The Clinical Pharmacist works collaboratively with clinicians, pharmacists and support staff to provide seamless, quality care for patients at Yale Health. Responsibilities include: Medication Management: Accurately and efficiently prepares and dispenses (and/or directly supervises) medications and pharmaceutical products, ensuring proper labeling, packaging, dosage information, and stock levels. Manage sterile compounding technician and daily IV workflow, in accordance with applicable guidelines and standards. Conduct timely and accurate review of medication orders for appropriateness. Monitor patients' responses to drug therapy and adjust medication regimens as outlined in the CDTM agreement and/or policy Counsel patients on the safe and appropriate use of medications. Clinical Support: Provide medication information to internal departments Support pharmacy operations with development of protocols/guidelines, formulary reviews, and medication use evaluations; and identifies opportunities to expand and improve clinical pharmacy services throughout the organization. Support clinical departments to address issues related to medication access and/or functionality of automated dispensing cabinets (ADCs). Provide on-call coverage as per assigned calendar. Safety and Integrity: Adhere to organizational and regulatory guidelines, standards and best practices.
