Responsibilities:
Review batch records and documentation for accuracy, completeness, and compliance with SOPs and cGMP.

Participate in deviation investigations, corrective and preventive actions (CAPA), and change control processes.
Assist in internal and external audit preparation and responses.
Verify labeling, packaging, and final product release documentation.
Complete annual product reviews.
Submit biannual reporting to FDA for compounded products.
Obtain and maintain pharmacist licensure in multiple states.

Basic Qualifications

JOB SUMMARY
The Oncology ClinicalPharmacist has a focused approach in working with OhioHealth's multi-disciplinary oncology care teams. Primary responsibilities include:
* Promoting evidence and value-based oncology & supportive medication therapy through close participation in the oncology medication therapy decision-making process with the multi-disciplinary care team.
* Recommending modifications and improvements in oncology & supportive medication therapy based on information obtained through patient interviews, objective monitoring parameters and the patient medical record.
* Modifying chemotherapy plans in the electronic health record (EHR), in collaboration with the multi-disciplinary team, to ensure plans are accurate for patient's age and performance status, pre-medications are appropriate for patient-specific factors and relevant drug interactions have been identified and addressed.
* Communication and documentation of findings and pharmacist interventions with providers and providing appropriate alternatives to current treatment plans as needed.
* Position may include optimization of patient medication adherence through counseling, education, and application of motivational interviewing skills. Documentation of patient education at therapy initiation and follow-up, including review of oncology & supportive medication effectiveness, side effects, lab monitoring, expected outcomes of therapy.
* Understanding of Specialty Pharmacy Benefit Managers (PBMs) and formulary requirements. Identifying and recommending alternative medications for requests for non-formulary specialty drugs required by insurances.
* Providing experiential instruction to Doctor of Pharmacy Students and OhioHealth pharmacy residents.
Responsibilities And Duties:
50% - Reviews physician orders for correct drug choice, dosage form, dosing schedule, and possible drug-drug interactions or allergies. Contacts physicians to clarify orders as appropriate. Dispenses and compounds medications and dispenses drug therapy-related devises in accordance to standards of practice and OhioHealth pharmacy policies. Ensures all medications, chemicals and supplies are dispensed without suboptimal integrity. Ensures appropriate labeling for proper administration of medication. Interprets prescriptions and determines compatibility in accordance with standards of practice. Prepares care plan for patient to ensure optimal therapeutic outcomes and maximize compliance with therapy instructions.
20% - Manages individual's drug therapy by initiating, monitoring and modifying prescription issued to patient in a consulting relationship with a physician. Provides drug information to physicians, other health care professionals, and patients as needed.
10% - Counsels individuals with regard to their drug therapy by recommending drug therapy related devices and assisting in the selection of non-prescription drugs and appliances for the treatment of common diseases and injuries and providing instruction in their proper use.
10% - Assists with staff development and training. Provides educational programs for pharmacy, nursing, and medical staff as needed.

Job Summary:

Mount Carmel St. Ann's is seeking a Contingent Patient Care Pharmacist to join our dynamic pharmacy team. This flexible role offers the opportunity to work across both operational and clinical areas, supporting patient care through medication management, order verification, and interdisciplinary collaboration. The pharmacist will rotate through various shifts based on departmental needs, gaining exposure to diverse clinical settings and workflows. Preferred candidates will have completed a PGY1 Pharmacy Residency or possess equivalent inpatient hospital experience, demonstrating strong clinical judgment, adaptability, and a commitment to excellence in patient-centered care.
Position Purpose
The Patient Care Pharmacist, within the scope of the OhioPharmacist Practice Act, will provide timely, accurate, cost effective, and patient-centric pharmaceutical care.
What you will do:
* Coordinates the preparation, distribution, and dispensing of medication as appropriate in compliance with state and federal regulations and laws, accreditation standards, and departmental policies and procedures; directs and assists pharmacy support staff.
* Maintains personal compliance and assists in maintenance of departmental compliance with continuous regulatory readiness. Also maintains records related to the transactions of the pharmacy, to achieve adequate control and accountability for all drugs, including controlled substances.
* Assists in the management of pharmacy inventory by providing information to pharmacy purchasing, removing expired contaminated or deteriorated stock when detected, helping maintain perpetual inventories, and processing non-formulary requests.
* Interprets, evaluates and verifies all medication orders.
* Collaborates with multidisciplinary team in order to assure optimal and cost effective drug therapy. This includes adjusting drug dosing and schedules, recommending the most efficacious therapy, practicing stewardship in antibiotics, performing drug consults and monitoring, and detecting possible adverse drug reactions, food/drug interactions, drug/drug interactions, and allergic cross reactivity.
Minimum Qualifications:
* Education: Bachelor or Doctor of Pharmacy required
* Licensure / Certification: Current license to practice as a Registered Pharmacist in the State of Ohio.
* Experience: PGY1 residency or three years of institutional pharmacy experience in an advanced clinical setting is preferred
* Effective Communication Skills
* Demonstrates the ability to plan, organize and manage pharmaceutical care, including delegating tasks to and providing direction to other members of the pharmacy team
* Performs work independently with minimum supervision, and is effective in a stressful environment

