Role Overview:

Requirements:
Doctorate: Graduation from an accredited school of Pharmacy.

CERTIFICATION & LICENSURE:
PHARMD-Licensed Pharmacist
OR PHARMR-Registered Pharmacist
ACLS-Advanced Cardiac Life Support (for those who will be responding to codes)

Basic Qualifications
BS in Pharmacy or Pharmacist Degree from an accredited educational institution.
Current pharmacist licensure in the states within the district.

Major Duties
Oncology Formulary Management and Utilization:
Manage the oncology formulary by evaluating and onboarding new agents.
Determine the appropriate site of care for patients receiving oncology treatment, considering factors such as inpatient, outpatient, or regional campuses.
Oversee the review process for off-label, non-formulary, or other non-standard use of antineoplastic agents.
Promote appropriate, standardized and guideline-based antineoplastic utilization across all El Camino Health (ECH) sites where appropriate.
Review provider requests for exception orders and evaluate for literature support to guide the prior authorization process when applicable.
Collaborate with relevant stakeholders, including physicians and other healthcare professionals to ensure the safe and effective use of these agents.
Collaboration with Pharmacy Informatics Teams:
Work closely with pharmacy informatics teams to contribute to the development and revision of Electronic Health Records (EHR) resources.
Ensure that EHR resources support the safe and effective use of oncology medications and adhere to current guidelines and best practices.
Education:
Coordinate, facilitate, and provide education, training, competency assessment and performance improvement to pharmacists, pharmacy residents and other healthcare professionals for the appropriate use of antineoplastics as well as on best practices and the latest developments in oncology pharmacy.
Collaborate with other disciplines to develop and implement best practices across the institute related to antineoplastic utilization.
Quality and Performance Improvement:
Contribute to the institution's antineoplastic stewardship program through compiling data and metrics on antineoplastic utilization in the outpatient and inpatient settings, reporting data and findings to relevant stakeholders.
Monitor antineoplastic utilization practices to identify areas for improvement and implement necessary changes.
Build and maintain databases and dashboards that support the work of the Antineoplastic Stewardship Program as required.
Collaborate with the Department of Quality and Patient Safety in the assessment, mitigation and dissemination of antineoplastic-related safety initiatives.
Stay updated on the latest advancements in oncology pharmacy and ensure that the pharmacy services align with current standards of care.
Contribute to the development and maintenance of policies and procedures to support the safe use of antineoplastic medications across all ECH sites and promote and enhance patient outcomes through safe, effective, cost-effective, and appropriate medication therapy use for patients receiving antineoplastic therapy.
Qualifications - All
Doctorate degree pharmacy (Pharm.D.) from an ACPE-accredited college of pharmacy.
A one (1) year accredited PGY1 (Post Graduate Year) pharmacy residency.
A one (1) year PGY2 Pharmacy Residency in Oncology/Hematology OR BCOP certification with minimum of 2 years of oncology experience that includes pharmacy regulatory compliance, compiling medication use evaluation (MUE) data or equivalent quality assurance (QA) data for Pharmacy and Therapeutics (P&T) Committee.

Job Description
Under general direction, the ClinicalPharmacist performs professional pharmaceutical duties in drug education and utilization, counsels patients on drug usage, educates health care personnel on drug utilization, and may compound drugs and dispense medicines in a clinical pharmacy. The essential functions include:
The essential duties of this position include but are not limited to:
Interviews and examines patients for drug evaluation; checks on drug adherence and side-effects; discusses symptoms and effects of prescribed drugs with patients and maintains a drug history on patients interviewed.
Consults with physicians on drug usage and the drug regime of specific patients particularly hypertensive, diabetic, and psychiatric patients; recommends changes in the drug or the dosage prescribed for these patients.
Conducts both formal and informal training for nursing staff, medical and pharmacy students and residents and other health care staff on drug utilization, dosage, incompatibilities, preferred dosage forms, evidence-based medication therapy and toxicity, etc.
Participates in the development and revision of medication use policies and guidelines.
Compounds drugs, prepares, and dispenses medications, checks pharmacy stock, and keeps records.
The 2454 ClinicalPharmacist also performs other related duties as assigned.
Nature of positions:
Positions are located throughout the Department of Public Health, including Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG), Laguna Honda Hospital and Rehabilitation Center (LHH), Community Primary Care and Mental Health Clinics, as well as San Francisco County Jails. ClinicalPharmacist Core Job Duties:
Performs duties as a ClinicalPharmacist in direct support of the provision of healthcare to patients admitted to the inpatient wards and intensive care units of the hospital in addition to outpatient clinics.
The Antimicrobial Stewardship/Infectious Diseases ClinicalPharmacist works as a part of a multidisciplinary team and co-leads the Antimicrobial Stewardship Program (ASP) with the ASP Medical Director. Monitors and manages in the development, implementation, and monitoring of the Antimicrobial Stewardship Program (ASP). The role of the Infectious Disease ClinicalPharmacist is to support the facility in a variety of pharmacy benefit management functions that provide integrated support to the direct care and purchased care components within the DoD pharmacy benefit. Incumbent serves as the expert Subject Matter Consultant (SMC) for drug therapy and pharmacy service issues. Accomplishes special quality assurance studies. Performs pharmacoeconomic analyses. Provides advice, professional consultation/opinions and written technical studies and professional reviews in support of the various boards and committees within DGMC involved with evaluation of patient care and treatment. Job Description
Under general direction, the ClinicalPharmacist performs professional pharmaceutical duties in drug education and utilization, counsels patients on drug usage, educates health care personnel on drug utilization, and may compound drugs and dispense medicines in a clinical pharmacy.
