Collaborates with the Nutrition Services leaders and with operational leadership to provide optimal nutritional care for residents/patients. This position utilizes both clinical and supervisory leadership skills to identify opportunities for performance improvement. In the role of the clinical dietician, this position identifies and assesses the nutrition needs of residents/patients. Initiates interventions to achieve and maintain individual's nutritional status. Provides education to residents/patients and the public. Acts as a resource to the department and the facility.
Licensure/Certification Qualifications:



Registered Dietician by the Commission on Dietetic Registration



Current Dietician licensure in the state(s) of employment preferred, CFPM preferred.

**Qualifications**

**You will be responsible for:**

+ Under the general direction of the Director and Lead Clinical Dietitian and in collaboration with other members of the health care team, identifies patients at risk of nutritional deficit or excess.

+ Assesses nutritional needs and defines nutrient requirements.

+ Develops and oversees the implementation of the nutrition care plan for patients with complicated nutritional problems.

+ Evaluates the effectiveness of nutrition interventions.

+ Provides nutrition education related to medical nutrition therapy, and counsels patients and family members on appropriate food selections, eating habit and behavior changes to accomplish optimum patient outcomes and defined goals.

+ Promotes continuity of care through effective discharge planning, care coordination, and communication with the health care team.

+ Works with the parameters of the Kansas Dietitian Practice Act, regulatory standards, established policies, and procedures; and utilizes other pertinent references as guidelines for clinical practice.

**What will you need?**

+ Bachelor's Degree

+ 1 to 2 Years of Position-Related Experience

+ Includes experience as a clinical dietitian in an acute care health setting

+ Kansas Licensed Dietitian

+ Master's Degree _Preferred_ 5+ Years of Position-Related Experience _Preferred_

+ Certified Diabetes Educator _Preferred_

Provides optimal nutritional care by effectively identifying age specific patient needs, assessing nutritional status, designing appropriate care plans and monitoring to maximize clinical outcomes. Provides sound nutritional information and guidance to physicians, staff, patients and families. Provides functional guidance to Diet Technicians and Nutrition Assistants.
Performs nutrition assessments as per Nutrition Practice Guidelines.

Develops and implements patient specific nutritional care plans, interventions and goals as per Nutrition Practice Guidelines and Clinical Nutrition Policies.

Monitors and evaluates patients' responses to nutrition interventions.

Assesses patients for Medical Nutrition Therapy (MNT) instruction need.

Assists in management of nutrition support (EN/PN) as per Nutrition Practice Guidelines.

Provides MNT instruction to patients on modified diets when appropriate.

Monitors for food and drug interactions and makes appropriate interventions. Educates patients on Food/Drug interaction when appropriate.

Reviews prescribed diets/supplements for appropriateness and makes appropriate changes as per Protocol Orders Policy or makes recommendations for change.

Documents all assessments, patient education, nutrition care plans, interventions and goals in the EMR utilizing Standardized Language and Nutrition Diagnosis.

Confers with physicians and the other allied health care team members to clarify orders, recommends appropriate MNT and exchange patient information.

Provides information on MNT to physicians and other allied health care team members as needed. Provides guidance to Diet Technicians to ensure appropriate MNT is being delivered to patients.

Provides guidance to Nutrition Assistants to ensure menus are appropriate for the diet order.

Participates in patient rounds, attends and contributes to patient care meetings.

Participates in training nutrition interns.

Completes PI activities as directed by CNM.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

Registration as dietitian with CDR and Connecticut license required. Maintenance of registration and licensure required as per guidelines.

Work requires the knowledge of theories, principles and concepts of human nutrition and metabolism.

Work requires the analytical ability to collect information from diverse sources, synthesize a problem list and to devise/implement intervention.

Work requires the ability to communicate effectively both orally and in writing. This may include explaining clinical, technical concepts to physicians, other health care team members, patients and their families, comforting patients and their families, enforcing policies and regulations in difficult situations, or performing other duties requiring a comparable level of communication skill.

WORKING CONDITIONS:

Requires working in a normal patient care area or office environment where there are relatively few physical discomforts due to dust, dirt, noise and the like.

REPORTING RELATIONSHIPS:

Reports to the Clinical Nutrition Supervisor.

Has no responsibility for leading or supervising the work of other employees.

This position would be based out of Johnson Memorial Hospital in Stafford Springs, CT. with potential coverage at Mercy Medical Center.
