Basic Qualifications
BS in Pharmacy or Pharmacist Degree from an accredited educational institution.
Current pharmacist licensure in the states within the district.
Experience performing prescription dispensing activities that demonstrate a strong working knowledge of applicable state and federal controlled substance laws.
Certified Immunizer or willing to become an immunizer within 90 days of hire.

POSITION SUMMARY:

Job Description
Under general direction, the ClinicalPharmacist performs professional pharmaceutical duties in drug education and utilization, counsels patients on drug usage, educates health care personnel on drug utilization, and may compound drugs and dispense medicines in a clinical pharmacy. The essential functions include:
The essential duties of this position include but are not limited to:
Interviews and examines patients for drug evaluation; checks on drug adherence and side-effects; discusses symptoms and effects of prescribed drugs with patients and maintains a drug history on patients interviewed.
Consults with physicians on drug usage and the drug regime of specific patients particularly hypertensive, diabetic, and psychiatric patients; recommends changes in the drug or the dosage prescribed for these patients.
Conducts both formal and informal training for nursing staff, medical and pharmacy students and residents and other health care staff on drug utilization, dosage, incompatibilities, preferred dosage forms, evidence-based medication therapy and toxicity, etc.
Participates in the development and revision of medication use policies and guidelines.
Compounds drugs, prepares, and dispenses medications, checks pharmacy stock, and keeps records.
The 2454 ClinicalPharmacist also performs other related duties as assigned.
Nature of positions: Positions are located throughout the Department of Public Health, including Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG), Laguna Honda Hospital and Rehabilitation Center (LHH), Community Primary Care and Mental Health Clinics, as well as San Francisco County Jails. * Teaches students the technical skills used in monitoring drug therapy; advises students on actions to undertake in solving drug therapy problems; and teaches interpersonal communication and patient counseling techniques; evaluates students' oral case presentation; grades students' reports; and assigns topics and leads group discussions.
* Monitors and evaluates adverse drug reactions and pharmacist interventions; compiles and reports statistical information; develops quality assurance plans to assess appropriate use of drugs.
* Coordinates clinical pharmacy activities with physicians and nursing, pharmacy and laboratory personnel; develops and assists in the implementation of clinical pathways and therapeutic guidelines.
* Provides in-service training programs for physicians, nursing and pharmacy staff members on drug therapy and related topics; edits and publishes information bulletin for health care professionals at the hospital.
* Develops protocols to monitor drug therapy; conducts routine prospective DUE's; and maintains an up-to-date drug information service.
* Serves on hospital committees as designated to provide clinical drug information and critical analysis of use.
* May interpret, fill and dispense prescriptions and perform other pharmacist duties as assigned.
MINIMUM QUALIFICATIONS
Education: A Pharmacy Doctorate degree from an accredited university.
Experience: Two years of professional experience as a pharmacist in an acute care hospital which included clinical responsibilities.
Substitution: The completion of an approved one-year clinical or general residency program in pharmacy may be substituted for the required experience.
License: Possession of a valid license as a Registered Pharmacist issued by the California State Board of Pharmacy.

Sick Leave: 12 working days of sick leave annually with unlimited accumulation. Bereavement Leave: 3 days of paid leave for the death of qualifying family member, 2 additional days of accrued leave for death of employee's spouse, domestic partner, parent or child.
Merit Salary Increase: New employees will receive the starting salary, which is the first step of the salary range. After employees serve 52 weeks (2080 hours) on each step of the range, they are eligible for a merit increase to the next step.
Job Sharing: Employees may agree to job-share a position, subject to approval by a Department Head and the Director of Human Resources.
Educational Reimbursement Program: Eligible employees may be reimbursed for career-related course work up to a maximum of $850 per fiscal year. Eligible employees enrolled in an approved four (4) year College or University academic program may be reimbursed up to $800 per semester for a maximum of $1600 per fiscal year.
Parking Supplemental Downtown Stockton: The County contributes up to $20 per pay period for employees who pay for parking and are assigned to work in the Downtown Core Area.
School Activities: Employees may take up to 40 hours per year, but not more than eight (8) hours per month, to participate in their children's school activities.
Selection Procedures
Applicants who meet the minimum qualifications will go through the following examination process:
* Written Exam: The civil service written exam is a multiple choice format. If the written exam is administered alone, it will be 100% of the overall score. Candidates must achieve a minimum rating of 70% in order to be placed on the eligible list.
* Oral Exam: The oral exam is a structured interview process that will assess the candidate's education, training, and experience and may include a practical exercise. The oral exam selection process is not a hiring interview. A panel of up to four people will determine the candidate's score and rank for placement on the eligible list. Top candidates from the eligible list are referred for hiring interviews. If the oral exam is administered alone, it will be 100% of the overall score. Candidates must achieve a minimum rating of 70% in order to be placed on the eligible list.
* Written & Oral Exam: If both a written exam and an oral exam is administered, the written exam is weighted at 60% and the oral exam is weighted at 40% unless otherwise indicated on the announcement. Candidates must achieve a minimum rating of 70% on each examination in order to be placed on the eligible list.
* Rate-out: A rate-out is an examination that involves a paper rating of the candidate's application using the following criteria: education, training, and experience. Candidates will not be scheduled for the rate-out process.
Note: The rating of 70 referred to may be the same or other than an arithmetic 70% of the total possible points.
Testing Accommodation: Candidates who require testing accommodation under the Americans with Disabilities Act (ADA) must call Human Resources Division at ************** prior to the examination date.
Veteran's Points: Eligible veterans, unmarried widows and widowers of veterans of the United States Armed Forces who have been honorably discharged and who have served during wartime shall be given veteran's points in initial appointment to County service. Physical Exam: Some classifications require physical examinations. Final appointment cannot be made until the eligible has passed the physical examination. The County pays for physical examinations administered in its medical facilities.
Pre-Employment Drug Screening Exam: Some classifications require a new employee successfully pass a pre-employment drug screen as a condition of employment. Final appointment cannot be made until the eligible has passed the drug screen. The County pays for the initial drug screen.
Employment of Relatives: Applicants who are relatives of employees in a department within the 3rd degree of relationship, (parent, child, grand parent, grand child or sibling) either by blood or marriage, may not be appointed, promoted, transferred into or within the department when;
* They are related to the Appointing Authority or
* The employment would result in one of them supervising the work of the other.
Department Head may establish additional limitations on the hiring of relatives by departmental rule.

