Summary:
This position provides individualized, goal directed general and critical care nursing, including assessment, procedure preparation, and procedure monitoring while administering sedation to patients that undergo diagnostic, interventional, emergent and therapeutic procedures. Nurses need to be familiar with caring for acutely ill intensive care patients and titrating intravenous medications. Care is delivered along the health illness continuum in diverse health care settings while collaborating with the health care team. We ensure high service standards while promoting an excellent patient experience. A high expectation to work as a team with the interventional radiologists, radiology technologists and anesthesiologists while providing personal and procedural needs and comfort for each patient. Accountable for the practice of nursing as defined by the Ohio Board of Nursing.
Responsibilities And Duties:
Assessment/Diagnosis - Performs initial, ongoing, and functional health status assessment as applicable to the population and or individual (30%).
Outcomes Identification/Planning - Based on nursing diagnoses and collaborative problems, documents planned nursing interventions to achieve outcomes appropriate to patient needs (30%).
Implementation/Evaluation - Evaluates and documents response to nursing interventions and achievement of outcomes at appropriately determined intervals; as part of a multidisciplinary team, revises plan of care based on evaluative data (20%).
Leadership - Actively participates in process improvement activities to achieve targeted measures of clinical quality, customer satisfaction, and financial performance (10%).
Operations (10%).

Assignment Cancelation Policy:
In the event Agency cancels an Agency Personnel assignment already accepted by Agency for assignment pursuant to an executed Assignment Confirmation, Agency shall give MSO not less than fourteen (14) days prior written/email notification of cancellation. In the event Agency fails to give fourteen (14) days prior written/email notification of such cancelation, Agency shall Agency shall owe to the relevant Platform Participant $1,000.00 as liquidated damages (to be paid or issued as a credit for future services via the VMS Platform, at MSO s direction).

Position Summary Hospice Clinical Manager:

General Working Conditions:
This position entails standing for long periods of time. We are seeking an experienced and compassionate Hospice Clinical Liaison to join our team. In this role, you will ensure that intake and referral processes are completed efficiently and effectively to meet the needs of patients, families, referral sources, and the organization. You will play a vital part in building strong professional relationships, coordinating admissions, and supporting community awareness of hospice services.
Essential Responsibilities
Receive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals
Assess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families
Educate patients and families on what hospice care includes, how it works, and why it matters-providing clarity, reassurance, and compassion
Complete necessary documentation and coordinate the intake and admission process, ensuring all legal, clinical, and payer requirements are met
Serve as a liaison between referral sources and our hospice care team-ensuring seamless handoffs, clear communication, and rapid response to needs
Make daily visits to hospitals and facilities, maintaining visibility and accessibility with discharge planners, case managers, and physicians
Build and maintain strong relationships with referral partners and identify new outreach opportunities to increase awareness of hospice services
Compensation may vary within the salary range provided based on several factors including but not limited to a candidate's location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.

About You
Education
Licensed Nurse with three to five years of hospice experience required.
BSN with five or more years of hospice experience preferred.
Experience
Minimum of three years of hospice experience required.
Prior experience as a discharge planner, case manager, or educator preferred.
Licenses and Certifications
Current RN license to practice nursing in the state of employment.
CPR certification.
Valid driver's license and automobile insurance.
Skills and Knowledge
Strong knowledge of hospice care and the unique needs of patients and families.
Familiarity with community resources and referral networks.
Excellent patient assessment and documentation skills.
Ability to supervise staff, coordinate services, and manage programs effectively.
Strong communication and interpersonal skills with sensitivity to diverse backgrounds.
Personal Qualities
Compassion and sensitivity to the needs of terminally ill patients and their families.
Flexibility to manage a variety of tasks and shifting priorities.
