Basic Qualifications
BS in Pharmacy or Pharmacist Degree from an accredited educational institution.
Current pharmacist licensure in the states within the district.
Experience performing prescription dispensing activities that demonstrate a strong working knowledge of applicable state and federal controlled substance laws.
Certified Immunizer or willing to become an immunizer within 90 days of hire.

**AMBULATORY CARE** **CLINICAL PHARMACIST** **VALUE BASED CARE** Responsible for providing comprehensive medication management services for patients cared for by the Saint Alphonsus Medical Group (SAMG) Clinics and/or patients managed by the Saint Alphonsus Health Alliance (SAHA). Works collaboratively with physicians, advanced practice providers, staff and other health care professionals to provide medication management, education, and care coordination across the health care continuum.
Focuses on supporting patients upon discharge from an acute care and post acute care setting and providing chronic disease state management of high and rising risk patients associated with assigned Saint Alphonsus Medical Group Clinics, through collaborative practice agreements, and with patients of practices in the Saint Alphonsus Health Alliance under the supervision of the Executive Medical Director Value Care.
Is an integral member of the health care team who works to ensure that excellent patient experience, patient safety, best practices and high quality standards of care are maintained across the continuum of care. Responsibilities include assisting the Saint Alphonsus Medical Group Clinic and Alliance network providers and staff in developing and coordinating a wide range of self-management support and chronic-care management activities for the clinic's affected patient population.

Makes recommendations to the appropriate provider if applicable. Completes medication reconciliation of list within E.H.R.
**Patient Education** : Educates identified patients on the management of their disease states and appropriate medication use with emphasis on adherence and identifying/resolving medication related problems. Enhances communication between acute and ambulatory care providers and between specialty care providers when multiple providers may be involved in a patient's care. Provides both verbal and written educational materials depending on patient preference in keeping with appropriate health literacy. Utilizes tools such as Ask Me 3 and Teach Back to ensure patient understanding.
**Staff Development:** Facilitates learning experiences to meet identified learning needs. Supports staff development through sharing of information/educational resources. Educates staff associated with the Saint Alphonsus Medical Groups on medication related issues. Provides medication updates through electronic methods, newsletters and presentations to providers and staff as applicable.
**Leadership:** Provides guidance and direction related to professional practice.
