Multi-state licensure required based on geographical territory. Preferred Experience Two years related experience working in a clinical healthcare setting or previous experience working in a clinical liaison role preferred. Demonstrated competency with the preadmission process including assessments and relationship building in a rehabilitation setting or in another post-acute care setting preferred.     The RN/LPN Extended Hours Essential Duties Are: Plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing. What You Should Know About the RN/LPN Extended Hours Role: This is a DRIVING position throughout Newark This is a Full-Time position: 8p-8a 3 days a week and every other week. Holiday rotation also required. We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed. RN/LPN Extended Hours Qualifications: Graduate of an accredited nursing school; Bachelor of Science in Nursing (BSN) preferred. Active Registered Nurse license in the State of Ohio without any board actions. Two years acute care nursing experience preferred. CPR Certified Certification in Hospice & Palliative Medicine (CHPN) preferred and will be made available to Ohio's Hospice employees who wish to obtain certification. Computer skills sufficient to properly document services and care. Ability to drive during daytime, nighttime, or inclement weather. Valid Driver's License with Safe Driving Record State Minimum Automobile Insurance Coverage Must be able to pass a criminal background check Must be able to pass a 10 panel drug screen Benefits & Perks: your health and happiness matters! Ensure the treatment plan of the physician or Medical Director is being followed. Giving and interpreting Mantoux Testing. Makes appointments for clients to see physician, provides lab work and referrals for additional follow up care. Assists in training staff in medical and health and safety matters as well as assisting in properly maintaining medical equipment. Provide training for staff development specific to health care needs i.e., CPR, First Aid, Blood Pressure, etc. Purchasing and ordering of medical supplies. Educates clients about methadone maintenance and detoxification as well as side effects and provides education to clients on lab results and all medical matters. Educates pregnant patients about prenatal care and assists primary counselor in coordinating outside services as needed.
