Primary Responsibilities:

The hourly range for this role is $43.17 to $84.95 per hour based on full-time employment. ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
Conduct targeted outreach to patients identified as at risk for non-adherence to chronic medications, focusing on CMS quality measure drug classes (e.g., diabetes, hypertension, cholesterol) and other high-priority therapeutic areas.
Review and verify prescriptions and medication order for appropriateness, including safety and efficacy.
Operate under a collaborative practice agreement to adjust medication therapy, order and interpret lab results, and manage chronic disease regimens to support adherence and clinical goals.
Perform comprehensive medication reviews, addressing cost, access, side effects, and complexity barriers to adherence
Collaborate with providers to implement care plans, streamline prescription workflows, and ensure timely refills.
Track and report adherence-related KPIs, including Proportion of Days Covered (PDC), polypharmacy measures, and specialty adherence metrics.
Provide education to patients, caregivers, and clinic staff on the importance of medication adherence
Represent the pharmacy department on relevant committees, payor calls,
quality teams, and market strategy groups within the market
Assist the department in addressing physician consultations as needed. Provide timely retrieval of reliable drug information from available sources.
Provide verbal and written consultations concerning medication therapy on a daily basis.
Assisting the Director to provide effective and efficient use of all resources and for ensuring budget and value proposition targets are met.

Position Summary:
* The System Antimicrobial Stewardship - Infectious Diseases Pharmacist (ASP-ID) is a clinical pharmacy specialist and leader responsible for the development, implementation, and documentation of the antimicrobial stewardship program (ASP) across Rush University Health System (Rush). This includes providing multidisciplinary education on safe antimicrobial use, creating and implementing guidelines, collaborating with multidisciplinary clinicians, administrators, and support staff across the system, and analyzing metrics for antimicrobial utilization to satisfy new and existing CDC and Joint Commission standards. The ASP-ID pharmacist will be responsible for developing and coordinating innovative clinical pharmacy programs across RUSH sites conducting scholarly and research activities and interdisciplinary teaching. To support the effectiveness of ASP initiatives, the ASP-ID pharmacist will maintain on-site service coverage rounding with inpatient ID team(s). The ASP-ID pharmacist will serve as a key faculty member in postgraduate pharmacy training programs and affiliated colleges of pharmacy. This position will focus their efforts on ASP support at Rush Oak Park hospital and Rush Copley Medical Center.
Essential Clinical Duties and Responsibilities of the ASP-ID Pharmacist:
* Develops metrics for performance and maintenance of ASP.
* Participates in the provision of pharmaceutical patient care through ASP staffing and clinical rounds.
* Participates in restricted/protected antimicrobial approval.
* Participate in ASP coordinated education of healthcare professionals across RUSH sites and external partners.
* Serves as a liaison to other healthcare providers in the ASP. Coordinate the implementation of identified ASP initiatives while assisting with maintaining relevant required regulatory standards.
* Assists with the strategic implementation of Department of Pharmacy and RUSH quality initiatives.
* Serve as a core preceptor for postgraduate year 1 and 2 (PGY1 and PGY2) pharmacy residents and students.
* Document clinical interventions in EPIC.
* Participate in the Antimicrobial Stewardship Subcommittee of the System Pharmacy and Therapeutics Committee.
* Complete medication use evaluations (MUE) and new drug and therapeutic class reviews for P&T as assigned.
* Provide pharmacy representation in multidisciplinary committees, when appropriate.
Additional Clinical/Operational Responsibilities include:
* Evaluate, clarify, and verify EPIC orders for therapeutic appropriateness, when necessary.
* Serve as content expert for ID related drug shortages.

* Maintain working knowledge of medical center and departmental policies and procedure.

POSITION SUMMARY:

EDUCATION:

Job Summary:

Maintain resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including OTC and home remedies)
Assist in the proper ordering, handling, receiving, documentation, and storage of all controlled substances; assist in maintaining a perpetual inventory for schedule II drugs
Work with appropriate pharmacy technician in reconciling Controlled Substances perpetual inventory as required by management for compliance with state and federal regulations
Provide clinical expertise by responding to drug-related questions and concerns initiated by internal staff and external customers
Document all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record
Preparation and/or supervision of all sterile IV drug products or solutions
Provide evening, weekend, and holiday coverage during rotation and as required by management
Follow all applicable government regulations including HIPAA
Other duties as assigned; Job duties may vary depending on business needs
