    Under the general direction of the Center for Medication Management(s) Manager, provides ambulatory clinical pharmacy care promoting the safe and effective use of drug therapy, according to standards established by the Department of Clinical Pharmacy and guideline supported, evidence-based recommendations to physicians of and patients requiring anticoagulant therapy, diabetes management, transitional care disease state management, vaccinations, preventative wellness services, or referred medication management consults. The pharmacist serves St. Joseph's Candler (SJC) Physician Network as a coordinated effort for ambulatory clinical pharmacy support for SJC. The pharmacist is responsible for working with physicians within the predefined guidelines approved by the P&T Committee for management of patients referred to the Center, and is responsible for working with physicians within primary care settings under the Georgia Board of Pharmacy approved scope of practice to support quality patient care delivery. The pharmacist assists in the development and coordination of clinical pharmacy services and educational programs for above departments and participates in committees as assigned. The pharmacist is responsible for technician and support staff oversight and for developing tools and educational opportunities to enhance services provided by all staff members. Maintain documentation of quality control (QC) tests completed. 4.Document timely and efficiently in eClinicalWorks electronic medical record platform. Apply templates as assigned. 5.Develop and implement clinical pharmacy training programs. Participate in the education and training of technical staff, pharmacy residents, pharmacy interns, and graduate pharmacy students. The ideal candidate is someone who works well in a team environment, detail-oriented, strong communication skills, and problem-solving skills. Essential Duties/Responsibilities: Fulfills all legal and regulatory requirements, including evaluation, selection, procurement and distribution of drugs, counseling and professional consultation Ensures compliance with all state and federal regulations as well as policies of the hospital and PharmD on Demand Interprets physician medication orders Compounds and dispenses prescribed medications and other pharmaceuticals and related products for patient care by performing related duties Utilizes pharmaceutical equipment to include automated dispensing machines, Laminar Flow Hoods and compounding equipment. Represents the hospital and PharmD on Demand in a positive and professional manner Maintains customer confidentiality at all times Complies with all organizational policies regarding ethical business practices Exercises sound judgment in decision-making Interacts effectively with diverse individuals Maintains professionalism in all situations Uses time effectively and efficiently Takes calls from facility and returns to pharmacy as needed Position Requirements: Current pharmacist licensure in the applicable state(s) Preferred experience in retail pharmacy setting Preferred experience using PioneerRx Schedule Details: 30-40 hours a week Top advantages: As a team member at PharmD on Demand, youll enjoy: Comprehensive benefits package Growth opportunities Social events Work-life balance Positive work environment
