The Care Coordinator will be the primary care manager for a panel of intellectually disabled/developmentally delayed members with varying risk and may be assigned other health plan populations as needed. Care coordination activities will focus on supporting members' medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care. Care Coordinator will be responsible for collaborating and coordinating care with community partners such as the CSB.
This is a Field-Based position with a Home-Based office. You must reside within a commutable distance of Culpeper, VA.

Required Qualifications:

1+ years of experience with MS Office, including Word, Excel, and Outlook
Driver's license and reliable transportation and the ability to travel within assigned territory to meet with members and providers if required

The Senior Field Clinical Specialist (Sr. FCS) will provide technical, educational and sales support as assigned and working with the sales team in a defined geography to meet sales and customer service objectives. The Sr. FCS will be the technical support person for Biotronik sales representatives and responsible for covering all product segments. The Sr. FCS will provide education, follow-up services, implant services, in service training, and other related services as necessary to salesforce, physicians and other cardiac pacing-related professionals. The Sr. FCS is responsible for providing mentorship and training to the field team. The Sr. FCS will report to the Regional Sales Director and take general instruction from local sales representatives for day-to-day activities.
Responsibilities
Provide pacemaker & ICD implant support within area of geographic responsibility. This will include follow-up to patient and physician customers.
Provide in service training to physician, nursing and technical hospital staff.
Provide technical procedure and product expertise in support of case coverage in the hospital setting.
Train and mentor peers in cardiac pacing and Biotronik products.
Provide technical and administrative support when required for clinical studies and scientific studies. This may include collection of patient data, support clinical sites and/or patient support.
Poses strong administrative skills to ensure all necessary paperwork associated with implant or follow-up associated with data collection for clinical/scientific studies is correct.
Maintain a competent and current level of knowledge in cardiac pacing, defibrillators and electrophysiology in general.
Maintain a comprehensive product and technical knowledge of all BIOTRONIK CRM products.
Competency in all aspects of ICD implant and follow-up. Sr. FCS that are not certified, must demonstrate technical proficiency and implant certification within six months of hire date.
Maintain or obtain North AMERICAN Society of Pacing & Electrophysiology (IBHRE) certification.
Maintain a comprehensive product and technical knowledge of competitive products including features, functions, and benefits.
Assist the sales representative when working within a specific sales territory.
Provide on-call coverage 24 hours per day and up to 7 days per week. Weekend call schedules shall rotate based on territory schedule. The Sr. FCS must carry a pager and be available at all times when on call.
Provide implant and follow-up coverage to Sales Representatives when he/she is unavailable or in training.
May be required to travel and provide implant support anywhere within the Area Sales Director's defined sales territory.
Out of town travel is expected to be no more than 30%.

Your Profile
Bachelor's degree (or equivalent) in Nursing, Physiology, Biology or Bio-Engineering. Graduate degree preferred.
Minimum seven years of experience working in pacing business with specific emphasis on pacemaker and ICD support.
Pacemaker implantation and/or follow up experience.
Prior experience in working with physicians and patients.
Excellent communication skills and the ability to work with all levels of the Company.
Travel/Availability Requirements

The Clinical Program Coordinator has the primary responsibility for coordinating the development and maintenance of evidence-based Order Sets, Smart Sets, Therapy Plans, and multidisciplinary Clinical Pathways. The Clinical Content Program Coordinator provides leadership, support and guidance to key stakeholder teams in planning, execution, monitoring, and control of Order Sets and Pathways to ensure deliverables are met on time and within budget/scope. The Program Coordinator partners with executive, clinical and medical staff leaders and hospital committees to ensure work aligns health system-wide initiatives and strategic goals. The Program Coordinator acts as an expert resource and an educator to operational leadership, clinician teams, and analysts regarding the development, implementation, and harmonization of Order Sets and Clinical Pathways across the health system. This role supervises the Agile Program Coordinator.
**RESPONSIBILITIES:**
Program Management:
+ Collaborate with University Medical Center and Health System leadership to manage the scope and implementation of the Order Set Review Committee and the Clinical Pathways Program.
+ Define the Order Set Review and Pathway development process and lead key stakeholders through the steps.
+ Manage programmatic input and support institutional liaison with any vendor partner/relationship insofar as supporting software (e.g. AgileMD) is used.
Order Set and Pathway Development:
+ Demonstrate strong communication skills using various mediums (e-mail, presentations, verbally, etc...).
+ Manage communication and expectations with various stakeholders, including healthcare professionals, IT teams, and other program participants.
+ Serve as a liaison to adjacent health system governance entities to ensure appropriate use of Order Sets and Pathways in alignment with policy and regulatory standards.
+ Foster harmonization of Order Sets and Clinical Pathways with health system priorities, and helps identify sources of redundancy or lack of alignment in clinical/technical content between various guidelines, order sets, and pathways.
+ Assist with Order Set and Pathway edits as necessary during the build process. As needed, act as a builder help to design, validate, and test pathways.
Governance:
+ Organize and maintain a governance committee to review and approve Order Sets and Clinical Pathways
+ Ensures regular review and prioritization of Order Set and Pathway proposals, adherence to standard formatting and technical review procedures, and the cadence of review of existing/live pathways.
+ Make recommendations for clarification, updates, and/or retirement of obsolete or outdated material to support streamlining of content across platforms.
+ Author and present programmatic updates to executive leadership in collaboration with governance committee.
Training and Coaching:
+ Train and coach new authors on Order Set and Agile principles and practices that align with UVA Health System standards.
+ Provide guidance and support to staff and/or management on complex problems or challenges.
Performance Monitor and Reporting:
+ Monitor the performance of order sets and pathways, and provide regular reports to stakeholders
+ Monitor and analyze program and project specific metrics and data outcomes; communicating to stakeholders and leadership as needed.
+ Implement and monitor a regular cadence of order set and pathway review which aligns with best practices and regulatory imperatives
MINIMUM REQUIREMENTS:
Education: Graduate of an accredited graduate school of nursing, physician assistant program with a master's degree or Pharm. D. degree.
Experience: 3-5 years of healthcare experience with demonstrated leadership experience. Experience in project management preferred. Epic proficiency required. Proficiency with Epic building or related activities and/or AgileMD proficiency preferred.
Licensure: Must be licensed to practice in the State of Virginia.
PHYSICAL DEMANDS:
This is primarily a sedentary job involving extensive use of desktop computers and may require occasional travel. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs.

This job requires extensive computer work, patient interaction, and the ability to traverse the footprint of the UVA medical center. Occasional travel to other UVA clinics. The job does occasionally require traveling some distance to attend meetings, and programs.
