Title: Clinical Pharmacist Department: Pharmacy Reports To: Pharmacy Manager Location: New Haven, CT. Cornell Scott-Hill Health Center is looking to add a Per Diem Clinical Pharmacist to our team. The Clinical Pharmacist is responsible for planning and providing clinical pharmacy services, disease management, Medication Therapy Management and other patient/staff education programs ensuring compliance with current regulations and in accordance with health center needs and national best practices. Assists the Director of Pharmacy with planning and operation of the Pharmacy Department. Assists in training, supervision and evaluation of technicians and pharmacy support staff. Performs all other duties as required. Per Diem Requirements: The position requires a flexible availability from Monday through Friday 8:30a to 5pm, and Saturday from 8:30 - 12:30pm, with shifts of up to 8 hours per day during the week, as needed. Responsibilities include providing coverage for colleagues' sick days and vacation time. To maintain a Per Diem status, the Clinical Pharmacist must work a minimum of 4 shifts per month, not to exceed 19 hours per week. Requirements/Qualifications: Degree from an accredited college of Pharmacy required. Residency preferred. Licensed or eligible to practice pharmacy in the state of Connecticut.
