Basic Qualifications

Job Summary
The ClinicalPharmacist plays a key role in advancing population health initiatives and optimizing performance across value-based care (VBC) programs. This role will support community health centers (CHCs) and contribute to clinical programming, quality improvement, and risk adjustment initiatives within the Clinically Integrated Network (CIN), Medicare Shared Savings Program, Medicare Advantage, and other VBC contracts.

What You'll Do
Chronic Disease Management:
Provide clinical oversight for medication management for patients with diabetes and hypertension.
Optimize medication regimens based on clinical guidelines, patient-specific factors, and barriers to adherence.
Conduct comprehensive medication reviews, including identification of polypharmacy risks, drug interactions, and therapeutic duplication.
Support clinical teams in medication titration protocols under collaborative practice agreements, where applicable.
Consult with providers on complex medication regimens and serve as a clinical resource on chronic disease pharmacotherapy.
Remote Patient Monitoring:
Actively manage patients enrolled in RPM by reviewing blood pressure and glucose readings, conducting patient outreach to address abnormal values, providing education and coaching, and supporting medication changes as needed.
Establish medication adjustment workflows aligned with RPM data and escalation protocols.
Review RPM dashboards and trends to proactively identify patients needing intervention.
Collaborate with care teams and providers to act on RPM alerts and abnormal readings.
Support documentation and billing workflows related to RPM services.
Medication Adherence and Pharmacy Technician Collaboration:
Oversee pharmacy technician workflows supporting medication adherence calls, refill alignment, outreach, and education.
Design, implement, and refine adherence strategies (e.g., refill sync, cost mitigation, side-effect counseling).
Prioritize patients for adherence support using pharmacy claims data, risk stratification lists, and payer adherence thresholds.
Ensure adherence programs are aligned with Stars, HEDIS, and quality contract expectations.
Medicare Advantage, Stars & HEDIS Quality Program Support:
Understand medication-related quality measures (i.e., Statin Use in Persons with Diabetes, Diabetes Control, Controlling Blood Pressure).
Identify performance gaps and partner with clinical and network teams to design pharmacist-led interventions that improve quality metrics.
Support documentation improvement and close care gaps through chart review, patient outreach, and provider education.
Participate in payer quality meetings, program evaluations, and reporting cycles as needed.
Data, Reporting, & Quality Improvement:
Use clinical, claims, and RPM data to identify trends, gaps, and opportunities for improvement.
Develop and track outcome metrics related to medication optimization, adherence improvements, and chronic disease control.
Contribute to dashboards, scorecards, performance reviews, and reporting for internal and external stakeholders.
Participate in quality improvement (QI) projects and support the design of scalable pharmacist-driven interventions.
Interprofessional Collaboration:
Work closely with physicians, advanced practice providers, care managers, and medical assistants to coordinate patient care.
Provide education on pharmacotherapy best practices, new medications, and guideline updates.

Pharmacist - PRN
Responsible for all operational aspects of the AllHealth Network Pharmacy focusing on customer service, quality, profitability and inventory management.
Maintain compliance with the Colorado Board of Pharmacy regulations.
Direct and monitor the daily work of Pharmacy Technicians.
Cross-train on different pharmacy processes
Develop and apply upgraded computer/technical skills that enable accurate and efficient dispensing/counseling processes
Develop, enhance, and/or maintain programs/services based on community-specific patient needs
Use appropriate technique(s) and clinical skills to accurately and effectively administer medications, including immunizations
Use effective communications skills when providing patient education (e.g., patient education materials, verbal education, and training) designed to enhance patient understanding and appropriate use of medications
Identify and research ways to implement new pharmacy process improvements
Ensure that the AllHealth Network Pharmacy provides quality care to patients including prescription information and assistance
Ensure strict pharmacy compliance with HIPAA regulations and other laws and standards related to the protection of confidential patient information
Receive, interpret, and prepare prescription fills and refills accurately in compliance with federal, state, and local laws and regulations
Complete all required paperwork legibly, by established time frames, and to accepted standards of practice
Ensure proper maintenance of all Pharmacy patient records as well as proper disposal of confidential consumer information
Assist with ordering, receiving and stocking of merchandise, ensuring proper stock rotation and inventory control
Answer questions and provide information to pharmacy customers on drug interactions, side effects, dosage, and storage of pharmaceuticals, if counseling offer is accepted
Maintain established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs
Maintain disease and treatment knowledge via current literature and clinical practice guidelines
Maintain awareness of third-party Rx systems and resolve third party rejection problems promptly, resubmitting rejected claims for payment on a timely basis
Report prescription errors immediately and adhere to company policy regarding medication incidents
Perform other duties as required within the scope of the position and the experience, education including, but not limited to:

May be selected to be the infection control officer
Qualifications:

Clinical pharmacist job in Denver, CO
This position demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient and cost-effective pharmaceutical care within the scope of their assigned practice. This position is responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team.
**Essential Functions**
+ Reviews patients' medication order for allergy interactions, duplication of therapy, appropriateness of drug therapy, and drug- drug interaction. Makes appropriate evidence-based interventions to optimize medication use. Ensures safe, appropriate, cost-effective therapies and accurately dispenses medications.
+ Oversees and/or participates in assuring appropriate medications are prepared and delivered timely to care areas and are stored both securely and appropriate for both stability and effective workflow.
+ Provides counseling to patients and promotes quality medication education and adherence. Ensures patient is well informed, treated with respect, and receives the highest level of professional service.
+ Completes all regulatory and compliance training and ensures documentation of regulatory requirements is clear and complete. Is knowledgeable and compliant with state and federal laws, regulations, pharmacy policies, procedures, and standard operating procedures.
+ Supervises and directs activities of interns, technicians, and support personnel as required. Provides medication related clinical consultation and clarification to other health professionals.
+ Takes the initiative to solve problems including, but not limited to, equipment, maintenance, drug availability, staffing issues, and extraordinary patient care concerns.
+ Identifies potential areas for improvement including processes that could be streamlined or revised to improve patient satisfaction. Identifies potential safety hazards that may create problems for patients and/or staff and takes appropriate actions. Participates in departmental meetings, contributing ideas or feedback
+ Supports clinic or hospital and system directives, including clinical, compliance, business operations, standardization, and customer service.

+ Entry Rate: Depending on experience

+ Shift Details: **This is a PRN position.** The clinic is staffed by pharmacists between the hours of 7A-4:30P currently (hours may change as required to support the infusion clinic). The shifts are typically 7-3:30 or 8-4:30. We are open Monday through Friday, with no holidays and no weekends.

**Qualifications**
+ Minimum
+ Doctor of Pharmacy or Bachelor of Science in Pharmacy degree from an accredited institution (education verified).
+ License to practice pharmacy in the state of practice and dispense controlled substances
+ Current knowledge of state and federal regulations and statutes regarding the practice of pharmacy in the state of practice.
+ Effective communication, people management, and project management skills.
+ Immunization certification if applicable to practice site needs
+ **O** **ncology Infusion experience** - Highly Preferred

+ Ongoing need for employee to see and read information, labels, documents, monitors, identify equipment and supplies, and be able to assess customer needs.
+ Frequent interactions with providers, colleagues, customers, patients/clients and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
+ Manual dexterity of hands and fingers to manipulate complex and delicate supplies and equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.
+ For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
+ Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
+ May be expected to stand in a stationary position for an extended period of time.
