Job Summary:
The Department of Pharmacy at SUNY Downstate Health Sciences University is seeking a full-time Ambulatory Care Clinical Senior Pharmacist.
Under the direction of the Director of Pharmacy and Associate Director of Clinical Pharmacy Services, the Ambulatory Care Senior ClinicalPharmacist will be responsible for the provision of clinical pharmacy services to the patients in Adult Outpatient Clinics at the University Hospital at Downstate.
The Ambulatory Care Senior ClinicalPharmacist works as part of various multidisciplinary teams to ensure the delivery of safe, evidence-based, cost-effective, and individualized drug therapy. The Ambulatory Care Senior ClinicalPharmacist provides support of centralized and de-centralized medication use systems. The Ambulatory Care Senior ClinicalPharmacist also serves as a Pharmacy Department liaison and subject matter expert in the Ambulatory Care settings.
Additional Duties & Responsibilities
* Review medication profiles of patients seen in outpatient clinics
* Perform medication reconciliation at clinic visits to determine appropriateness of medication orders and to assess patient adherence
* Collaborate with physicians to optimize drug regimens
* Provide individualized medication counseling and education to patients, significant others or caregivers Provision of medication counseling includes, but is not limited to, intended use, expected side effects, correct dosage, special storage requirements, correct administration of drugs delivered thru devices, injection techniques, possible drug interactions or food/drug interactions
* Document interactions with patients in the Electronic Health Record (EHR)
* Identify barriers to medication adherence and implement measures to facilitate access to medications whenever necessary
* Collaborate with practitioners to identify areas for improvement in the medication management process for outpatients
* Work collaboratively with the Outpatient Pharmacy to ensure medication access to our patient population
* With New York State (NYS) Collaborative Drug Therapy Management (CDTM) Certification:
* Develop collaborative practice agreements with outpatient physicians based on institutional policies and procedures
* Initiate and make dosage adjustments based on clinical evaluation based on institutional policies and procedures
* Perform physical assessments and uses laboratory data to support drug therapy recommendations and dosage adjustments based on institutional policies and procedures
* Develop medication therapy management (MTM) services including billing for services as allowed Detect and report clinical interventions, medication safety events and adverse drug reactions Participate in the hospital's medication safety initiatives
* Serve as a member of the Pharmacy & Therapeutics Committee, pharmacy-specific and various institutional committees
* Participate in departmental and hospital-wide initiatives for quality improvement
* Assist in the review and development of policies, procedures and guidelines to promote safe and effective medication use in Ambulatory Care settings
* Assist with Pharmacy Department formulary management
* Design and deliver educational programs for pharmacy staff and other healthcare professionals including medical and nursing staff to promote the safe and effective use of medication therapy Responsible for the clinical training of pharmacy residents, pharmacy students, or other students completing rotations in the adult outpatient clinics
* Identify opportunities for improvement and create requests for changes in the EHR that will facilitate safer medication ordering and administration
* Participate in scholarly activities including peer-reviewed publications, medication use evaluations, clinical research projects, and cost-savings initiatives
Required Qualifications:
* Doctor of Pharmacy (PharmD) Degree from an ACPE accredited College of Pharmacy
* Completion of PGY-2 Specialty Pharmacy Residency
* OR, Completion of ASHP-accredited PGY-1 Pharmacy Residency with Clinical Pharmacy Experience (3-5 years) in Ambulatory Care
* New York State Pharmacist Licensure or eligibility
* New York State CDTM Certification or eligibility

