    Full-time Description Homeless Clinical Care Specialist FLSA CLASSIFICATION: Exempt REPORTS TO: Coordinator for Peer Support and Integrated Care None POSITION OVERVIEW: The Homeless Clinical Care Specialist is responsible for providing mobile, community-based integrated mental health and case management services to individuals experiencing homelessness or at risk for homelessness. This position will maintain an office at the multiagency center. This role involves conducting clinical assessments, developing individualized treatment plans, providing therapeutic interventions, and connecting clients to mental health, medical, housing, and substance use services. The specialist works in close partnership with outreach teams, shelters, hospitals, and community-based organizations and collaborates with multidisciplinary teams to meet the client's needs. ESSENTIAL POSITION RESPONSIBILITIES: Conduct comprehensive clinical assessments and diagnostic evaluations in field-based settings (e.g., shelters, encampments, street outreach). Develop individualized service plans in collaboration with clients and multidisciplinary teams. Provides short-term therapeutic support to help clients stabilize and transition to longer term care. Assist clients with crisis intervention and safety planning as needed. Monitor client progress and adjust plans as needed to ensure successful engagement and outcomes. Develops and maintains partnerships with housing navigators, outreach workers, and healthcare providers to support housing stability and treatment adherence including documentation in the HMIS system. Collaborate with case managers, outreach workers, housing navigators, and healthcare providers to develop and implement holistic service plans. Serve as a liaison between the client and service systems including behavioral health, medical providers, and legal entities. Conduct eligibility screenings and assess clients for Medicaid and other relevant public assistance programs. Assist clients in completing Medicaid applications, renewals, and appeals, including gathering required documentation. Utilize trauma-informed, harm-reduction, and client-centered approaches in all interactions. Maintain detailed and timely clinical documentation in accordance with agency standards. Participates in regular team meetings, supervision, and trainings. Attend required trainings, staff meetings, and professional development opportunities. Provides QMHP oversight with treatment plans in terms of reviewing, recommending continued or differing treatment options, and signature as needed. Serves as a liaison with all external partners regarding client care and needed collaboration or documentation. Completes peer reviews for departments within timeframe required. Assists with program revisions consistent with CARF accreditation guidelines. OTHER POSITION REQUIREMENTS: Maintains acceptable overall attendance record, to include department staff meetings, agency meetings, and trainings as required. Ensures appropriate notification to supervisor for absences and ensures that work is covered. Flexible in work schedule when needed. Exhibits appropriate level of technical knowledge for the position. Produces quantity of work necessary to meet job requirements. Works well with a team, keeps others informed of information needed. Treats others with respect, maintaining a spirit of cooperation. Maintains effective and professional verbal and written interactions with peers, customers, supervisors and other staff. POSITION REQUIREMENTS: The Homeless Clinical Care Specialist is expected to have completed a Master's Degree in Social Work, Marriage and Family Therapy, Psychology, Professional Counseling or Nursing and have a license in current standing with the BSRB or Kansas Board of Nursing. Preferred previous Case Manager experience. Preferred areas include knowledge of community resources, housing alternatives and vocational services; experience working with homeless population in the community and with individuals who have serious and persistent mental illness; knowledge of adult learning systems, ability to write and communicate verbally in a clear and concise fashion; and the ability to develop and maintain rapport with consumers, constituents and staff. Must maintain current licensure as required for position. A valid Kansas driver's license and access to personal vehicle required. Job Summary and Qualifications The Clinical Nurse Coordinator (CNC) ensures and delivers high quality, patient-centered care and coordination of all functions in the unit/department during the designated shift. What you will do in this role: * Assists with admission and discharge processes to ensure efficient throughput and high quality, patient-centered care. * Participates in the ongoing assessment of the quality of patient care services provided in the unit, in collaboration with other members of the management team. * Collaborates with subject matter experts and other managers to create an environment of teamwork that supports improved outcomes and service. * Supports a patient-first philosophy and engages in service recovery when necessary. * Supports the efforts of the facility to improve engagement by operationalizing current nursing strategies, including employee rounding, hourly rounds, and other initiatives. * Provides recommendations related to interviewing, selecting, and training new staff. Recommends and implements courses of action, including training and development, conflict resolution, personnel policy compliance, completion of performance evaluations, and/or disciplinary actions to ensure a competitively better organization. * Assists with staff scheduling. Manages all practical aspects of staff labor in accordance with patient care needs and established productivity guidelines. * Supports proper inventory control and assists with managing supplies and equipment.
