TheField Care Coordinator- HIDE SNPis an essential element of an Integrated Care Model and is responsible for establishing a set of person-centered goal-oriented, culturally relevant, and logical steps to ensure that the person receiving LTSS receives services in a supportive, effective, efficient, timely and cost-effective manner. Care coordination includes case management, disease management, discharge planning, transition planning, and addressing social determinants of health and integration into the community.
This position is Field Based with a Home-Based office. The expected travel time for member home visits is typically 75% within a 50-mile radius and/or 50-minute drive from your home pending business needs.
If you reside in or near Kalamazoo County, MI or surrounding area, you will have the flexibility to telecommute* as you take on some tough challenges.
Primary Responsibilities:
Develop and implement care plan interventions throughout the continuum of care as a single point of contact
Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
Advocate for persons and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team
Assess, plan, and implement care strategies that are individualized by the individual and directed toward the most appropriate, least restrictive level of care
Identifies problems/barriers to care and provide appropriate care management interventions
Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
Provides resource support to members for local resources for services (e.g., Children with Special Health Care Services (CSHCS), employment, housing, independent living, foster care) based on service assessment and plans, as appropriate
Manage the person-centered service/support plan throughout the continuum of care
Conduct home visits in coordination with the person and care team
Conduct in-person visits, which may include nursing homes, assisted living, hospital or home
Gathers, documents, and maintains all member information and care management activities to ensure compliance with current state and federal guidelines

Required Qualifications:
Must possess one of the following
Current, unrestricted independent licensure as a Registered Nurse (RN) in state of Michigan

1+ years of experience working in electronic documentation systems and with MS Office (Outlook, Excel, Word)
Access to reliable transportation and the ability to travel within assigned territory to meet with members and providers up to 75% of the time depending on member and business needs
Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
Ability to travel to Southfield, MI office for quarterly team meetings
Must reside within the state of Michigan
Preferred Qualifications:

**The Opportunity**
This is a field-based position, supporting Abbott's Vascular division. Abbott Vascular provides innovative, minimally invasive and cost-effective products for treatment of vascular disease. Our extensive portfolio includes drug-eluting stents, bare metal stents, guide wires, balloon dilatation catheters, imaging catheters and software, vessel closure devices, peripheral stents, thrombectomy catheters and atherectomy devices.

The Clinical Specialist will primarily be responsible for case planning, case support coverage, and product pull- through throughout the designated territory, and throughout the Region as needed. The Clinical Specialist will promote Vascular products through education, service and training of customers in the hospital setting. The Clinical Specialist will provide clinical education and sales support in order to assist in achieving projected sales goals, increasing sales revenues within assigned product lines, and increasing market share. This position will report to the Regional Sales Director.
**What You'll Work On**
+ Serves as the technical procedure and product expert in support of case coverage in the hospital setting.
+ Focuses on Interventional Cardiologists and hospital staff to support the complete Coronary portfolio including atherectomy, carotid, PCIO, DES and base coronary and future product releases.
+ Meet with existing and potential customers (e.g., physicians, physician office groups at hospitals) to identify their clinical needs, goals, and constraints related to patient care and to discuss and demonstrate how Vascular products can help them to achieve their goals.
+ Develop relationships with hospital personnel; make new contacts in hospital departments; identify key decision makers.
+ Serve as primary resource for clinical support in case coverage, troubleshooting and in-service education for company products.
+ Educate customers on the merits and proper clinical usage of company products by giving presentations and demonstrations using a wide variety of formats and platforms (e.g., slides, transparencies, manuals) to keep all customers abreast of the latest product, therapy, and technology developments and current items of interest in the industry.
+ Attend clinical procedures in the Cardiac Cath Lab, Interventional Radiology Lab, and Operating Room to ensure customer and patient success with Vascular products.
+ Respond to customer needs and complaints regarding products and service by developing creative and feasible solutions or working with other related personnel (e.g., sales, clinical research, marketing, product development) to develop optimal solutions.
+ Support the broader Region as needed with case support in addition to defined territory.