The Pharmacist is responsible for dispensing medication in response to a physician's prescription, exercising professional judgment in accordance with established policies, procedures, standards, and government regulations to ensure appropriate drug therapy for optimal patient care. Bachelor's degree in Pharmacy including 1000 hours of internship under a licensed preceptor and ninety days of on-the-job experience at BHG required
Licensed Pharmacist in good standing with the state of Michigan required; must complete required number of hours of continuing education for re-licensure every two years
ACLS certification is required for pharmacists who are regularly scheduled to staff the ED or Adult MICU (one or more shifts per month on average).
Oncology Specific: At a minimum, PGY-1 training is preferred; PGY-2 training in oncology, board certification in oncology, or equivalent experience is preferred.
Basic word processing skills and able to operate hospital programs used in the Pharmacy
Anticoagulation Stewardship Specific: Responsibilities include developing protocols, monitoring therapy, conducting risk assessments, educating patients and staff, and ensuring compliance with clinical guidelines specific to anticoagulation. The role also involves meeting The Joint Commission (TJC) requirements by establishing protocols, providing patient education, ensuring regular monitoring, and implementing safety measures. The pharmacist will report to the System Clinical Coordinator and work directly with the lead provider to ensure safe and effective anticoagulant use. There will be tight alignment with the Bronson Anticoagulation Clinics. The scope of this position includes the acute care hospitals and Bronson practices. A PharmD, state licensure, and experience in anticoagulation management are required. Strong clinical knowledge and communication skills are essential.
Work which produces high levels of mental/visual fatigue, e.g., interactive and repetitive or small detailed work requiring alertness and concentration for sustained periods of time, the operation of and full attention to a personal computer or CRT between 40 and 70 percent of the time.
Involves considerable standing or walking, regular lifting of light-weight objects (i.e., 20 pounds or less) and assisting with heavier tasks such as assisting in lifting and moving patients. Minor straining or fatiguing positions must be assumed, and some fatiguing physical motions are required.
* Reviews, interprets, and fills prescriptions. Consults patient drug profile for allergies or drug interactions, refill status, and pricing. Ensures that proper type and dosage of drug is prepared and dispensed; ensures that prescription is complete and that route of administration is appropriate.

* Prepares or oversees the preparation and dispensing of IV medications. Evaluates prescriptions for accuracy of drug dose and schedule of medication, and enters information on patient profile. Prepares large volume parenterals, IV piggybacks, total parenteral nutrition, chemotherapeutic and antineoplastic agents, IV push medications, irrigations, patient controlled analgesics, ophthalmic preparations, and enteral feedings.
* Assists with overseeing the work of Unit dose and IV Additive Technicians the in performance of their regular duties. Checks and verifies accuracy of medications in carts; releases carts for delivery to patient care areas.

* May provide pharmacokinetic evaluation and dosing of patients on specific medications and monitors patient progress. Evaluates and acts on laboratory data and patient response.

Bachelor's degree in Pharmacy including 1000 hours of internship under a licensed preceptor and ninety days of on-the-job experience at BHG required
Licensed Pharmacist in good standing with the state of Michigan required; must complete required number of hours of continuing education for re-licensure every two years
ACLS certification is required for pharmacists who are regularly scheduled to staff the ED or Adult MICU (one or more shifts per month on average).
Oncology Specific: At a minimum, PGY-1 training is preferred; PGY-2 training in oncology, board certification in oncology, or equivalent experience is preferred.

Anticoagulation Stewardship Specific: Responsibilities include developing protocols, monitoring therapy, conducting risk assessments, educating patients and staff, and ensuring compliance with clinical guidelines specific to anticoagulation. The role also involves meeting The Joint Commission (TJC) requirements by establishing protocols, providing patient education, ensuring regular monitoring, and implementing safety measures. Essential Duties:

