Principal Responsibilities:
1. Will participate and maintain a culture within The CorvallisClinic that is consistent with the content outlined in the Service and Behavioral Standards document. To this end, employee will be expected to read, have familiarity, and embrace the principles contained within.
2. Work providing direct patient care. Obtain and evaluate patient medication regimens, allergies, and past/current medical history. Design, recommend and monitor patient specific therapeutic regimens with evidence-based medicine. Use collaborative drug therapy management protocols for disease state management. Examples include diabetes, hypertension, hyperlipidemia, and pain management.
3. Participate and support guideline, pathway, and other disease management efforts. Participate in providing drug use feedback to providers using available data.
4. Collaborates with nurse care managers to provide expert advice on prescribing practices for selected patients with complex and unstable disease to improve their health and prevent unnecessary hospital and emergency care costs.
5. Conduct target drug programs or other interventional efforts designed to optimize the use of pharmaceuticals.
6. Professional Practice: Remains current in pharmaceutical regulations and applicable laws and advises The CorvallisClinic to ensure compliance.
7. Perform other duties as assigned.
Education, Licensure and Experience:
1. Bachelor's degree in Pharmacy RPh or a PharmD degree from an accredited college or university required.
2. Oregon Pharmacy licensure required.
3. Pharmacy practice residency or a minimum of three (3) years practice experience in direct patient care required.
4. Experience in an ambulatory care clinicor inpatient setting preferred.
Knowledge and Skills:
1. Ability to communicate well in an interdisciplinary team setting.
2. Lead and train where necessary and accurately represent the standards of pharmaceutical practice.

Qualifications

International Health and Medical Services is looking for an experienced Pharmacist to manage the procurement, dispensing, and monitoring of medications to ensure safe and effective pharmaceutical care for patients. This role includes collaborating with healthcare providers to optimize medication management, provide drug information, and contribute to the overall health and well-being of the patient population.
Key Responsibilities
Responsible for the day-to-day operation of the medication delivery system and assures that the integrity of all medications is maintained and is properly stored.
Responsible for participating in quality assurance activities regarding medications to maintain quality patient care and cost-effective medication management.
Responsible for the records and reports related to the purchase, receipt and disposition of drugs and related supplies.
Responsible for the delivery of pharmaceutical care including patient centered, outcomes-orientated pharmacy practice.
Work in concert with the patient and other healthcare providers to promote health; prevent disease; and assess, monitor, initiate and modify medication use to ensure that drug therapy regimens are safe and effective.
Dispense formulary medications.
Prescriptions are filled at local pharmacies which are part of the Script Care Network (or other designated Pharmacy Benefits Manager).

Qualifications
Basic Requirements/Certifications:
Current and unrestricted license to practice pharmacy in the state of Oregon

International Health and Medical Services is looking for an experienced Pharmacist to manage the procurement, dispensing, and monitoring of medications to ensure safe and effective pharmaceutical care for patients. Responsible for participating in quality assurance activities regarding medications to maintain quality patient care and cost-effective medication management.

Qualifications
Basic Requirements/Certifications:
Current and unrestricted license to practice pharmacy in the state of Oregon
