The successful candidate will play a critical role in improving patient outcomes through effective medication management, patient education, and collaboration with our healthcare team. Must be able to work Eastern Time Zone hours 8-5pm and have an unrestricted and active practicing pharmacist Fl license. Role Specific Responsibilities (executed in person and/or virtual)
+ Utilize evidence-based medicine and clinical judgment to evaluate the suitability of pharmacotherapy for high-risk patients with chronic conditions, including but not limited to diabetes, hypertension, heart failure and COPD
+ Execute clinical protocols as allowed per organization and state regulatory agencies
+ Optimization of drug coverage benefits for appropriate care and reduced costs (preferred formulary products, lower cost alternatives and home delivery pharmacy)
+ Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care
+ Participate in high-risk rounds with a multi-disciplinary team, utilizing highly effective communication skills to discuss patient progress and adjust treatment plans as necessary
+ Excellent patient counseling skills and the ability to communicate complex medical information to patients and healthcare providers
+ Disease state education for patients on appropriate use of medications and self-management
+ Engage in quality improvement initiatives aimed at enhancing medication safety and efficacy
+ Post-discharge medication reconciliation for high-risk transition of care patients
+ Document all activity within clinic EMR and other required pharmacy or clinical management systems
+ Support CMS Stars and HEDIS quality measures for successful clinic performance
+ Maintain accurate and thorough documentation of clinical interventions and patient care activities
+ Conduct in-service education and training sessions for healthcare staff as well as develop and disseminate educational materials related to pharmacotherapy
+ Stay current with advancements in pharmacotherapy and clinical guidelines.

**Required Qualifications**
+ Active and unrestricted pharmacist license within the state of Florida.
+ Willingness to become licensed in additional states as needed for market support
+ Pharm.D. or other advanced pharmacy degree
+ 5+ years' experience as a practicing pharmacist
+ Excellent organization and communication (verbal and written) skills
+ Ability to work in a team environment and support common goals
+ Strong attention to detail; able to concentrate without distraction in a fast-paced environment
+ Ability to prioritize multiple tasks, anticipate and solve problems
+ Intermediate level or higher competency with MS Office programs such as Word, Excel and Power Point
**Preferred Qualifications**

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. This role includes collaborating with healthcare providers to optimize medication management, provide drug information, and contribute to the overall health and well-being of the patient population.
Key Responsibilities
Responsible for the day-to-day operation of the medication delivery system and assures that the integrity of all medications is maintained and is properly stored.
Responsible for participating in quality assurance activities regarding medications to maintain quality patient care and cost-effective medication management.
Responsible for the records and reports related to the purchase, receipt and disposition of drugs and related supplies.
Responsible for the delivery of pharmaceutical care including patient centered, outcomes-orientated pharmacy practice.
Work in concert with the patient and other healthcare providers to promote health; prevent disease; and assess, monitor, initiate and modify medication use to ensure that drug therapy regimens are safe and effective.
Dispense formulary medications.
Prescriptions are filled at local pharmacies which are part of the Script Care Network (or other designated Pharmacy Benefits Manager).

Qualifications
Basic Requirements/Certifications:
Current and unrestricted license to practice pharmacy in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
American Heart Association (AHA) Basic Life Support (BLS).
Trained in Basic First Aid.

International Health and Medical Services is looking for an experienced Pharmacist to manage the procurement, dispensing, and monitoring of medications to ensure safe and effective pharmaceutical care for patients. This role includes collaborating with healthcare providers to optimize medication management, provide drug information, and contribute to the overall health and well-being of the patient population.
Key Responsibilities
Responsible for the day-to-day operation of the medication delivery system and assures that the integrity of all medications is maintained and is properly stored.
Responsible for participating in quality assurance activities regarding medications to maintain quality patient care and cost-effective medication management.
Responsible for the records and reports related to the purchase, receipt and disposition of drugs and related supplies.
Responsible for the delivery of pharmaceutical care including patient centered, outcomes-orientated pharmacy practice.
Work in concert with the patient and other healthcare providers to promote health; prevent disease; and assess, monitor, initiate and modify medication use to ensure that drug therapy regimens are safe and effective.
Dispense formulary medications.
Prescriptions are filled at local pharmacies which are part of the Script Care Network (or other designated Pharmacy Benefits Manager).

Qualifications
Basic Requirements/Certifications:
Current and unrestricted license to practice pharmacy in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
