The contract Health Care Provider (HCP) will work in conjunction with other health care providers, professionals, and non-contract personnel. The Contractor HCP shall provide the personal services of Clinical Pharmacist to perform services of the nature and scope described below. The quality of care provided by the Contractor HCP shall be of a quality meeting or exceeding currently recognized national standards as established by the Joint Commission (JC). Shall have minimum of one year of successful clinical practice within the past three years. The time spent completing a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy may serve to meet this requirement. d. The Contract Clinical Pharmacist performing under this Contract shall be able to read, write, speak and understand English well enough to effectively communicate with all patients and other health care providers. e. Shall have and maintain current certification in Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS). The on-call rate is only for the contract HCP to carry the pager and be available to answer inquiries from patients and staff and/or report to the facility to see patients. Once the contract HCP arrives at the facility to see patients, then the hourly rate will apply, and the on-call rate will no longer apply.
