About the Role We're looking for a dedicated Pharmacist to join our team and help us continue delivering the level of care our patients and families deserve. In this role, you'll be a vital part of our clinical team, ensuring excellent service, collaboration, and patient outcomes in a fast-paced healthcare environment. As the Pharmacist, you will ensure the safe and appropriate use of drugs, while optimizing the pharmaceutical care of the patient and provide drug information to other healthcare professionals and patients. You will participate in leadership of drug information program and provide clinical information to hospital and medical staff. What You'll Do * Interprets, reviews and evaluates medication orders for optimal dose, dosage form, frequency, concentration, duration, drug-drug interactions, drug-disease state interactions, drug allergy interactions, and potential cross allergencity, taking into account the individual patient's special or age related needs to ensure optimal drug therapy and timely dispensing of pharmaceuticals. * Coordinates daily activities of the department for proper workflow and supervises others to promote efficient use of Pharmacy personnel. Works with other staff members to ensure coverage of all pharmacy areas/duties in addition to assigned workload/staffing requirements. * Carries out policies and directives of Pharmacy and Therapeutics Committee, including clinical activities as assigned, and encourages compliance with hospital formulary. Accurately monitors clinical activities, including drug therapy, aminoglycosides or other drugs as requested, selects dosages based on patient's lab data, condition and other relevant data.; takes action as needed. * Resolves all problems that arise on the shift or from previous shift; if resolution cannot be accomplished within the shift, efforts documented/communicated to next shift pharmacist; problems not resolved within 24 hours are documented and referred to Director. * Provides continuing education or consults to hospital and medical staff concerning new drugs, drug warnings, drug therapy, drug actions and other pharmaceutical information necessary for proper/effective pharmaceutical care; documents interactions. * Participates and documents quality improvement process within the department, helps maintain continuous Joint Commission compliance. * Identifies potential/actual drug related problems including drug interactions/incompatibilities, adverse drug reactions, allergies, use of drugs without indications; seeks out and reviews identified necessary laboratory data; documents all activities/actions taken. Exercises initiative in developing patient and quality orientated pharmaceutical care program to improve patient outcome. * Deliver compassionate, patient-centered care in alignment with Saratoga Hospital values * Collaborate with interdisciplinary teams to support health, healing, and service excellence * Perform clinical responsibilities with a focus on safety, quality, and efficiency * Use hospital systems and tools to document care and support operations * Continuously seek opportunities to improve processes and support patient and staff satisfaction * Serve as a positive, professional representative of our hospital and community What You Bring * Graduate of an accredited school of pharmacy. Doctor of Pharmacy preferred. Completion of PGY1 residency preferred. * Current license and registration with New York Board of Pharmacy required.
