Responsibilities Fill prescriptions, verifying instructions from physicians on the proper amounts of medication to give to patients. Check whether the prescription will interact negatively with other drugs that a patient is taking or any medical conditions the patient has. Instruct patients on how and when to take a prescribed medicine and inform them about potential side effects they may experience from taking the medicine. Advise patients about general health topics, such as diet, exercise, and managing stress, and on other issues, such as what equipment or supplies would be best to treat a health problem. Give flu shots and other vaccinations. Listen carefully to customers to interpret their needs and issues and offer information and advice. Prepare medicine when appropriate using correct dosages and material for each individual patient. Keep records of patient history and of all activities regarding heavy medication Keep abreast of advancements in medicine by attending conferences and seminars and collaborating with other healthcare professionals. Comply with all applicable legal rules, regulations and procedures. Requirements Proven experience as a pharmacist Thorough understanding of dosage administration and measurement, chemical compounds, medical brands etc. Outstanding knowledge of MS Office and Pharmacy information systems (McKesson) Excellent organizing skills Excellent communication skills with a customer centric approach Integrity and compassion Degree in Pharmacy/Pharmacology Valid license to practice the profession in the State of NY Must be available to work a flexible schedule. PHARMACIST-IN-CHARGE (PIC) (IF APPLICABLE) NEW JERSEY The PHARMACIST-IN-CHARGE is responsible for all the duties listed for Retail Pharmacist along with the following listed guidelines set forth by the New Jersey State Board of Pharmacy: Every pharmacy shall name a pharmacist whose license is in good standing in New Jersey as the pharmacist-in-charge (PIC) of the pharmacy. No pharmacy shall operate without a PIC for longer than 30 days. Whenever the pharmacist-in-charge (PIC) is absent from the pharmacy for more than 30 days, the PIC and the permit holder shall notify the Board of the name of the pharmacist who shall act as the interim PIC. A pharmacist shall not assume the responsibilities of a PIC of more than one pharmacy or pharmacy department simultaneously. Whenever there is a change of a PIC of a pharmacy, an inventory of all controlled dangerous substances shall be performed consistent with NJ requirements. Whenever a pharmacist assumes or terminates the duties as a PIC of a pharmacy, the PIC and the permit holder shall so advise the Board in writing within 30 days by completing a form provided by the Board. A PIC shall be a full-time employee, employed for a minimum of 35 hours per week and shall be physically present in the pharmacy or pharmacy department for that amount of time necessary to supervise and ensure that: The pharmacy is staffed by sufficient, competent personnel in keeping with the size, scope and complexity of the pharmaceutical services provided by the pharmacy. Accurate records of all prescription medications received and dispensed are maintained. Policies are in place regarding accurate dispensing and labeling of prescriptions and compliance of policies. Security of the prescription area and its contents are maintained at all times consistent with the regulations. Only pharmacists and interns or externs under immediate personal supervision provide professional consultation with patients and practitioners. Only pharmacists, interns or externs accept telephone prescriptions and only pharmacists, interns or externs or pharmacy technicians consistent with Board of Pharmacy requirements, accept renewal prescriptions. No misbranded, deteriorated, adulterated, improperly stored or outdated drugs or any drugs marked 'sample' or with any like designation or meaning are dispensed or present in the active stock of the Pharmacy. The prescription area is maintained in an orderly and sanitary manner. The pharmacy and all pharmacy personnel provide pharmaceutical services in accordance with acceptable professional standards and comply with all Federal and State statues, rules, and regulations regarding the practice of pharmacy. There are written policies and procedures to ensure the proper storage and delivery of all prescription drugs and chemicals consistent with the requirements set forth in N.J.A.C. 13:39-5.11 and that such policies and procedures are followed. The pharmacist-in-charge is responsible for the accuracy and completeness of the biennial inventory of all controlled dangerous substances required under N.J.A.C. 13:45H-5.5 and shall sign and date the biennial inventory upon its completion. This requirement applies whether the inventory is conducted by the pharmacist-in-charge or another licensed pharmacist. If the pharmacist to pharmacy technician ratio exceeds 1:2, the pharmacy shall maintain a policy and procedure manual with regard to pharmacy technicians. The pharmacist-in-charge shall review at least every two years and, if necessary, amend the policy and procedure manual. Documentation of the review shall be made available to the Board upon request. Manages all aspects of the pharmacy operation and compliant with all Curexa SOPs, policies and procedures, NJ State regulations and federal law in accordance with Curexa's accreditation bodies. PIC and Compounding Pharmacist The PIC and Compounding Pharmacist shall be responsible for the Retail Pharmacist duties along with the following listed guidelines set forth by the NJ State Board of Pharmacy 13.39 Subchapter 11A for Compounding Non-Sterile Preparations in a retail pharmacy which ensures that: 1) Compounded non-sterile preparations have been properly prepared, labeled, controlled, stored, dispensed, and distributed compliantly. 