Collect, analyze and provide regular written updates on patient outcomes to HJA Leadership. Oversees a continuum of patient care that includes collection of pertinent patient data, drug utilization reviews, and patient counseling. Performs prospective and retrospective drug utilization reviews (DURs); discusses the recommended interventions with the patient and the patient's Primary Care Provider (PCP). Reporting to the Pharmacy Coordinator, the Pharmacist will provide safe and effective pharmacy services in addition to dispensing medications. These services include the provision of pharmaceutical care, medication dispensing, patient counseling, drug information services and patient and staff education. Responsibilities Provide patient education on medication and reason for use, frequency and dosages, side effects, and desired therapeutic effects. Enhance educational experience using drug information flyers or pamphlets appropriate for patient level of health literacy. Fills in-house generated prescriptions in accordance with all federal regulations and all state regulations as per the NJ Board of Pharmacy. Fills outside provider prescriptions in accordance to OHI Outside Prescription Policy in addition to all federal and state regulations as per the NJ Board of Pharmacy. Serves as a resource of information on medications, pharmacokinetics and interactions, to providers. Assist providers in the management of patients with polypharmacy including conducting medication reconciliation, writing medication lists, completing the patient medication card, and providing other means for patients to understand how to take their medications. Works in the CEC periodically to assist providers with requests from pharmacies regarding therapeutic interchanges, questions regarding prescriptions and continuity of care to ensure patients get prescriptions in a timely manner. Work collaboratively with the Coumadin Management Team to evaluate, assess and assist patients with their INR once notified of results by nurse. Then update Coumadin Management Flowsheet as required and renew warfarin prescription to ensure patient has sufficient supply. Works collaboratively with referring primary care providers for the management of smoking cessation patients. Pharmacist will call patients are needed when due for follow-up and ensure patient has sufficient supply of smoking cessation aid. Conduct prospective and retrospective chart reviews on selected patients and discuss findings with providers and/or Chief Medical Officer. Maintains confidentiality of all pertinent information to assure that patient and staff rights are protected. Assist in the identification of potential adverse drug events and in recording such events according to policy and procedures. Perform literature based research and medication formulary reviews. Assist in development of patient education materials and literature that enhances the clinical pharmacy services provided, in language and at health literacy level of the patient population. Maintains a clean and organized pharmacy by assuring all pharmacy personal are held accountable for their workspace. Files complete prescription bundles daily or in a timely manner in an appropriately labeled prescription retention box. Maintains accurate on hand counts of medications by conducting inventory audits. Maintains an appropriate level of pharmacy supplies (vials, bags, bottles, rx labels, etc.). Maintains appropriate inventory of vaccines and medications for in-house use. Notifies Pharmacy Coordinator in a timely manner when vaccines or in house meds are running below par allowing for adequate procurement time. Maintains an appropriate inventory of medications in each OHI pharmacy to minimize out of stocks. Uses prudent judgment when purchasing medications from McKesson wholesale. The pharmacist on duty will always attempt to purchase the least expensive generic medication available (depending on availability). Maintains and updates the pharmacy formulary when needed. Utilizes the Liberty help desk to address any pharmacy software issues that cannot be handled by their immediate supervisor and shares that information with all pharmacy staff. Utilizes PBM help desks to address any 3rd party prescription rejection claims. Designates appropriate tasks and projects to pharmacy technicians and holds techs accountable for completion. Assures pharmacy refrigerator/freezer logs are completed daily and filed accordingly. Compares and rectifies daily McKesson invoices with the automatically generated RXQ (Liberty) order and assures the dollar value is equal. Participate in quality assurance/performance improvement activities, Pharmacy and Therapeutics (P&T) committee and in other committees as assigned. Handles all information regarding the organization in a manner that assures that strictest confidentiality is maintained at all times.
