Part-time Description The Primary Care Pharmacist is responsible for the interpretation of medical orders and counseling patients on proper medication administration and storage. The Primary Care Pharmacist must have strong communication with fellow colleagues to ensure the expectations of Patient Aligned Care Team metrics are met. * 3 days per week schedule Core Responsibilities Evaluates and enters provider's orders/prescriptions for appropriateness of therapy. Evaluates incompatibilities, side effects and interactions of drugs and drug products, and resolves drug-related questions with the prescriber. Documents all medical information into the patient's electronic medical record utilizing the VA's VISTA/CPRS electronic patient record system. Maintains confidentiality of veterans' data and information. Provides patient consultation by reviewing patient profiles and medication orders. Provides drug information consult to providers, staff, and other healthcare professionals. Maintain a record of all C2 prescriptions ordered by providers in your clinic. Maintain a record of all 10-day urgent prescriptions prescribed by our clinic provider and obtained by the patient from a local pharmacy vendor. Evaluates patients' records from medical providers outside the VA and works with these patients utilizing rules set forth by the VA for co-managed care. Completes any and all clinical reminders “due” at the time of each patient visit. Completes the documentation of the medical record within twenty-four hours of patient encounter. Complies with all VA guidelines in regard to appropriate and timely clinical documentation and response to patient requests. Complies with all VA and company training requirements. Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission(JC), Environment of Care (EOC) oversight, lab compliance and other related items. Remain focused on achieving excellent clinical outcomes through the specified VA guidelines. Must provide excellent customer service to each veteran, both in person and over the phone, as well as to fellow colleagues and clinic visitors. Participate in the clinic's outreach events to help support the clinic's enrollment initiatives. Support patient enrollment and retention by providing guidance, when necessary, regarding scheduling, follow-up visits or nurse visits. JOB DESCRIPTION Job SummaryProvides customer service support for inbound/outbound pharmacy calls from members, providers, and pharmacies. Contributes to overarching pharmacy strategy for optimization of medication related health care outcomes, and quality cost-effective member care. Essential Job Duties - Handles and records inbound/outbound pharmacy calls from members, providers and pharmacies in accordance with departmental policies, state regulations, National Committee of Quality Assurance (NCQA) guidelines, and Centers for Medicare and Medicaid Services (CMS) standards. - Provides coordination and processing of pharmacy prior authorization requests and/or appeals. - Explains point-of-sale claims adjudication, state, NCQA and CMS policies/guidelines, and any other necessary information to providers, members and pharmacies. - Assists with clerical tasks and other day-to-day pharmacy call center operations as delegated. - Effectively communicates plan benefit information, including but not limited to: formulary information, copay amounts, pharmacy location services and prior authorization outcomes. - Assists members and providers with initiating verbal and written coverage determinations and appeals. - Records calls accurately within the pharmacy call tracking system. - Maintains established pharmacy call quality and quantity standards. - Interacts with appropriate primary care providers to ensure member registry is current and accurate. - Supports pharmacists with completion of comprehensive medication reviews (CMRs)through pre-work up to case preparation. - Proactively identifies ways to improve pharmacy call center member relations. Required Qualifications - At least 1 year related experience, including call center or customer service experience, or equivalent combination of relevant education and experience. - Excellent customer service skills. - Ability to work independently when assigned special projects, such as pill box requests, case management referrals, over the counter (OTC) requests, etc. - Ability to multi-task applications while speaking with members. - Ability to multi-task applications while speaking with members. - Ability to develop and maintain positive and effective work relationships with coworkers, clients, members, providers, regulatory agencies, and vendors. - Ability to meet established deadlines. - Ability to function independently and manage multiple projects. - Excellent verbal and written communication skills, including excellent phone etiquette. - Microsoft Office suite (including Excel), and applicable software program(s) proficiency. Preferred Qualifications - Certified Pharmacy Technician (CPhT) and/or state pharmacy technician license (state specific if state required).