The San Francisco Health Network is the City's only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers.
Role description
Under general direction, the ClinicalPharmacist performs professional pharmaceutical duties in drug education and utilization, counsels patients on drug usage, educates health care personnel on drug utilization, and may compound drugs and dispense medicines in a clinical pharmacy. The essential functions include:
The essential duties of this position include but are not limited to:
* Interviews and examines patients for drug evaluation; checks on drug adherence and side-effects; discusses symptoms and effects of prescribed drugs with patients and maintains a drug history on patients interviewed.
* Consults with physicians on drug usage and the drug regime of specific patients particularly hypertensive, diabetic, and psychiatric patients; recommends changes in the drug or the dosage prescribed for these patients.
* Conducts both formal and informal training for nursing staff, medical and pharmacy students and residents and other health care staff on drug utilization, dosage, incompatibilities, preferred dosage forms, evidence-based medication therapy and toxicity, etc.
* Participates in the development and revision of medication use policies and guidelines.
* Compounds drugs, prepares, and dispenses medications, checks pharmacy stock, and keeps records.
The 2454 ClinicalPharmacist also performs other related duties as assigned.
Nature of positions: Positions are located throughout the Department of Public Health, including Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG), Laguna Honda Hospital and Rehabilitation Center (LHH), Community Primary Care and Mental Health Clinics, as well as San Francisco County Jails. Some positions may require employees to work directly with individuals within locked jails and psychiatric facilities, the homeless population and/or individuals who have or are carriers of infectious/communicable diseases.
How to qualify
Minimum Qualifications
License: Requires possession of a valid Registered Pharmacist license issued by the California State Board of Pharmacy. (Please attach a copy of license);
* License status must be "Valid and Current" according to search.dca.ca.gov/
AND
Experience: One (1) of the following:
* A. Completion of six (6) months of a pharmacy residency program; OR
* B. Completion of six (6) months of a post-graduate pharmacy fellowship program; OR
* C. Two (2) years of professional level experience as a ClinicalPharmacist (equivalent to class 2454 ClinicalPharmacist within the City and County of San Francisco); OR
* D. Possession of a Board Certification in a pharmacy specialty area.
One year of full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week).

Possible Special Conditions (some positions may require the following):
* Completion of a PGY2 Pharmacy Residency program in emergency medicine, critical care, or toxicology; OR two years of full-time professional level experience as a clinicalpharmacist in an Emergency Department (ED) setting.
* Completion of a PGY2 Pharmacy Residency program in psychiatry; OR two years of full-time professional level experience as a clinicalpharmacist in a psychiatric setting.
* Bilingual fluency in a variety of languages depending on the department's needs. Only those eligibles that pass the bilingual proficiency exam will be considered for bilingual positions.
* The ability to obtain and maintain security clearance with the San Francisco Sheriff's Department.
* Recent and verifiable experience: (1) integrating, supporting and maintaining an Electronic Health Record (EHR) or Pharmacy Information System (PIS); and (2) implementing, developing, and evaluating Computerized Prescriber Order Entry (CPOE), within the last three years.
DESIRED QUALIFICATIONS: The stated desirable qualifications may be used to identify job finalists at the end of the selection process when candidates are referred for hiring.
Acute Care practice:
* Demonstrated experience with the acute care medicine patient population.
* Clinical and/ or lived experience with diverse communities.
* Excellent oral and written communication and interpersonal skills.
Ambulatory Care practice:
* Completion of an ambulatory/primary care pharmacy residency, specialty residency, or fellowship.
* Fluency in Spanish and/or Cantonese is highly desired.
* Mission-driven, passionate candidates with Primary Care clinical expertise and lived experience with Black/African American, Latinx, LGBTQIA+ and other diverse and intersecting communities.
* Meets requirements for prescribing as required by Standardize Procedure for Ambulatory Care Pharmacist within San Francisco Health Network.
Behavioral Health practice:
* Completion of a psychiatric pharmacy residency.
* Clinical experience working in substance use disorder or mental health settings.
* Fluency in Spanish and/or Cantonese is highly desired.
* Mission-driven, passionate candidates with Mental Health / Behavioral Health / Substance Use clinical expertise and lived experience with Black/African American, Latinx, LGBTQIA+ and other diverse and intersecting communities
* Board Certification as a Psychiatric Pharmacist.
* Possession of a valid Advanced Practice Pharmacist license in California.
Long Term Care practice:
* Completion of a geriatric pharmacy residency.
* Certification in Geriatric Pharmacy.
