The Home Infusion Pharmacist is responsible for the clinical management and oversight of parenteral nutrition and other infusion therapies provided in the home setting. This role emphasizes patient assessment, prescribing recommendations, therapy optimization and interdisciplinary collaboration. The pharmacist serves as a clinical resource to providers, patients, and internal teams, ensuring safe, effective, and evidence-based therapy while supporting quality assurance and continuous performance improvement initiatives.
Clinical Responsibilities
Conduct comprehensive patient assessments to determine appropriateness for home infusion therapy, including review of medical records, laboratory values, nutritional status, and overall clinical condition.
Review and evaluate prescription orders for clinical appropriateness, accuracy, completeness, and safety.
Recommend, initiate, and adjust TPN formulations based on patient-specific clinical needs, laboratory trends, and established practice guidelines.
Communicate with prescribers, dietitians, and nursing staff to clarify orders, recommend therapy modifications, and support ongoing clinical decision-making.
Ensure all prescriptions meet legal, regulatory, and accreditation standards prior to dispensing.
Maintain accurate and up-to-date patient profiles within the pharmacy system, including diagnoses, lab data, allergies, therapy history, and clinical interventions.
Provide support with sterile compounding activities, including review of compounding records and product verification while maintaining compliance with all regulatory and documentation requirements.
Patient Care & Education
Provide patient and caregiver education regarding TPN therapy, including administration expectations, potential adverse effects, monitoring parameters, and adherence to the treatment care plan.
Serve as a clinical point of contact for patient-related concerns, including assessment of reported side effects, line-related issues, or therapy complications.
Support timely clinical intervention in emergent or urgent situations such as adverse drug reactions, suspected infections, or therapy intolerance, escalating as appropriate.
Interdisciplinary & Care Coordination
Collaborate with physicians, nurses, dietitians, discharge planners, and case managers to facilitate smooth transitions of care from inpatient or outpatient settings to home infusion therapy.
Partner with the billing and reimbursement team to verify insurance coverage requirements, support prior authorization needs, and explore clinically appropriate alternatives when coverage limitations exist.
Operational, Regulatory & Oversight Responsibilities
Maintain knowledge of and comply to USP , state board of pharmacy, and federal regulations as they relate to clinical practice and patient safety.
Assist with pharmaceutical recalls, medication safety reviews, incident and clinical intervention assessments as needed.
Participate in surveys and inspections conducted by authorized regulatory and accreditation agencies, representing clinical pharmacy services.
Quality Assurance & Performance Improvement
Document medication errors, adverse events, and near misses for Quality Assurance review.
Report incidents to appropriate management in accordance with internal policies and regulatory requirements.
Actively participate in the companys Performance Improvement Program, contributing clinical expertise to process improvement and risk mitigation initiatives.
Professional Responsibilities
Maintain active pharmacist licensure and complete continuing education aligned with clinical practice, TPN therapy, and regulatory requirements.
Assume on-call responsibilities as required to support patient care continuity.
Oversee pharmacy staff as assigned
Additional duties and responsibilities in different areas of the department on an as-need basis or determined by management

Required Qualifications
PharmD or B.S. Pharmacy
At least two years of related home infusion experience, specifically in managing TPN patients required
Unrestricted active CAPharmacist license

The Clinical Formulary Pharmacist will play an important role in providing operational support for evaluation, implementation, and maintenance of Medicare Part D Formulary and Utilization Management, review of new drugs with the Pharmacy & Therapeutics Subcommittee/Committee. This position is accountable for ensuring regulatory compliance with respect to the Medicare Part D formulary submissions to CMS, pharmacy benefits, formulary and transition program implementation with PBM, website content for all business lines, pharmacy ne work and CMS's Part D regulatory audits. The Clinical Formulary Pharmacist stays abreast of pharmacy, PBM, payer industry changes, pipeline, drug trends, best practices, and CMS's rules and regulations, including Inflation Reduction Act. The role requires strong cross functional collaboration and communication skills, and an understanding of Medicate Part D formulary and Utilization Management (UM) processes.
You Will
Ensures regulatory compliance with P&T operations, Formulary & Benefit Administration, Transition Programs, annual Part D bid submission process, pharmacy-related member and other stakeholders' communications and marketing materials, including member formulary drug list, website content, trainings, pharmacy network and CMS's regulatory audits.
Produces, maintains, and implements Part D Formulary, Prior Authorization criteria, Step Therapy criteria, Supplemental files, and clinical rules in accordance with CMS requirements on monthly and annual basis and submits these files to CMS.
Develops and maintains formulary exception criteria.
Evaluates and reviews new drugs/biosimilar with the Pharmacy & Therapeutics
Subcommittee/Committee within 90-180 days of being marketed and makes Formulary and UM recommendations based on the clinical appropriateness, cost analysis and CMS guidelines.
Works in collaboration with Medical Directors to evaluate and analyze treatment protocols and procedures related to the SCAN Health Plan formulary, at least annually.
Provides CMS Part D regulatory audit support for respective areas.
Produces member communications and marketing materials, within internally determined timeframes, with accuracy and in accordance with CMS marketing guidelines. Partners with Marketing, Compliance and Member Services and vendors, as needed. Prepares training materials.
Maintains on-line comprehensive formulary on a monthly basis on accordance with CMS requirements.
Assists with Formulary and UM modeling for the annual Formulary and BID submissions to CMS. Reviews accuracy of the Formulary, UM files, and Pharmacy Benefits for the annual BID submission to CMS.
Prepares Formulary/Pharmacy Benefits Industry Analysis annually. Complies and implements Inflation Reduction Act requirements with PBM and CMS Innovation Programs, as needed.
Monitors drug pipeline, drug trends, and payer industry changes throughout the year to identify potentially high Part D impact drugs. Manages Part B drug trend and value-based opportunities.
Oversees PBM operations in the areas of Formulary, Opioid Safety Edits, Pharmacy Benefits, Transition Programs, Prescriber Enrollment Requirements and Pharmacy Network Implementation.
Ensures AEP and 1/1 readiness and assists in training of other departments. Serves as the clinical subject matter expert for respective areas, providing guidance as needed.
Maintains current knowledge of Medicare Part D regulations by participating in CMS calls and reading released guidance and implementing changes to maintain compliance.
Manages multiple deadlines and completes projects with 100% accuracy.
