The Clinical Nurse Coordinator (CNC) ensures and delivers high quality, patient-centered care and coordination of all functions in the unit/department during the designated shift. We are searching for a **Senior Clinical Research Monitor** to join Edwards Aortic Field Monitoring team. As a Senior Clinical Research Monitor, you ensure trial patients' safety by assuring trials are conducted ethically and within regulatory compliance, data accuracy, and protocol adherence. **How you'll make an impact:**
+ Field monitoring of studies and data collection for clinical trials, assess all data documentation, reports, records, transcripts, exam results for consistency with case report form, and determine if clinical trial/study subject documentation is within parameters of study hypothesis, as well as to monitor clinical trial/study safety, in keeping with protocols, GCP (Good Clinical Practices) and regulatory requirements.

Act as a mentor to new or junior level employees.
+ Provide oversight of and insights into trial activities (e.g., trends in protocol compliance, enrollment, monitoring deviations, data quality) to principal investigators and support staff, and study sponsor management and leadership staff, including Institutional Review Boards/Ethics Committees/Research Ethics Boards, Contract, and Training, developing process improvements.

Job Information Exemption Status: Exempt The Manager is responsible for the oversight of Clinical Documentation Improvement (CDI) and HCC Risk Adjustment Program. Oversight includes, yet not limited to, clinical screening, data compilation, documentation and entry into the SVH vendor database. Manages the CDI Specialists (CDIS) who provide concurrent review of clinical documentation to produce accurate and complete patient encounter documentation. Manager supports accurate documentation to effect profiling of the severity of patient illness, mortality statistics and proper reimbursement. Supports daily tasks in areas of CDI as directed by the Vice President of Quality and Population Health. Management functions include scheduling, daily assignments, communications, employee evaluations, employee discipline, and oversight of various documentation and administrative processes. Serves as ICD-10 hospital program liaison for CDI. Manage the Hierarchical Condition Categories (HCC) population health program. Responsible for the entire life cycle of HCC Risk Adjustment Factor (RAF) scores from assessment to submission and verification of policies and procedures. Oversees the encounter, data capture, and submission process. Assesses current procedures throughout the program and offers recommendations for enhancement, standardization and data gathering optimization. Presents and promotes the SVH HCC Program strategy to ambulatory leadership, physicians, and other stakeholders. Presents and explains performance data. Monitors and escalates performance improvement information to assist physicians who perform below benchmark. Oversees the performance of the staff and vendors involved with chart audits, clinical assessments, and the performance of internal resources devoted to the HCC Program efforts.