Provide training for counseling staff as well as master's level Clinicians throughout the Agency that can be arranged with the Wedgwood Institute.
Work collaboratively with referral sources to provide excellent clinical care with resources available.
Responsible for tracking and monitoring utilization and outcome data, including quarterly reporting for the all-agency report card.
Responsible for quality assurance compliance, including but not limited to participating in peer supervision, peer utilization reviews, compliance with service delivery criteria and reporting requirements for each individual served.
Responsible for timely Quality Assurance compliance as related to employee's roles and functions.

Key Responsibilities :
Optimal Care Hospice Clinical Managers are Registered Nurses who manage the coordination, supervision, and implementation of professional and supportive services to clients in a safe, cost-effective manner. The Clinical Manager may assist in education and training related to the clinical practice, regulation, and reimbursement changes.
In this role you will be responsible for:
Planning, directing, and participating in the delivery of professional services to hospice patients and families
Communicating significant findings, problems, and changes in condition or environment to the physician and other personnel involved with patient care
Initiating a plan for patient safety, using patient, family, and community resources
Identifying deviations from normal nursing and normal patient condition status
Directing all aspects of operations and care delivery in accordance with the mission and values of Optimal Care Hospice
Supervising the coordination of services with providers, vendors, or facilities in the delivery of care and related issues
Processing and coordinating patient referrals, physician orders, staff assignments, and patient re-certifications, and communicating between the referring physician, various caregivers, the patient and their family
Supervising and providing clinical direction to the RN, LPN, Chaplain, MSW, Aide, Music Therapist, Massage Therapist, Volunteers, and Bereavement Coordinator to ensure quality and continuity of services provided
Meeting with all field teams regularly to provide guidance and information
Maintaining adequate staffing levels to meet patient needs
Coordinating services to ensure the availability and competency of staff to respond to agency referrals and provide assessment visits within 24 hours of referral
Coordinating weekend on-call services for the field team
Required Qualifications :
Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
Current State License as a Registered Nurse
2-3 years experience as a Registered Nurse
Nursing skills as defined by accepted nursing standards
Current and valid driver's license
Reliable transportation and valid automobile insurance coverage
Knowledge of business management, governmental regulations, and CHAP standards
Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment
Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles
Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines
Desired Qualifications
BSN preferred
Home Health or Hospice experience is preferred

WORKING CONDITIONS
Work in an environment with medications and chemicals typical for an inpatient pharmacy.
ESSENTIAL FUNCTIONS
* Provides training, audit, and feedback for Pharmacy staff on clinical practice and medication use process.
* Oversees resident pharmacists, serves as program director for residency program(s), oversees student pharmacists, and coordinates education programs.
* Assesses the safety, efficiency, and effectiveness of pharmacy practice and leads process improvement work in these areas.
* Works with stakeholders to establish and implement best practices in pharmaceutical care.
* Serves as a liaison to nursing and medical staff and works directly with these groups to meet the needs of the department and organization.
* Assures compliance with policies, procedures, and TJC Medication Management standards.
* Supports leadership in designing and maintaining appropriate pharmacist coverage plans.
* Serves on committees as assigned.
* Works with department leadership to develop new pharmacy programs and services.
* Develops and executes goals that align with the strategic priorities of the department and organization.

* Responsible for tracking and monitoring utilization and outcome data, including quarterly reporting for the all-agency report card.
* Responsible for quality assurance compliance, including but not limited to participating in peer supervision, peer utilization reviews, compliance with service delivery criteria and reporting requirements for each individual served.
* Responsible for timely Quality Assurance compliance as related to employees roles and functions.

* Required to provide proof of certification within applicant's defined area: Certified Social Worker, Licensed Psychologist, Limited License Psychologist, Licensed Marriage and Family Therapist, or Professional Counselor.
* Additional therapeutic certifications preferred (i.e. DBT, TFCBT, etc.) but not required.
* Prior experience working with an interdisciplinary team preferred.