* Performs and/or oversees retail aspect of the pharmacy.
* Greets customers, receives prescriptions, obtains patient information, checks inventory, prepares product for dispensing and dispenses, selects appropriate container, maintains clean and organized work area, files prescriptions according to legal requirements, identifies recipient, counsels customer, performs financial transactions and uses computer to process prescription orders.
* Performs day to day maintenance activities on computer systems.
* Ordering of medications including C-II.
* Corrects errors in claims and resubmits them, reviews past due accounts, places drug orders as delineated.
* Performs and/or oversees institutional aspects of the pharmacy including, but not limited to, pharmacokinetic dosing, anticoagulation dosing, renal dosing, and antimicrobial stewardship.
* Receives, transcribes, processes physician orders.
* Prepares and maintains documentation related to patient care, quality assessment.
* Maintains inventory for nursing units, maintains controlled substances records.
* Oversees delivery of medication to nursing units per schedule.
* Able to pass IV additive training program, mixes IV solutions and chemotherapy as required using aseptic technique with the laminar flow hood or biological safety cabinet.
* Compounding and supervising the compounding of sterile, non-sterile and hazardous medications up to and including medium-risk compounding as defined by USP, USP and USP .
* Compounding caregivers adhere to the prescription or medical order for the strength, quality and purity of compounding preparations. Compounded products are packaged, labeled and dispensed according to current laws and regulations.
* Conducts orientation for new employees.
* Handles consumer concerns, uses proper telephone etiquette, follows instructions, demonstrates initiative and dependability.
* Exhibits self-confidence, resourcefulness, productive work habits and cooperation with others.
* Performs clerical and housekeeping duties as needed.
* Accurately measures substances.
* Administer vaccines.
* Able to process prescriptions for medications with risk evaluation and mitigation strategies (REMS)
* Able to follow regulations on sales of ephedrine and pseudoephedrine products.
Job Requirements
General Requirements Current license to practice as a pharmacist in the State of Michigan. Current license to dispense control drugs in the State of Michigan. Work Experience Previous retail pharmacy experience preferred. Experience as a pharmacist preferred. Education Bachelor's degree in pharmacy or PharmD. Specialized Knowledge and Skills Able to follow regulations on sales of ephedrine and pseudoephedrine. High degree of concentration is necessary when picking drugs for dispensing and typing directions. Able to pass IV additive training program, if required.

Job Summary:
The Department of Pharmacy at SUNY Downstate Health Sciences University is seeking a full time Outpatient/Retail Staff Pharmacist. The Staff Pharmacist reports to the Associate Director of Retail Pharmacy Services and prepares, compounds and dispenses medications for outpatient and ambulatory care use as well as dispenses retail and specialty pharmacy related items. Additionally, the successful candidate will:
* Provide information to health care practitioners to promote the rational use of medications.
* Provide drug monitoring service.
* Provide prescription medication counseling to patients as requested prior to discharge, time of dispensing and at patient bedside.
* Participate in teaching programs for staff.
* Monitor orders to ensure individualized dosage for both pediatric and adult patients.
* Monitor serum levels and make individualized patient recommendations based on pharmacokinetic and pharmacodynamic properties.
* Apply knowledge of pharmacology and pharmacokinetics to prevent duplicate therapy and drug interactions.
* Provide direct supervision of medication preparation and dispensing; demonstrate an efficient use of resources (labor, services, supplies/materials, communication/data, and space).
* Conduct mediation reconciliation and drug utilization review (DUR) to assure safety and appropriateness of medications.
* Review patient profile to ensure appropriate drug selection and dosage (no drug duplication, interactions or allergies) and route of administration, with accurate and complete information.
* Document all relevant interactions with patient and health care providers; document adverse drug reaction order clarifications per state and federal laws and regulations.
* Efficiently perform pharmacist duties including the safe procurement, preparation, labeling, distribution and control of all drugs.
* Participate in the pharmacy on-call rotation, as applicable.
* Complete quality assurance functions, including quality improvement projects, as assigned; perform all other duties as assigned by management.
* Contribute to the organization's overall process improvement activities; demonstrate critical thinking skills in identifying and analyzing problems and recommending solutions.
* Promote a culture of safety as it relates to identifying safety and security risks proactively, and working to prevent injuries and/or harm to patients, visitors, or members of the SUNY Downstate community; observe the patient safety goals and commit to safe behavior and standards.
