The Pharmacist Clinical will be responsible for managing the selection and utilization of pharmaceuticals and supports core clinical programs such as DUR and formulary management.
How you will make an impact:
* Leads Medicaid formulary management processes.
* Responsible for all member, stakeholder, and provider communications regarding formulary changes.

Minimum Requirements:
* Requires BA/BS in Pharmacy.
* Minimum of 2 years of managed care pharmacy (PBM) experience or residency in lieu of work experience; or any combination of education and experience, which would provide an equivalent background.
* Requires a registered pharmacist.
* Must possess an active unrestricted state license to practice pharmacy as a Registered Pharmacist (RPh).
Preferred Qualifications, Skills, and Experiences:
* PharmD strongly preferred.
* Formulary management experience in Medicaid strongly preferred.
* Strong oral, written, and interpersonal communication skills, problem-solving skills, facilitation skills, and analytical skills.
* Proficiency in Microsoft Excel desired.
* Experience with Python, SAS, or SQL preferred.

Essential Duties and Responsibilities

Adheres to organization policies and procedures.
Oversees all day-to-day activities in absence of Pharmacy Manager such as:

Monitors the outcomes from drug therapy by collecting, evaluating, and interpreting the results of laboratory data, clinical data other pertinent patient information.
Communicates the results of therapeutic drug monitoring activities to the physician, nursing, and other appropriate persons.

Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
Maintains thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians, and other healthcare professionals.
Attends "morning meetings" and other patient-oriented meetings with nursing personnel.
Participates in the preparation of progress reports for the referring physician on a regular, periodic basis.
Supervises, coordinates and/or participates in the preparation of sterile admixtures, dispenses all pharmaceutical and related supplies and equipment in accordance with federal, state, and local laws and standards of practice which govern the profession of pharmacy.

Position Summary
The Pharmacist will manage healthcare of TFHC's high-risk patient population through a collaborative approach to patient care management between the clinical and pharmacy departments.
Essential Duties & Functions
Assure safe, effective, and cost-effective use of drugs
Maintains accuracy in age specific issues
Strictly follows all HIPAA and accreditation agency guidelines; assures compliance with Tampa Family Health Center policy and procedure with quality related event disclosure(s)
Maintains currency in Florida Board of Pharmacy laws, rules, and regulations; maintains strict record keeping
Provide drug information to clinicians and patients
Strict compliance with formulary management and therapeutic substitution initiatives
Assure ongoing improvements in therapy and systems through collaborative problem solving, research and participation in facility and department organizational performance initiatives
Maintain a working knowledge of the 340(b)-drug program
Direct supervision of Pharmacy Technicians, including training and development as well oversite and monitoring of daily activities, monitoring of refrigerator temperatures and expired meds
Assist in developing and maintaining quality improvement programs, clinical services, an effective formulary, protocols, guidelines, policy, and procedure
Participates in multi-disciplinary and pharmacy committees, as deemed appropriate by CPO
Assist in Adverse Drug Reaction and Medication Error reporting
Participate in pharmacy accreditation and audit preparation and process
Responsible for maintaining the operations of pharmacy location, including daily cash and inventory and supplies; ensure workspaces and surrounding areas are clean
Responsible for maintaining and integrating educational updates on CMS regulations (i.e., fraud, waste, and abuse); currency in developments in the community and pharmaceutical fields
Required Education, Certifications, Licenses, & Training (minimum requirements)
Active and unencumbered license to practice pharmacy in the State of Florida
Current BLS Certification
Obtain a Statement of Completion Certificate from 340B UNIVERSITY ONDEMAND during first 2 weeks of employment
Complete annually - 340B University renewal course, Peer Review as well as Fraud, Waste and Abuse training
Maintain a valid BLS card (renew every 2 years)
Certified to immunize required

Job Summary
Working in a hospital system pharmacy outsourced to Cardinal Health, the Pharmacist II adjusts and dispenses medication to patients and drives the compliant and efficient operations of the pharmacy. The Pharmacist verifies physician orders using the pharmacy information system, makes adjustments according to protocols for a given medication, and provides patient education. This job also supervises Pharmacy Technicians and manages compliance tasks related to accreditation, narcotics control, and inventory management.
Responsibilities
Applies expert knowledge of drugs and related protocols, to verify physician orders for medications using pharmacy information system. Checks drug and dose in relation to stated patient issue and addresses automated alerts from the system regarding drug type, dose, and other metrics.
Supervises Pharmacy Technicians to ensure the compliant and efficient operations of the pharmacy. Checks all work of Pharmacy Technicians in line with State Boards of Pharmacy regulations and provides guidance on complex prescriptions, pharmacy automation systems, orders, and other issues.
Adjusts dosages based on established protocols for each drug, ensuring that dosages are appropriate for the patient's condition and in light of other medications the patient is using and completes clinical tasks related to antimicrobial stewardship.
Educates patients on the proper use and expected effects of medications, especially when patients are using multiple medications, are beginning new regiments, or are being discharged.
Manages narcotics and inventory control and compliance, including regular documentation and completion of checklists. Audits outdated medication in states where Pharmacy Technicians are not permitted to do so.
Contacts Nurses and Physicians as necessary to verify orders and obtain additional information regarding the order.
Assists the Pharmacy Director in maintaining the pharmacy's accreditation and in implementing operational changes within the pharmacy, such as the use of new systems or adjustments of standard procedures.
Qualifications
Licensed, “good standing” pharmacist in the state of Florida required
In-patient hospital pharmacy experience preferred
Flexibility to work various shifts and on-call as needed
IV experience preferred
Ability to hear, write, and speak clearly in order to communicate with customers and health care professionals
Strong customer service skills.
Manual dexterity required for occasional reach, lifting and holding of small objects.
Computer savvy to learn and perform successful operation of multiple pharmacy information systems.