What you will do
Ensures competency of all CDIS and HCC staff by conducting on-going reviews and skills assessments; provides action plans and timely constructive feedback, as needed, to enhance staff development; leads hiring, training and performance management processes for all CDI and HCC employees; coaches, mentors and empowers employees to complete tasks by providing information and support.
Maintains time and attendance logs, approves PTO, and troubleshoots technical issues, in collaboration with the appropriate technology supportive roles, as needed for the CDI and HCC teams.
Works with direct reports to identify trends in the needs to clarifying documentation in the medical records and initiates an educational plan to communication with providers in order to capture the documentation in the medical record and accurately support the patient's severity of illness, risk of mortality.
Oversees the process of concurrent review and ongoing re-reviews for all selected admissions to initiate the tracking process, document findings on the DRG worksheets, and identify other key pathway or quality indicators as appropriate.
Maintains knowledge of all federal, state, and local documentation guidelines and facilities a plan to communicate and educate all CDI staff in collaboration with HIM coding manager on the changes as they are released; understands ICD-10 and coding guidelines.
Utilizes monitoring tools to track the progress of the concurrent review program, interprets tracking information, and reports findings regularly to the Clinical Performance Director and upon request by any other collaborating department, to include the overall CDI coverage rate, query rate, CC/MCC capture rates, physician response and agree rate, improvement strategies, and tactics.
Provides information and education as necessary in collaboration with HIM coding manager and the physician advisor to physicians and ancillary staff.
Leverage technology to the fullest to ensure revenue optimization, coding efficiency and improve the quality and timeliness of CDI staff. Be forward thinking with regard to the use of its application to the documentation improvement process.
Creates an environment in which staff contributes to decision making.
Develop, validate and implement data mining strategies for members covered under risk-based models (Medicare managed care, health exchanges, Medicaid, etc.). Work with physician leaders to develop population selection criteria (based on ICD-10 coding and other clinical indicators) and manage chart review validation process.
Monitor individual physician and clinic performance for key HCCs and diagnoses.
Present HCC Program performance results and findings regularly to the Clinical Performance Director, SVH physicians and SVH clinic administrators, to include the overall HCC RAF score, improvement strategies, and tactics.
Works collaboratively with the Health Information Management Coding manager to: 1. Assure CDIS and Medical Records coders are appropriately identifying principal diagnoses, secondary diagnoses, and procedures, and assigning a working DRG. 2. Conduct ongoing meetings with the CDIS and Coders to cover case studies, educational updates, monitoring report target DRG areas and process improvements impacting the collaborative workflow. 3. Assure the educational opportunity is carried out between CDI and coding when a DRG mismatch occurs. 4. Address and manage any DRG audits/denials that are received from Revenue Cycle. This includes clinically validating the audited/denied case with the lead CDIS and coder, drafting an appeal letter if appropriate and then reviewing it with the physician advisor.
Remains informed of SVH regulatory audit process with SVH designated stakeholders
Stays abreast of changes in regulations and data collection requirements and communications necessary changes and actions required on a regular basis.
Complete special projects as assigned by Stormont Vail Health, which require defining problems, determining work sequences, summarizing findings, and implementing solutions.
Utilize monitoring reports to identify low weighted DRGs with high volume, and high weighted DRGs with low volume. Focus documentation reviews and work with physicians to appropriately document for accurate MS-DRG assignment.
Run monthly data reports for use in analyzing success of the CDI and Clinical Appeals program.
Responds to, addresses and refers incoming communications from physicians and others responsible for medical record documentation.
Ensure workflow system operations are functional; escalate to pertinent stakeholders if indicated.

Job Summary

Summary
The MDS Nurse uses independent judgment in the planning, organizing, directing, and evaluation of activities of the professional and supportive nursing staff engaged in resident plan of care. Evaluates care provided to each resident and keeps care plans current. Is the direct lead in regards to the Resident Assessment Instrument (RAI) process. MDS Nurse assists in assessment and evaluation of potential admissions. Upon admission, quarterly and annual, the MDS Nurse completes resident assessments. Collaborates with the resident, family or advocate, other inter-disciplinary colleagues, including providers, to assure ongoing care of each resident to provide the best quality of life possible.
When functioning on the floor, the Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to patients. The RN demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned clinical setting. The RN cares for residents in all phases of preventative care, health maintenance, diagnosis, and treatment. The RN is responsible for the coordination of care, resident assessment, resident education and various other nursing interventions.
The RN may serve as charge nurse and delegate appropriate tasks to licensed and unlicensed assistive personnel that are within the job descriptions, skills, resident care needs, and the competence level of the delegate. As charge, the RN provides adequate assistance and support to delegates to ensure safe, reliable administration of resident care and resolution of care and service concerns.
The RN functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.
Qualifications
Graduate from a National League of Nursing (NLN) or American Association of Colleges of Nursing (AACN) accredited nursing program is preferred. Bachelor's Degree in nursing preferred.
Minimum of one-year experience working as licensed nurse with the senior population preferred. Specific services/positions may have additional education, training or experience requirements. Resident Assessment Coordinator-Certified (RAC-CT), management or supervisory experience preferred.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required certifications and completes required continuous education units (CEU) and competency training. Affiliation with an outside professional nursing organization is preferred.