2) All aspects of the compounding process set out in N.J.A.C. 13:39-11A.15 are documented and that accurate compounding records for all compounded non-sterile preparations prepared by the pharmacy are maintained. 3) Pharmacists and Compounding Lab Technicians are capable of performing and qualified to perform their assigned duties including compounding medications. The compounding pharmacist shall provide immediate personal supervision to pharmacy technicians, pharmacy interns, or pharmacy externs who are performing non-sterile preparation compounding. Supervision shall include, but is not limited to, the checking of each ingredient used, the quantity of each ingredient whether weighed, measured, or counted, and the finished label. The compounding pharmacist may delegate to pharmacy technicians, pharmacy interns, or pharmacy externs only the following tasks: recording of the prescription, selection of the drugs and container, typing of labels, and compounding of preparations. The compounding pharmacist shall ensure that each task has been performed correctly. 4) Ingredients used in compounding have their expected identity, quality, and purity consistent with the requirements of N.J.A.C. 13:39-11A.12. 5) Compounded preparations are of acceptable strength, quality, and purity, with appropriate packaging and labeling, and are prepared in accordance with good compounding practices, official standards, and relevant scientific data and information as set forth in USP 795. 6) Critical processes are recorded and validated to ensure that procedures will consistently result in the expected qualities in the finished preparation. 7) The compounding environment is suitable for its intended purpose. 8) Appropriate stability evaluation is performed or is determined from the literature for establishing reliable beyond-use dating to ensure that the finished preparations have their expected potency, purity, quality, and characteristics, at least until the labeled beyond-use date. 9) Compounding conditions and procedures are in place to minimize the potential for errors. 10) Adequate procedures and records exist for investigating and correcting failures or problems in compounding, testing, or in the preparation itself. 11) The patient is advised that the product dispensed is a compounded preparation. 12) A pharmacist shall not compound preparations that contain drug products that appear on the Federal Food and Drug Administration's list of Drug Products Withdrawn or Removed from the Market for Reasons of Safety or Effectiveness, codified at 21 CFR 216.24. b) A pharmacist shall not compound any commercially available drug products except as provided in N.J.A.C. 13:39-11A.4. Any confirmed incidents of product contamination shall be reported by the pharmacist-in charge to the New Jersey Board of Pharmacy within 48 hours of becoming aware of any such incidents. The compounding pharmacist shall ensure that each task has been performed correctly. All Pharmacists and Pharmacy Technicians that are compounding preparations comply with all USP 795, USP 800 guidelines, federal and state board of pharmacy laws along with all Curexa accreditation body requirements in compliance with FDA 503a pharmacy guidelines. MEASURES OF PERFORMANCE The Pharmacist should conduct themselves in an ethical and prudent manner in compliance with all company policies and procedures. The Curexa code of ethics should be followed in all duties performed as described by the employee handbook. All prescriptions are filled, checked and dispensed accurately. Compliance with all federal and state board of pharmacy laws pertaining to checking of prescriptions, dispensing, and patient confidentiality. Patient consultation is completed proficiently and according to state and HIPAA guidelines including all drug and pharmacy information required regarding adverse reactions, drug interactions, drug allergies, disease states, storage and disposal of medications, complaint and return policy and pharmacy contact information. The pharmacy should have no unaccountable inventory shortage or financial shortage which includes investigation of any negative profit claims. Customer complaints from individuals or practitioner offices should be few and professionally, efficiently, and expeditiously resolved. Complaint forms should be completed along with APMS(PQC) platform utilized for accountability and analysis of workflow efficiency. All Curexa SOPs and PPs are followed regarding Retail and Compounding Pharmacy practices. Compounding Pharmacy practices are compliant with all USP 795, USP 800 guidelines and state board of pharmacy and federal laws along with Curexa accreditation requirements for an FDA 503a pharmacy. If conflicting regulations, the most stringent laws are followed. Curexa recall procedures and adverse reaction reports are completed properly and followed-up, as applicable. Participation in marketing and sales programs in-store and external activities performed with positive energy and professionalism for company growth. Continuing education courses, reading didactic literature sources, attending live seminars, and overall furthering of knowledge pertinent to retail pharmacy and compounding practices should be ongoing to keep compliant with accreditation guidelines and state licensure renewals.
