    What You'll Do The Clinical Pharmacist will play a critical role in executing Suvida's Healthcare model to strengthen the provider-patient relationship and engagement by enhancing the quality and affordability of care. This individual will focus on supporting providers and patients through medication management support, pharmacist-led clinical programs, and practice transformation. You will be responsible for optimizing medication therapy to improve clinical outcomes such as blood pressure and A1c control, reduce preventable ED and hospital utilization, and monitor adherence to medication regimens and care plans. The clinical pharmacist meets the patient where their needs are (virtually, in-office, and at-home) to identify, resolve, and prevent medication-related problems. As time allows, the pharmacist will also participate in various committees and initiatives (e.g. program development for chronic diseases (i.e. COPD, DM, CHF), HEDIS strategies to close gaps, clinic operations, etc.).
