Position Summary: The Clinical Pharmacist works with patients with chronic conditions and the primary care clinician team, to ensure optimal results from drug therapy for these chronic conditions, and to support patients in taking more of an active role in medication and healthcare self-management; as well as to assist clinicians in providing individualized patient care for patients.
Duties and Responsibilities:
Manage medication therapy for patients served at SCC including but not limited to Diabetic, Hypertension, Hyperlipidemia, in collaboration with patients' primary care providers and following SCC protocols.
Review medical records in OCHIN-EPIC and other systems (Pharmacy software) as needed; Identify qualifying and co-morbid chronic medical conditions.
Evaluate treatment regimen and identify potential drug related problems.
Adjust treatment plan based on identified potential drug related problems, and medical conditions.
Assess and monitor the patient's knowledge of medications/disease, patient's perceived medication-management issues, medication adherence, adverse effects, drug interactions and lifestyle modifications.
Order appropriate laboratory test(s) related to medication therapy.
Initiate/ adjust refill/ discontinue medication(s) per established protocols.
Monitor for adverse effects, complications, and therapeutic outcomes.
Provide and reinforce patient education
Respond to medication refill requests following SCC Medication refill protocol Clinical.
Review and evaluate treatment plans by Saban Community Clinic providers at all 4 clinic sites and make recommendations to providers when appropriate.
Sort, review, and respond to requests for medication refills as per protocol approved by SCC,
assist with approving alternate medication requests for ones that are not covered by insurance, assist with prior authorizations, and requests for new medications.
Approve, adjust dosing, deny refill requests according to departmental protocols approved by Saban providers.
Suggest formulary substitutions for medications.
Order appropriate laboratory tests as indicated.
Troubleshoots transmission errors in e-prescribing/e-Scripts.
Assist in coordinating patient appointments when applicable to medication refills.
Responsible for serving patients of the SCC pharmacist when coverage is needed
Accurately filling prescription orders and managing the dispensing function, consulting with patients and prescribers about medication regimens, providing pharmacist care and performing patient consultations on drug therapies.
Provide appropriate support to pharmacy technicians including directing technicians' workflow and verifying their order entries, preparations and labeling of pharmaceuticals.
Actively participate in pharmacy purchases and inventory management in compliance with CA Pharmacy Laws.
Ensure accurate electronic billing of medications dispensed to insured patients, according to PBM contracts.
Will be required to precept pharmacy students and residents through Saban Community Clinic's collaboration with pharmacy schools and residency programs.
Position Requirements:
A Doctor of Pharmacy (Pharm.D.) degree (Required)
PGY1(Community or Ambulatory) residency training or equivalent clinical experience (One to two years prior experience in a similar position providing Medication Therapy Management in a community clinic setting, particularly working with patients with Diabetes, hypertension, and hyperlipidemia (Required)
California Advanced Practice Pharmacist License, and or board certification (Highly Preferred)
Bilingual in Spanish preferred.

THE MISSION:

Role Description
We are currently looking for a Pharmacist at Janna Pharmacy LLC. The Pharmacist will be responsible for overseeing daily pharmacy operations, ensuring regulatory compliance, managing inventory, providing patient care, and collaborating with the healthcare team. This role requires working onsite in Torrance, California.
Duties and Responsibilities:
You will be primarily responsible for:
Dispensing prescribed medications in accordance with all applicable laws and regulations;
Providing direction related to quality, clinical and compliance for Apotheco operations initiatives in compliance with state and federal laws
Providing consultation and other pharmacy services to patients
Providing supervision of Apotheco' s support staff; and

Education/Experience:
Have earned a BS or Pharm D degree from an accredited pharmacy school.
License/Certification:

OVERVIEW
Practices Pharmacy in a community setting with clinical functions. Responsible for day-to-day pharmacist duties.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Prescription processing and verification, coordination of prescription delivery, insurance claim transmission issue resolution, and workflow maintenance.

8. Oversee pharmacy operations through each respective pharmacy team member, ensuring compliance with State and Federal laws, as well as conformity with OptioRx Policies and Procedures.
9. Assessing customer feedback and using your creativity to establish, improve, and refine services.
10. Resolve pharmacy patient concerns and complaints.

Qualifications
· Education and/or Training:
a. Bachelor of Science in Pharmacy or Doctor of Pharmacy Degree Pharmacy
b. HQAA/ACHC or similar accreditation training a plus
· Professional Experience:
a. Experience in customer service, leadership
b. Ability to balance multiple projects and adapt to new issues/assignments as business needs change
c. Operates with the highest level of ethics, compliance, professionalism and integrity
d. Experience in core elements of pharmacy and business operations, including purchasing, customer service, financial management, operational excellence, clinical programs, and inventory control
e. Experience in hospice and long term care pharmacy
f. Experience hiring, training, and developing teams
· Licenses/Certifications:
a. Valid and active pharmacist license in corresponding state(s)
· Specialized Skills:
a. 5+ year experience in the hospice and long-term care environment
b. Previous experience collaborating with hospice and assisted living facilities, fostering and maintaining relationships with DoN, medical director, and nursing team
c. Clinical understanding of palliative care, hospice assessment tools, and treatment formularies
d. Understanding of the hospice, assisted living, group home and other alternative long-term care industry sector preferred
e. Experience with IDT meetings and partnering with Hospice facilities on licensing and compliance. Extensive knowledge in third party billing (Medicare, Medicaid, commercial PBMs) and corresponding residence code, pharmacy service type, diagnosis code, etc. and ability to interpret classification for non-covered services and products
f. Ability to organize and plan the activities of subordinates, and to delegate appropriately
g. Organizational skills sufficient to maintain consistently accurate records
h. Ability to evaluate options and to make efficient decisions
· Technical Skills:
a. High degree of proficiency MS Office Suite (Excel, Word, PPT), Outlook & Internet applications
b. Must have excellent communication skills, both written and verbal, and experience with communication at an executive level.
c. Strong analytical, prioritizing, interpersonal, problem solving, and planning skills

Responsible for safe, effective and timely medication use throughout the medical center. Procurement, storage, preparation and administration of all medications are under the supervision of the Pharmacist. The pharmacist ensures optimal medication use throughout the entire medical center and across all ages of the patient population.
