Facilitates/participates in P&T subcommittee activities related to practice areas.
4. Actively participates in departmental &/or hospital committees/meetings as medication management authority in specialized practice area. Performs inspections, research, audits, MUE, &/or performance improvement activities in support of clinical practice and measurement of best practices/regulatory standards. Works with medical & nursing staff to enhance patient outcomes, safety, and regulatory standards.
5. Educates and communicates effectively with patients on medication therapies to ensure that patients are informed of proper drug usage and knowledge of potential side effects. Leverages experience with various patient populations, literacy levels, and all available patient information both documented & through patient interview to individualize medication regimens and adherence tools communicated to the patient/caregiver.
6. Provides a lead role in answering complex drug information requests and applies outcomes to general and specific patient situations. Consults scientific publications for practical clinical applications of the data to practice area. Objectively evaluates medical and advertising claims for medication products. May be required to staff or be on-call to respond to patient care medication related questions as the resident expert when needed.
7. Provides mentoring/training/education to pharmacy students, residents, pharmacists, technicians through individual patient cases with more formal education regarding disease or drug therapy, or formal education programs such as CE or staff development programs. In addition, educates providers, nurses, residents, students, patients, and the public regarding drug therapy.
8. Attends, participates, & provides feedback in journal club, in-services, resident topic discussions, and resident/student case presentations
9. Documents interventions for resolved prescribing, formulary & therapeutic issues. Identifies & discusses adverse drug events with the care team & documents safety opportunities. What You'll Do
The Clinical Pharmacist will play a critical role in executing Suvida's Healthcare model to strengthen the provider-patient relationship and engagement by enhancing the quality and affordability of care. This individual will focus on supporting providers and patients through medication management support, pharmacist-led clinical programs, and practice transformation. You will be responsible for optimizing medication therapy to improve clinical outcomes such as blood pressure and A1c control, reduce preventable ED and hospital utilization, and monitor adherence to medication regimens and care plans. The clinical pharmacist meets the patient where their needs are (virtually, in-office, and at-home) to identify, resolve, and prevent medication-related problems. As time allows, the pharmacist will also participate in various committees and initiatives (e.g. program development for chronic diseases (i.e. COPD, DM, CHF), HEDIS strategies to close gaps, clinic operations, etc.).