Duties
1. Prepares and assists the Pharmacy team in the preparation of medications by reviewing and interpreting physician orders; weighing, measuring and mixing ingredients; detects therapeutic incompatibilities.
2. Dispenses medication by planning, implementing or maintaining procedures for mixing, packaging, or labeling pharmaceuticals, according to policy and legal requirements to ensure quality, security and proper disposal.
3. Assess the identity, strength or purity of medications.
4. Assists in the maintenance of records, such as pharmacy files, patient profiles, charge system files and inventories, narcotics, or controlled drugs.
5. Controls medications by monitoring drug therapies; advising interventions.
6. Completes pharmacy operational requirements by organizing and directing technicians' work flow; verifying their preparation and labeling of pharmaceuticals; verifying order entries.
7. Manages the workflow processes which may include pharmacist verification, labeling/packaging, quality assurance with barcode verification and drug images and delivery with barcode tote checking.
8. Utilizes computer systems to screen for drug interactions, therapeutic duplication, drug-disease contradictions, drug allergies, geriatric precautions, drug-food precautions and adult min-max dose checking.
9. Provides pharmacological information by answering questions and requests of health care professionals; counseling patents on drug therapies.
10. Develops staff pharmacological knowledge by participating in clinical programs; training pharmacy staff, interns, residents and health care professionals.
11. Complies with state and federal drug laws as regulated by the State Board of Pharmacy, the Drug Enforcement Administration, and the Food and Drug Administration by monitoring unit inspections, maintaining records for controlled substances; removing outdated and damaged drugs from the pharmacy inventory, follows disposal of drugs per drug disposal guidelines.

Hours and compensation potential:
•The positions are full time(need 2) and also need a part time(1) person:
-One location is open Mon-Fri 8am-8pm(1 RPh and 3 techs begin at 7am to get a head start) and 9am-5pm on Saturday & Sunday.
RPh's work 1 Saturday every 4 weeks and 1 Sunday every 12 weeks. They schedule 3 RPh's working on the counter in addition to the RPh manager.
Because of this overlapping RPh coverage they can accommodate an RPh who prefers to work three 12- hour shifts or four 10-hour shifts. In a typical 12-hour weekday at the location there are 4 RPh's and 15-18 technicians.
They fill around 1000 rx's on Mondays, 700-800 rx's on the other weekdays and 250-300 on the weekends.
-One location is open Mon-Fri 9am-7pm, Sat 9am-5pm, closed on Sunday. The RPh works either 9am-5:30pm or 10:30am-7pm and one Saturday every four weeks. There are always 2 RPh's scheduled on weekdays plus a 3rd RPh on Monday's and the busier days of the month.
They have 8 to 10 technicians working daily during the week. They fill 500-600 rx's daily during the week and 150-175 on Saturdays.
•Both locations are closed on Thanksgiving and Christmas. Rotate other holiday so person works one holiday every year or two.
•The range starts at $54.85hr($114K)-$75hr($156K) depends on years of experience. Someone around 10 years experience would be $64.52hr($134K) for example.

Pharm D or Bachelor's degree in pharmacy.
Emergency Department experience preferred
Licensure as a Pharmacist in the State of Ohio.
Epic experience beneficial
Analytic ability to determine drug appropriateness and evaluate drug instructions.
Ability to provide care for a neo-natal, pediatric, adolescent, adult and geriatric patient population.
PALS and ACLS Certification preferred. (training will be provided)
