intensive, community-based service occurring within the individual's natural settings including their home. The ideal candidate must have knowledge, experience, and skills in: Supporting individuals with substance use disorders and mental illness. Provision of case management services. Assessing an individual's strengths and needs. Conducting comprehensive psycho-social needs assessments. Developing and implementing a recovery-oriented treatment plan. Supporting individuals with substance use disorders and serious mental illness. Facilitating intra-agency and interagency coordination. Working as a member of a multi-disciplinary team to include law enforcement, court officials and staff, treatment providers, psychiatrists, family members and other community service providers. Judicial process and procedures. Linking individuals to requested services and supports while promoting individual choice. Monitoring service delivery. Effective oral and written communication. Strong organizational abilities. Proficiency in electronic health records and operating software. Work schedule requires the flexibility to include evening hours. Office locations include the Leesburg and Eastern Loudoun Centers. Hiring salary commensurate with experience. Minimum Qualifications Bachelor's degree in social work, psychology, or a related field; three (3) years related professional experience in the provision of mental health, substance use, and/or case management services. Preferred Qualifications: Master's degree Experience and/or background in American Society of Addiction Medicine (ASAM), Illness Management and Recovery (IMR), and Moral Reconation Therapy (MRT). Bilingual in both Spanish and English highly desirable. QMHP eligible. Definition: Under the supervision of the Senior Manager of Case Management Services, the Case Manager provides community-based case management to adults and children with acquired brain injury and their families in the Northern Virginia region. The adult brain injury program includes long-term case management services and public awareness. These services are provided using a community-based case management model. Distinguishing Characteristics: This position requires a professional level of intervention with complex cases. This includes participating in the discharge planning teams, when possible, at childrens facilities and rehabilitation centers nearby (e.g., Cumberland Hospital in New Kent, Kluge Childrens Rehab Center in Charlottesville, NRHs Pediatric Program in Washington, Kennedy-Krieger Institute in Baltimore). Determine when the maximal level of functioning has been achieved and outside services, such as life skills training/tutoring, need to fade. Completes all paperwork as described in the Policy and Procedure Manual and always maintains charts orderly. Establish a solid therapeutic relationship with clients, their families, and their community providers. Supports clients in developing their own goals and plans based on the needs assessment and utilizing various planning tools. Completes an Individualized Service Plan and provides a written copy to the client. Review the ISP every 90 and adhere to the MCOs rules and regulations about service review. Clients signatures are required to ensure the clients consent and participation in services. Identify, coordinate, and monitor goal-directed community services to meet the clients needs and desired activities as outlined in their plans. In addition, to recognize and reinforce the importance of professional development and enhance future recruitment of licensed professionals, if approved, the licensure supervisors will receive a stipend of $1,500 paid out biweekly over a 12-month period whether for one or more Department of Health licensure candidates. Maintains a diverse caseload of clients with a variety of substance use, developmental disabilities and/or mental illnesses; Develops client and/or family centered therapeutic treatment plans, with consideration given to the nature and severity of the behavioral health problem, family, cultural and employment related circumstances, legal involvement, individual and family histories, physical condition, and other pertinent factors; Implements therapeutic treatment plan under clinical supervision; Conducts independent behavioral health intakes and assessments; Provides individual, group, and/or family treatment under supervision; Collaborates with service professionals inside and outside of the Community Services Board; Adapts and modifies treatment interventions as indicated; Provides case management, outreach and engagement services as needed; Conducts wellness and health promotion services; Maintains all electronic health records according to federal, state and department regulations, ensuring that records of clients on their caseload are complete, accurate and comprehensive; Advocates on behalf of clients regarding rights and needed services; Develops and maintains cooperative relationships with representatives of community groups and public and private agencies; Assesses and monitors the impact of medication on client's functioning and consults with medical professionals as needed; Administers medication as ordered by a physician in accordance with department regulations; Provides crisis stabilization and crisis management with support and supervision as needed; Provides ongoing risk assessments for dangerousness to self and others with support and supervision as needed; Provides entry, navigation and/or referral services for individuals, families and other concerned persons; Prepares and administers behavioral health educational curricula; Testifies in court to regarding client's services, progress and recommendations as required. Knowledge of the principles, theories, and methods of behavioral health services and social development of the individual; Knowledge of evidence based behavioral health treatment methods; Knowledge of psychological, physiological and psychiatric terminology, theories and practices; Knowledge of assessment processes, methods of gathering social history and interviewing techniques; Knowledge of existing public and private agencies and community resources; Ability to formulate diagnoses and appropriate treatment plan; Ability to perform behavioral health and risk assessments and apply evidence based/informed treatment approaches/modalities; Ability to manage a caseload; Ability to establish rapport and maintain effective relationships with clients; Ability to maintain effective working relationships with co-workers, public and private organizations, community groups, and the general public; Ability to communicate effectively, both orally and in writing; Ability to maintain records and prepare reports. Employment Standards MINIMUM QUALIFICATIONS: Graduation from an accredited college or university with a bachelor's degree in psychology, social work, nursing, counseling, or a related field, plus 1,500 hours of clinical experience; OR a bachelor's degree and a valid Qualified Mental Health Professional (QMHP) certification or Certified Substance Abuse Counselor (CSAC) certification plus 1,500 hours of clinical experience; OR a master's degree in psychology, social work, nursing, counseling, or a related field plus 500 hours of clinical experience. CERTIFICATES AND LICENSES REQUIRED: MANDT Certifications within three months of appointment and annually thereafter; CPR certification, within 90 days of appointment and maintained throughout CSB employment; Medication Administration Assistance Certification within 90 days of appointment; Valid Motor Vehicle Driver's license with fewer than six demerit points (or the equivalent in another State) maintained throughout employment with CSB; Qualified Mental Health Professional (QMHP) - within 1 month of hire. NECESSARY SPECIAL REQUIREMENTS: The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry check, a driving record check, and sanction screening to the satisfaction of the employer. A TB screening upon hire and annually thereafter. This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies.) PREFERRED QUALIFICATIONS: Master's degree in psychology, social work, or closely related field. Experience working with individuals with serious mental illness. Experience working with individuals in their homes or in a residential setting. Professional (QMHP) certification Licensed or receiving supervision towards licensure in the Commonwealth of Virginia in the Licensed Clinical Social Worker, or Licensed Professional Counselor. PHYSICAL REQUIREMENTS: Ability to respond quickly to crisis, physically support clients, walk stairs, work with clients teaching housekeeping/cooking/shopping skills. Ability to walk/bend/carry items and lift light to moderate loads. Ability to independently drive vehicles to travel to various sites providing services to clients, transport clients/observe client appearance/hear conversations in a group.
