The Lead Home Infusion Pharmacist is responsible to supervise, lead, evaluate, and develop the clinical operations of the IV department while ensuring the highest level of patient care from a clinical perspective. Duties and Responsibilities Clinical and Patient Care Oversight: ● Manage and oversee the clinical functions of the IV department to ensure high-quality patient care. ● Provide direct support to Home Infusion Pharmacists, stepping in as needed to guarantee optimal patient outcomes. ● Efficiently manage both ongoing and newly onboarded patients. ● Review medication orders for accuracy, clinical appropriateness, and safety. ● Conduct and review patient assessments and Care Plans. ● Resolve patient concerns and complaints promptly and effectively. ● Provide patient counseling when necessary. ● Communicate with healthcare providers regarding patient status and treatment adjustments. ● Assume on-call responsibilities when required. ● Perform other duties as assigned by the supervisor.
