Monitor for adverse effects, toxicities, drug- drug interactions, and therapy complications. Support oral oncolytic management, adherence monitoring, and toxicity assessment. Perform medication assessments and reassessments prior to medication dispense. Collaborate with pediatric hematologists, oncologists, advanced practice providers, nurses and other members of the care team. Assist pharmacy liaison with PA submission, appeal letters, insurance troubleshooting, etc. Participate in clinics, case conferences, tumor boards, and multidisciplinary rounds as appropriate. Provide evidence-based pharmacotherapy recommendations aligned with pediatric oncology protocols and guidelines. Support specialty pharmacy dispensing workflows for hem/onc therapies, including limited distribution drugs and REMS programs. Facilitate medication access by assisting with prior authorizations, appeals, manufacturer programs, and patient assistance resources. Coordinate with payers, manufacturers, and specialty distributors to prevent delays in therapy initiation or continuation. EDUCATION: A PharmD or bachelor's degree from an accredited College of Pharmacy is required. POSITION SUMMARY: Clearway Health is looking for a Clinical Pharmacist that will be responsible for providing comprehensive, patient-centered medication management for pediatric patients receiving care in a multidisciplinary weight management, endocrinology, and obesity medicine clinic. This role focuses on optimizing pharmacotherapy for obesity and obesity related comorbidities, ensuring medication safety, supporting lifestyle interventions, and collaborating closely with pediatricians, endocrinologists, dietitians, behavioral health providers, and nursing staff. The pharmacist serves as a medication expert, educator, and clinical resource for patients, caregivers, and the healthcare team, with a strong emphasis on evidence-based pediatric obesity treatment, medication adherence, and long-term health outcomes. ESSENTIAL RESPONSIBILITIES/DUTIES: Participate in quality improvement initiatives related to pediatric obesity treatment outcomes, medication safety, and clinic efficiency Contribute to data collection, outcome tracking, and reporting as required Assist with medication access, including prior authorizations, formulary management, appeals, and patient assistance programs Work closely with specialty and outpatient pharmacies to ensure continuity of care and timely medication initiation Provide guidance on insurance coverage and cost-effective medication options Actively participate in multidisciplinary clinic visits and case conferences alongside physicians, advanced practice providers, dietitians, psychologists, and nurses Provide pharmacotherapy recommendations to the care team based on current pediatric obesity and pharmacotherapy guidelines. About the Job We are looking for an experienced Oncology pharmacist to join our team at MedStar Montgomery Medical Center in Olney, Maryland as a full-time Clinical Pharmacy Specialist I. Schedule is Monday - Friday 8:30a-5p with rotating weekends (day or evening shift on weekend). Under limited supervision provides a higher level of direct patient care pharmacy services for an assigned specialized patient population to assure adequate patient care according to professional standards and practices. Advises/consults with patients and their families and with the medical and nursing staff. Participates in research and drug use outcomes management programs within specialty area; instructs and trains pharmacy residents and students. Acts as a mentor to clinical pharmacists. Primary Duties and Responsibilities * Evaluates therapeutic options and provides clinical guidance to staff clinical level I and II pharmacists residents and residents. Monitors drug therapy and develops patient care plans of assigned patients. * Evaluates medication orders for accuracy and thoroughness. Resolves discrepancies of medication orders through communication with the physician clinical pharmacist or nursing staff and dispenses medication. Enters and verifies computer entries and data for drug orders as needed. * Provides functional supervision for support personnel.
